1. trxisting Power transformer of FWP Construction site has failed on

fi.qfl.ft 19.01.2019 due to the fault in Power cable dunng excavation work at
vsP site by SMI. Break down report of the same is attached at C 11.
Upon inquiry, it is noticed that the repair of the failed transformer w'i1l
take 'a minimum of 45 days from the date of receipt at
mamufactu r er f r ep atrer's premises.

Affect of B/S-No power supply to RINL- FWP construction site

Observations-1. Transformer PRV supply open at oilgushed out from vent pipe

2.Bulging of transformer body near HT bushings observed

3.HT and LT bushings are intact

4.On preliminary testing,it is found

a)HT winding got open in allthree phases

b)LT winding resistance is not getting stabilized at around 0.9 ohm

c)HT lR values are in the order of 2 giga ohm

d)LT lR values are fourrd ok

5,No protection relay is operated either in Transformer feeder or outgoing feeder

6.Both transformer feeder and outgoing feeder VCB's are in ON condition

Analysis- l.While GDCL, a sub agency of M/S SMI excavating the cable trench withcut taking proper

clearance from MECOII and RINL ncar MRSS,thc 5.6 KV cable from CSS-1to CSS-2 got cut

and Short circuited among allthree phases,

2.This has resulted in failure of power transformer
