Giant pulmonary hamartoma though a benign lesion can grow up to large size occupying a major part of the hemithorax and can cause mechanical compression to the adjacent structures, It is a spot radiological diagnosis in Chest X ray owing to it s characteristic pocorn calcifications.So far only 15 cases of Giant Pulmonary hamartomas are reported in English Literature and hence the interest in publishing.

The aetiology of pleural effusion in ESRD has been considered transudative unless otherwise proved. However there are several other alternate aetiologies for the same in a patient with end stage renal disease which requires proper evaluation and timely management especially the pleural fluid is altered by prolonged use of high dose diuretics and the high prevalence of tuberculosis in our community.

OSA is recently picked up as an independent risk factor for increased mortality among young individuals and syndrome Z is the occurrence of OSA in COPD patients.Apart from uncontrolled hypertension, mortality from ischaemic cardiovascular events, OSA is supposed to be a concern in those cases with unexplained Pulmonary hypertension which may be overlooked in lean OSA patients where Upper airway MRI and tests like cephalometry may be required to establish the diagnosis. However this presentation is an humble effort to simplify the polysomnography interpretation from a purely Pulmonologist perspective leaving aside the complex variants of OSA which may require multidisciplinary evaluation for proper management.

Dyspnoea and haemoptysis are the two common cardinal symptoms pertaining to the respiratory system. Though there are common causes for both, there could be very rare reasons for the same which may require specialized care including thoracic surgery intervention.Here we reported a case which required three major surgical procedures for correction of his dyspnoea and haemoptysis and after a very stormy clinical course survived with no residual disease.

Treating MDR  TB & XDR TB is probably one of the worst situations and a dreaded nightmare for a majority of health care workers worldwide.This paper is an attempt to share the experience which we gained by treating more than 500 such patients registered under the DOTS Plus programme from the DRTB site at Kozhikode which is the first centre in India to try decentralisation of DRTB services in a successful manner and has been credited for considering this strategy under the excellence in preventive health segment by the prestigious British Medical Journal-Awards India for 2014.This paper was also well accepted by the International Union against TB & Lung Diseases (IUATLD) at Paris in 2013.

Cases of malignant transformation in bullous lung disease are not infrequent, and most of these cancers turn out to be non small cell lung cancer. However, they are often difficult to diagnose because of their uncertain appearance. Here we report a case of bullous lung disease, now presenting with an air – fluid level, in which a diagnosis of malignancy was made by ultrasound, guided fine needle aspiration cytology. Hence this possibility has to be kept in mind when a known case of bullous lung disease presents with worsening symptoms or new radiological shadows which is often treated as secondary infection of a preexisting bulla.

Background: It was observed that there were a large number of diabetic patients, both previously known as well as recently detected amongst the MDR-TB (Multi drug resistant tuberculosis) patients who were being started on CAT-IV regime Aims: To analyze the profile of these Diabetic patients with MDR-TB so as to pick up any possible trends in their dual co-occurrence . Materials & Methods: The study was a descriptive analysis conducted in the Department of Pulmonary Medicine in 40 patients who had diabetes-MDR-TB co existence and were enrolled under DOTs Plus programme for CAT-IV regime during the period between February 2009 and August 2010 with the intention for identifying trends in the development of MDR-TB in diabetic patients. Results: There was a clear male predominance with 80% (32/40) cases belonging to the male sex.Seventy percent (28/40) patients were current or Exsmokers. Contrary to the general notion that most of the diabetic-MDR-TB patients may be underweight, only 30 % of patients with diabetes belonged to the < 45 Kg category. There was an alarmingly high proportion of diabetics in the group of CAT-I failures which accounts for  85 % (34/40) cases, of which a vast majority 52.5 % (21/40),  had uncontrolled diabetes at the time of diagnosis which makes it possibly a very strong risk factor for the development of MDR-TB and a significant proportion ,35% (14/40), were not even started on a proper diabetes control regimen till this period.Conclusion:The trend analysis of our diabetic MDR-TB patients points towards the fact that uncontrolled diabetes mellitus, is possibly a strong risk factor for the development of MDR-TB in smear positive tuberculosis, which is definitely a modifiable risk factor, if properly addressed as a component of the DOTs strategy, can influence the outcome & occurrence of multidrug resistant tuberculosis in a large number of patients.

This is a presentation of real cases encountered in our clinical practice over last 12 years regarding the various difficult to treat situations in tuberculosis management.
