Gap nonunion of long bones is a challenging problem, due to the limitation of conventional reconstructive techniques more so if associated with infection and soft tissue defect. Treatment options such as autograft with non-vascularized fibula and cancellous bone graft, vascularized bone graft, and bone transportation are highly demanding on the part of surgeons and hospital setups and have many drawbacks. This study aims to analyze the outcome of patients with wide diaphyseal bone gap treated with induced-membrane technique (Masquelet technique). This study included 9 patients (7 males and 2 females), all with tibial bone-gap. Eight of the 9 patients were infected and in 3 patients there was associated large soft tissue defect requiring flap cover. This technique is two-stage procedure. An analysis of clinicopathological features of 240 cases presenting as mass in nasal cavity, paranasal sinuses and nasopharynx observed, both retrospectively and prospectively, over a period of 5 years in Jawaharlal Nehru Medical College. Aligarh. The incidence of masses in nasal cavity, paranasal sinuses and nasopharynx was 34.3 cases per year. Amongst the 240 cases studied, there were 144 cases (60%) of non-neoplastic lesions, 56 cases (23.33%) of benign lesions and 40 cases (16.67%) of malignant lesions. All age groups were involved and the mean age of presentation with the increasing age were: - non-neoplastic (22.5 years), benign tumors (26.8 years) and malignant tumors (35.3 years). The male to female ratio was 1.7:1 for non-neoplastic lesions; 3:1 for benign tumors: and 2.3:1 for malignant lesions. The adopted choice of this mode of disposal has been based on a study of the site and the quality and quantity of waste generated at the 5 MW reactor with HEU fuel. Specific measures regarding the radiation safety of the workers and environmental protection have been adopted. The waste disposal operations are conducted to meet local regulatory requirements, IAEA recommendations and internationally endorsed principles such as ALARA (as low as reasonably achievable - economic, social and other relevant factors being considered). The 3+1 condensation of symmetrical 16-selena/telluratripyrranes with symmetrical selenophene-2,5-diols / tellurophene2,5-diols in presence of BF3-etheratre or BF3-methanol followed by oxidation with DDQ gave 5,10-porphodimethenes; whereas with unsymmetrical selenophene-2,5-diols / tellurophene-2,5-diols gave 5-porphomethenes. Further the reaction of unsymmetrical 16-selena/telluratripyrranes with symmetrical selenophene-2,5-diols / tellurophene-2,5-diols gave the corresponding porphotrimethenes; whereas with unsymmetrical selenophene-2,5-diols / tellurophene-2,5-diols gave the 5,15-porphodimethenes. The structures of different products were characterized by IR, 1H, 13C NMR, 1H-1H COSY, CHN analysis and mass spectrometry. The binding of mercury with above calix[4]phyrins had been observed in the decreasing order of porphodimethenes ˃ porphomethenes ˃ porphotrimethenes by UV-vis and 1H NMR spectroscopy.

To evaluate the use of fibular grafting for fresh femoral neck fractures with posterior comminution. 18 women and 15 men aged 20 to 60 years underwent osteosynthesis and fibular strut grafting supplemented with 7.0-mm cannulated hip screws for Garden grades III (n=21) and IV (n=12) femoral neck fractures associated with posterior comminution. All fractures were reduced by closed methods, and no hip was aspirated. Clinical and radiological outcomes were evaluated. The mean delay in presentation after injury was 3.2 (range, 1-12) days. The mean delay in operation was 8.8 (range, 5-21) days. The mean follow-up period was 2 (range, 1-4) years. According to the Harris hip score, outcome was good to excellent in 20 patients, fair in 7, and poor in 6. 27 of the 33 patients achieved bone union after a mean of 4.7 (range, 4.2-7) months. Closed degloving skin lesions of the limbs are uncommon, and diagnosis may be missed at the time of acute trauma. Usually, they are associated with pelvic fractures but may occur in absence of any bony injury. When this closed internal degloving occurs over the greater trochanter, it is known as a Morel-Lavallee lesion. We report a case of unusually delayed presentation of Morel-Lavallee lesion treated successfully by debridement, packing, and delayed closure.

To evaluate the role of locked compression plates (LCPs) in management of peri- and intra-articular fractures around the knee. Twenty distal femoral and 20 proximal tibial fractures were fixed with LCPs. The types of femoral fractures were A1 (four), A2 (three), A3 (two), C1 (one), C2 (seven) and C3 (three). The types of tibial fractures were A2 (one), A3 (two), B2 (two), C1 (four), C2 (five) and C3 (six). All patients were followed up for up to 18 months (mean, 12 months). Fourteen patients with distal femoral fractures and 19 with proximal tibial fractures underwent surgery using a minimally invasive percutaneous plate osteosynthesis (MIPPO) technique. The others were treated by open reduction. The average time of fixation was 8 days after injury (0-31 days). Knee Society scores were used for clinical and functional assessment. All fractures, except one of the distal femur and one of the proximal tibia, united. The mean union times for distal femoral and proximal tibial fractures were 15.2 and 14.9 weeks, respectively. One patient with a distal femoral fracture had implant failure. One patient was quadriplegic and did not recover the ability to walk. The average Knee Society scores of the remaining 18 patients were 82.66 (excellent) and 77.77 (functional score, good). There was one case of implant failure and one of screw breakage in distal femoral fractures. One case of nonunion occurred in a proximal tibial fracture. Provided it is applied with proper understanding of biomechanics, LCP is one of the best available options for management of challenging peri- and intra-articular fractures.

