9.14.1Cell Analysis

The Endothelial Cell Analysis System uses an endothelial cell image captured by the IMAGEnet system.
The image must have a correct scale factor which represents the actual pixel size in the image.

 To start Cell Analysis:

1. Display an endothelial cell image.
2. Select the ‘Cell Analysis’ command from the ‘Tools’ menu. The following window will be displayed:

3. Drag the cross shaped mouse cursor to specify the region to be analyzed, and analysis of the
endothelial cell will automatically start and the image window for the analysis result will be displayed.

The software assumes all cell boundaries are properly corrected by the operator. If the
image to be analyzed contains erroneous boundaries, the image can not be accurately
analyzed. Diagnoses and treatments based on the analysis results shall be made by the
CAUTION
responsibility of pertaining doctors.

107
Icons to be used for boundary lines

To add boundary lines, click this icon once, and then move the mouse cursor to
the image. Press the left button down at one end point of the line to be added,
drag the mouse to the other end point of the line and release the button.
Continue adding lines as needed. This mode will stay current until another edit
icon is chosen.
If erroneous boundaries were found, they may be deleted using this function.
Simply choose the icon, move the mouse cursor to the line to be deleted, and
click the left button once. Continue deleting lines as needed. This mode will stay
current until another edit icon is chosen.
To perform analysis without taking into account certain cells, choose this icon,
and then move the mouse cursor over the cell to be excluded and press the left
button once. The color of the cell will turn blue. To include this cell again, repeat
the procedure. It is not possible to perform this function while the boundary lines
are operated.
To find the area and number of sides for an individual cell, choose this icon, and
then move the mouse cursor over the cell, and press down the left button. The
area and number of sides will be shown in a small box while the button is
pressed.

This function swaps the left (original) and the right (enhanced) images. However,
the boundary lines always stay overlaid on the right image.

This function is enabled only when the icon is pressed. The boundary lines will
be hidden in order to get a better view of the image underneath. The lines will
return when the icon is released.
If there are numerous errors, it may be faster to erase an entire area of lines than
to delete them individually. After choosing this icon, move the mouse cursor to
the upper left corner of the region, then press down the left button. Drag the
mouse to the lower right corner and release the button. This mode will stay
current until another edit icon is chosen.
This function toggles between a zoomed and a normal image. Editing boundaries
with a zoomed image may be easier than with a normal size image.

Icons To Be Used for Analysis

The following shows the map shape as an aberration for each coefficient:

154
(Reference) Point Spread Function (PSF: Point Spread
Function) on retina

PSF is defined as the strength distribution of point images, and in this simulation, represents imaging of
light on a retina. While most ideally, imaging is focused onto a single point, imaging is not centered on a
single point but is observed as spread light even with an ideal lens due to the influence of diffraction and
the like. The ratio of the center strength of an ideal lens to the center strength of the current PSF under
calculation is defined as the Strehl ratio. The Strehl ratio of 1.0 is the best optical value.

(Reference) Space frequency (MTF: Modulation Transfer

Function)

MTF represents the space frequency characteristic of an optical system and may be calculated via Fourier
Transform of PSF. MTF may be thought as an image in which how far you can discern black and white
stripes in a certain direction as they are microscopically viewed.
In case the space frequency is low, good contrast is obtained to ease observation. A higher space
frequency makes observation more difficult. Generally, Modulation Transfer Function in X and Y directions
only is rendered as shown on the left while in this example display extends in all directions covering 180
degrees. Thus, the same value is displayed with respect to the origin.

(Reference) Visibility simulation

One method of simulation of an image seen by an eyeball on the retina is to consider the spread of an
image on the retina. Corresponding simulation may be thought as a convolution integral of the strength of
point images (PSF) and the observed image. In other words, a convolution integral of the PSF on the
retina and the target of a size that may be projected onto the retina is made. Human sight captures a
target image upside down and reversed left to right so that the simulation is displayed symmetrically with
the spread of PSF vertically and horizontally.

155
Expanded display

A double-click on a map provides an expanded display of the map or image. A second double-click
returns to the original screen.

Setting change:
A right-click on a map displays a context menu related to the map thus allowing setting change.

