IECEx Test Reports (ExTRs)

The following are the official and current versions of IECEx Test Reports (formerly IECEx Assessment and Test Report (ATR)) blank forms for use by IECEx Accepted Certification Bodies and Ex Test Laboratories They are available for download in MS Word format. Minor variations in pagination may occur due to MS Word reformatting for your local printer. NOTE: It should be noted that the recent publication of the revised edition of the IECEx scheme rules: IECEx 02 Equipment Certification Program covering equipment for use in explosive atmospheres Rules of Procedure has resulted in changes to some terminology, IECEx Accepted Certification Bodies (ACBs) are now called IECEx Certification Bodies (ExCBs) and IECEx Assessment and Test Report (ATR) are now called IECEx Test Report (ExTR). As new versions of the ExTRs become available the terminology will be changed accordingly.

The ExTR package in which this ExTR is incorporated (comprised of a single ExTR document or multiple ExTR documents) is to be accompanied by a single ExTR Cover Sheet, which is to be approved by the ExCB. ExTR Addendum and/or ExTR Report of National Differences may also supplement this ExTR.

Blank ExTR Test Report Template

This Blank Ex Test Report template is to be used by ExTLs and ExCBs when creating new or revised Ex Test Reports blank for submission to ExTAG WG1 Convenor for approval as an official ExTR form. (Posted 100820) For use with all IECEx ExTRs. (Revised and posted 101216) This document is to be compiled and reviewed by the ExTL, with the ExCB giving the final approval, or compiled by the ExCB without the involvement of the ExTL. It is to serve as the sole cover for an ExTR package, which may be comprised of a single ExTR document or multiple ExTR documents. This ExTR Cover is to be completed and attached to the completed ExTR package. For use with all IECEx ExTRs. (Revised and posted 100820) This ExTR Addendum is to be compiled and reviewed by the ExTL. The Addendum is to supplement an already issued Ex Test Report and is to be accompanied by a single ExTR Cover Sheet, which is to be approved by the ExCB. Only those clauses applicable to the supplemental issue being addressed are to be tabulated and remarked upon as part of this document. ExTR Report of National Differences may also supplement this ExTR Addendum.

IECEx Test Report National Differences

For use with all IECEx ExTRs. (Revised and posted 100820) This Ex Test Report of National Differences is to be compiled and reviewed by the ExTL. This document is to supplement an Ex Test Report or ExTR Addendum, with a separate such document issued for each intended country / region. All National Differences of the intended country / region are to be tabulated and remarked upon as part of this document. An ExTR of Partial Testing provides a clause-by-clause documentation of the initial evaluation and testing that verified compliance of an item or product with only select requirements from an IEC Ex standard. This ExTR of Partial Testing is part of an ExTR package that may include other Ex Test Report, Addendum and National Differences documents, along with a single ExTR Cover. An ExTR of Partial Testing is to be compiled and reviewed by the ExTL. The Issuing ExCB indicates final approval of the ExTR of Partial Testing as part of the overall ExTR package on the associated ExTR Cover.
