2. That on, I discovered that the Certificate of Registration ( CR ) of the said

Vehicle was lost without my knowledge that I took effort to locate the same
but to no avail considered it lost forever and beyond recovery;
3. That I am executing this affidavit to attest to the truth of the foregoing facts
and circumstances and to hereby report to proper authorities concerned for
and in the connection for the replacement in lieu of the lost Certificate of
Registration above mentioned, and finally I execute the same for all legal
intents and purposes may served best.