For a three curve contact lens, BC, TD, OZ, SC, SCW, PC, and PCW are necessary. If for a two curve
lens, SC and SCW, or PC and PCW may be omitted. Either SC or PC can be ignored if their width field
(SCW or PCW) is zero.

Handling Each Lens Type

Lens types can be set in the parameter setting dialog box.

 RGP
For a RGP lenses, a total of three curves including a base curve (BC), a secondary curve (SC), and a
peripheral curve (PC), are available. Alternatives (i.e. BC+SC only, or BC+PC only) are also available.

 TORIC
When a TORIC lens type has been selected, the BC2 and PC2 data fields will be activated. There are
basic curve 2 and peripheral curve2, respectively. Listed below are two available lens structures in the
TORIC lens type.
BC (toric) + SC (spheric) + PC (spheric)
BC (toric) + PC (toric)
If the PC value is same as the PC2 value, the contact lens simulation function assumes the lens has
spherical peripheral curves. In this case, the SC, and SCW fields will be activated. However, if PC differs
from PC2, since this is a toric peripheral, the SC and SCW fields become disabled.

 ASPHERIC
When an ASPHERIC lens type has been selected, the E1 and E2 fields will be activated. The meanings of
these fields are eccentricity 1 and 2 respectively. These values are calculated from the corneal
topography, and initially, the two values are the same. One or both of them may be changed according to
the manufacture’s lens specification. For a spherical secondary curve, set E2 equal to 0 to make it
represent a spherical secondary curve. Use the two lens structure as follows:

189
BC ( aspheric ) + SC ( aspheric ) + PC ( spheric )
BC ( aspheric ) + SC ( spheric ) + PC ( spheric )
In both cases, PC can be omitted when PCW is zero.

 SCL
Most SOFT lenses have one BC and One PC, so secondary curve fields are disabled. The fluorescein
simulation is not available for soft lenses.

 PMMA
For a PMMA lens type, the software treats the lens structure in the same way as a RGP lens.

Parameter Setting

When a contact lens simulation report is open or the “Contact Lens Fitting” function from a color map
report has been called for the first time, the parameters may be changed for that report. To get the
parameter setting dialog box menu (Figure 9-9), choose ”Change Parameters” from the [Utility] menu, or
click the parameter icon on the tool bar. The following dialog box will be displayed to change parameters.
(“Select Data Set” item will be appeared only when some contact lens data were registered – See Section
19.4.5):

Figure 9-9 Parameter Setting dialog box

Lens Type
Please refer to 9.16.4Contact Lens Simulation Report Data-Handling Each Lens Type.

Map Type
Absolute map type connects tear layer thickness to an actual fluorescent color. Normalized map type
assigns colors to express the entire thickness range.

Overlays

190
Same as the overlay for the color map report. Refer to 9.16.4Contact Lens Simulation Report Data-
Handling Each Lens Type.

Reference BC
Parameter helps to find a base curve that meets a certain apical clearance demand. The software finds
the radius of curvature of a sphere, which is defined by three points. These points are the center of the
lens with a certain apical clearance, and two optic zone end points, which touch the cornea.

Sagittal Vault
If the sagittal vault value is supposed to be a certain value, input that value. An appropriate TD value will
be calculated for this sagittal vault.

Axial Edge Lift

The required AEL (Axial Edge Lift) value is specified, the software finds the appropriate PC value.

Graph Setting: Profile Graph

The tear layer thickness profile graph is displayed at the specified angle value.

Graph Setting: Isometric Graph

The circular tear layer variation graph is displayed at the specified radius value.

Select Data Set

The pull down scroll list appears when more then one contact lens data sets have been registered. The
software will set the parameters based on the selected contact lens data set.

Image View
The Image View option of the contact lens report has three buttons, Map, Image, and Overlay. The same
setting as for the image view of the color map report is available.

Map: In case this button is ticked, a fluorescein pattern is displayed in the simulation display
area (Figure 9-7).
Image: In case this button is ticked, an image of the subject eye is displayed rather than a
fluorescein pattern in the simulation display area.
Overlay: In case this button is ticked, a fluorescein pattern and an image of the subject eye are
displayed overlapped one on the other in the simulation display area (Figure 9-8).
