    During the 20th century, hernia repair trend changed several times. A very interesting competition has been going on between the opponents of Lichtenstein and laparoscopic repairs. We recently questioned the same issue by the same method to see if laparoscopic hernia repair found more opponents and preference rate in surgeons and physicians in Ankara, Turkey. In general 88.9% of the respondents preferred open repair, while only 11.1% chose laparoscopy. The majority of the participants who preferred an open repair stated that it was a better known technique to choice. Almost half considered local anaesthesia an advantage. Laparoscopic repair was mainly preferred for its advantages of less pain and early return to work. When three consecutive surveys among the same population in Ankara in 1997, 2001 and 2007 were compared the laparoscopic repair preference rates did not show a statistical difference (9.1%, 16.1% and 11.1% respectively).

    Background and aim: Umbilical hernia frequently accompanies cholelithiasis. It is possible to repair these hernias after completing cholecystectomy. We herein describe a simple modified technique for the repair. Patients and method: The technique was applied to 10 patients undergone laparoscopic cholecystectomy. After cholecystectomy has been performed, periumbilical trocar incision is extended toward the umbilicus. The hernia sac is sent into the abdominal cavity and one or two simple sutures are put to approximate the fascial edges of the umbilical hernia. A similar approximation is done for trocar hole. A piece of polypropylene mesh covering both defects with an adequate overlap at four edges is fixed in onlay position. Results: No wound complications were recorded. After a median of 23 months (6-40 months) follow-up no recurrence was observed. Conclusion: This simple modified repair may especially be useful in centers where the surgeons can easily perform cholecystectomy laparoscopically, but are not familiar with laparoscopic hernia repair and mesh placement or haven't the equipment and material necessary for a laparoscopic repair.

    We aimed to compare the three techniques of dartos pouch orchiopexy, suture fixation to the scrotal wall and narrowing of the neck of pouch with or without suture fixation, in regard to the postoperative ascensus of testes. We operated on 150 unilateral palpable undescended testis with the scrotal pouch orchiopexy technique. In this prospective study, patients were randomly divided into three groups: Testes were fixed to the scrotal wall in the first group, they were placed into the scrotal pouch without fixation but the neck of the dartos pouch was narrowed around the vas deferens and the vessels in the second group and testes were fixed to the scrotum with a suture and the dartos fascial opening was narrowed concomitantly in the third group. Patients were followed between 6 and 48 months (28.0 +/- 11.4). Only four testes of the first group (8.0%) were replaced upward after the operation where all the other two groups' testes were in their places. We believe that only narrowing of the dartos fascia around the vas deferens and vessels without fixation to the scotum prevents ascending of the testis upward after orchiopexy operations. We think this technique decrease the risk of supposed damage to the testicular tissue due to suture material.

    The role of free oxygen radicals in inflammatory conditions is well known. Free radicals cause lipid peroxidation of cellular membranes resulting in cell death. The purpose of this study was to investigate the levels of total anti-oxidant status (TAS), as a marker of anti-oxidant defense system and malondialdehyde (MDA), as a marker of oxidative stress, in the plasma of patients with acute appendicitis. Fifty-one adult patients with a median age of 31 years who underwent operations with a preoperative diagnosis of acute appendicitis were included in this prospective study. Blood samples for C-reactive protein (CRP), MDA and TAS were collected preoperatively. Groups were compared by using the Mann-Whitney U test.     Adrenal myelolipoma is a rare, benign and biochemically inactive tumor. It is usually diagnosed incidentally by radiological methods and is known to be associated with obesity, hypertension, endocrinological disorders and some malignancies. We report herein the association of a myelolipoma with a gastrointestinal stromal tumor. To our knowledge this is the first report of such an association to date. A 67-year-old male patient admitted to our clinic with abdomimal pain and fever; he had a history of hypertension and diabetes mellitus. In physical examination, a mass involving the right quadrants was palpated. Computerized tomography revealed a right retroperitoneal mass, probably originating from the kidney or cecum. In laparotomy, the tumor (12 cm radius and 1500 g) localized on the superior of right kidney was excised. Abdomen exploration revealed another mass with 10 cm radius 100 cm distal to the ligamentum of Treitz and segmental jejunal resection and anastomosis were applied. ...

    Diffuse nodular lymphoid hyperplasia represents a rare disease that is grossly characterized by the presence of numerous visible mucosal nodules measuring up to, and rarely exceeding, 0.5 cm in diameter. These may involve the entire small intestine, the large intestine, or both. The etiology is unknown. When diffuse nodular lymphoid hyperplasia is found predominantly in the colon, it can mimic a variety of polyposis syndromes and this may cause difficulties in diagnosis. The disease may be associated with other pathologies, especially gastrointestinal malignancies. This causes controversy when deciding the treatment options. Following patients without any treatment may lead to malignant progression, while surgical treatment may result in unnecessary radical resections because of obscurity in the diagnosis. We report here a diffuse nodular lymphoid hyperplasia case who underwent a radical resection because of obscurity in the diagnosis.

