Some underlying causes of unsafe acts
➢ Lack of knowledge or training.
➢ Belief that it can not happen to me or it would not happen this time.
➢ A habit.
➢ Unavailability of correct personal protective equipment.
➢ Belief that the practice is acceptable because no one has corrected
it in the past.
➢ An attempt to get attention of the part of group.
➢ An assertion of independence
➢ Sense of priorities that places comfort, production or quality above
safety
➢ A moral problem that reflects conditions, on or off the job.

You can probably think off other underlying causes of unsafe acts.
Of course these underlying causes are not excuses. They simply
help you understand why a person has acted in a certain way.