The purpose of this study was to assess repeated exercise on the B-200 Isostation as part of rehabilitation work tolerance for nonsurgical patients with lumbar spine disorders. For a consecutive 7-month period, treatment subjects were randomly assigned according to birth date for participation in two groups: a standard work tolerance program only or standard work tolerance program plus inclusion of exercise on the B-200 Isostation. Each patient had similar referral diagnosis requiring conservative treatment. Treatment groups were compared with a control population of volunteers who had neither back pain nor known underlying spinal pathology. All study patients had objective measurement of range of motion, isometric strength, and velocity of motion, on the B-200 Isostation before treatment and at follow-up 3 weeks after treatment. The data showed no significant difference of percent improvement when comparing patients in either of the randomized assigned treatment groups. There was a higher percentage of improvement for each treatment group as compared with the control individuals, however. Based on our study using the B-200 Isostation, there is little objective justification for including exercise on the B-200 dynametric Isostation as part of the rehabilitation routine for improvement of functional physical capacity.

A large number of plants and their isolates have been shown to potentiate immunity. Some plants exert anti-inflammatory and anti-stress effects, others hepatoprotective activity. In this study, 320 1-day-old broiler chicks were randomly divided into four major groups A, B, C and D and fed rations supplemented with 0, 15, 20 and 22.5 g Berberis lycium kg⁻¹ ration respectively. Each group was further divided into two subgroups, one vaccinated against Newcastle disease (ND) and infectious bursal disease (IBD), the other non-vaccinated. Antibody titre against IBD and ND, relative weight of lymphoid organs, post-challenge morbidity and mortality, serum hepatic enzymes and total serum protein were observed. Group C had higher anti-IBD and anti-ND antibody titres. Relative bursa weight in groups C and D was higher until day 28, but birds in group C performed better at later stages of examination. Relative spleen weight was highest in group C. During initial stages there was no effect on relative thymus weight, but at later stages the effect was significant. Groups C and D performed similarly in terms of relative thymus weight. The birds were challenged to field IBD through intramuscular injection at a dose rate of 0.5 mL per bird. Post-challenge morbidity was lowest in groups C and D, while treatment significantly (P < 0.001) affected mortality amongst affected (morbid) birds. Levels of serum alanine aminotransferase and alkaline phosphatase were lowest in group C. Serum protein was similar in all groups and in both vaccinated and non-vaccinated broiler chicks. Berberis lycium added to feed at 20 g kg⁻¹ is effective in improving immunity against ND and IBD as well as liver function in broiler chicks.

ABSTRACT This study estimates quantitative and qualitative demand for housing using household survey data in Delhi. Both housing demand and demand for housing attributes are further stratified by settlement type–slum and non-slum, and by tenure – owner and renter. The estimation results indicate that housing demand is inelastic with respect to price and income, whereas the magnitude of price elasticity is overall smaller than that of income elasticity in absolute term. In slum households, however, price elasticity is larger than income elasticity. The estimates of housing attributes also show that floor area, availability of a separate kitchen, permanent material of roof, independent latrine, drainage, and flat type of dwelling structure, among others, are important determinants of rent. In slum households, however, only quantity variables matter while quality variables have little effect on rent. This study concludes with policy prescriptions including sufficient urban land supply, which is required to cope with income growth of non-slum households on one hand and to provide low cost dwelling for slum household on the other hand.

Few long-term studies have compared the efficacy of shoulder stabilization using arthroscopic or open procedures. To directly compare the outcomes of shoulder stabilization using arthroscopic transglenoid sutures versus open capsulolabral repairs. Uncontrolled retrospective review. Fifty patients (30 arthroscopic and 20 open) were followed for a minimum of 5 years (mean, 68 months). Pain, motion, strength, function, stability, patient satisfaction, radiographs, and ability to return to preinjury activity levels were evaluated. The arthroscopic group had a 17% (5 of 30) dislocation rate and a 60% (18 of 30) rate of instability. Sixty-seven percent (6 of 9) of patients participating in collision sports had repeat instability. Eight of 18 patients with instability required open repair for persistent symptoms. There were no limitations of motion following arthroscopic stabilization. The open group had no episodes of dislocations or instability. Forty-five percent (9 of 20) had some loss of external rotation (range, 10 degrees -40 degrees ) with a mean loss of 18. The one failure in this group had a 40 degrees loss of external rotation and difficulty with recreational activity. No patients participating in collision sports had repeat instability. We recommend open stabilization for patients participating in collision sports (for example, football, wrestling) or activities where better stability is required. For those participating in sports where performance may be compromised by loss of external rotation (such as swimming), newer arthroscopic techniques are recommended.