    Retained postoperative foreign bodies, of which sponges are the most common, are a rare situation. This may be due to the care of the surgical team or a reluctance to publish incidences that could lead to medicolegal problems. Despite this rarity in reporting, retained foreign bodies appear to be encountered more commonly than expected. All surgical tools used during an operation can become a retained foreign body. Surgical instruments, sponges and drains left in the operation site may be responsible for bizarre and varied complications. We describe a case of a Penrose drain retained in the axillary region discovered during the dissection of the lymph nodes due to epidermoid carcinoma.

    Elektif kolon cerrahisi uygulanacak hastalarda cerrahi girişim sonrası iyileşmenin hızlandırılması (Enhanced Recovery After Surgery ERAS) amacıyla 2001 yılında Kuzey Avrupa ülkelerinden beş merkez (İskoçya, İsveç, Danimarka, Norveç ve Hollanda) bir araya gelerek bir protokol geliştirdi (1). ERAS protokolü preoperatif, intraoperatif ve postoperatif dönemleri ilgilendiren 21 başlıktan oluşmaktadır (2). Protokol klasik cerrahi ve anestezi uygulamalarının dışına taşmakta ve radikal olarak nitelendirebileceğimiz yenilikler getirmektedir.     Diffuse nodular lymphoid hyperplasia (DNLH) isa very rare disease of unknown etiology. The cha-racteristic of DNLH is the presence of numerousvisible mucosal nodules in the small intestine, co-lon, or both. Mucosal polypoid lesions rarely exce-ed 0.5 cm in diameter. The presence of the lesionsin the colon may mimic a variety of polyposissyndromes, and this may cause some problems indiagnosis and treatment. Histologically in DNLH, there is enlargement ofthe mucosal B cell follicles caused by hyperplasiaof the follicle centers. These hyperplastic folliclesare confined to the mucosa and surrounded by anormal appearing mantle zone. DNLH may be as-sociated with intestinal lymphoma with or witho-ut dysgammaglobulinemia (1-6).     This study investigated the effects of an amino acid mixture containing arginine, glutamine, and β-hydroxy-β-methyl butyrate on secondary healing of ischemic wounds in a rat model (N = 18). After the formation of a bipediculated flap on each rat, 2 full-thickness excisional skin wounds (2 × 2 cm) were created on every flap. The rats were then randomized into the control and treatment groups. Every rat received standardized rat food throughout the study. The rats in the treatment group were administered an extra 200 mg/kg of L-arginine, 200 mg/kg of L-glutamine, and 40 mg/kg of β-hydroxy-β-methyl butyrate per day. Wound sizes were measured on days 0, 4, 10, and 14. The rats were sacrificed, and the wounds were excised for biochemical and histologic examination on the 14th day. As compared with the control group, the treatment group's wound sizes were significantly smaller on days 10 and 14 (P < .001), as was its inflammatory cell accumulation score (P = .008).     AIM: Bacterial translocation and septic events is strongly associated. Pro-prebiotics and symbiotics use in the prevention of bacterial translocation has been widely studied. The aim of this study was to investigate the effects of intestinal Bifidobacterium breve A28 as a newer probiotic, inulin as a prebiotic and both as a symbiotic on a rat model of obstructive jaundice induced-bacterial translocation. METHODS: To create a bacterial translocation obstructive jaundice model was used. Four groups, each have 12 rats, designed. Postoperatively 10 days the groups were fed with normal rat food plus pro-pre and symbiotic, and control group was fed only normal rat food. After this period, systemic blood, liver, spleen and mesenteric lymph nodes samples were obtained for microbiological culture. The Pearson’s chi-Square test was used for the statistical analysis. RESULTS: Probiotic, prebiotic effectively reduced bacterial translocation, seperately and as a symbiotic. No significant difference was observed between probiotic and prebiotic groups, and between symbiotic and probiotic groups regarding the rate of bacterial translocation. And there were a significant difference between control and prebiotic group. CONCLUSIONS: This new mixture of pro and prebiotic on bacterial translocation is promising.

    The aim of the present prospective randomized controlled trial is to compare the effectiveness of intravenous and oral antibiotic prophylaxis for cost and surgical site infection in elective laparoscopic cholecystectomy. Three hundred twenty patients were split into two groups as to include 160 patients in each, and they were evaluated in a prospective and randomized fashion. While one group was subjected to 1 g cefazolin via intravenous route during anesthesia induction, other group received 1 g cephalexin monohydrate via oral route 1 h prior to the operation. Demographic findings and operation results of the patients were compared by analyses. Our 320 patients (278 females and 42 males) received elective cholecystectomy and were followed up for a period of 6-26 months. Each group had 160 patients. Both groups were similar with regard to demographic characteristics and inclusion criteria. Among all, only five (1.5 %) cases demonstrated postoperative surgical site infection. Surgical site infection at postoperative period was determined in three (1.8 %) cases of intravenous prophylaxis group and two (1.2 %) cases of oral prophylaxis group. There was no statistically significant difference between the groups in terms of surgical site infection. Oral antibiotic prophylaxis can be used in elective laparoscopic cholecystectomy prophylaxis due to its cost-effective, reliable nature, and low surgical site infection rate.
