The efficacy and effectiveness of dental implants have been proven during the last years. An economic evaluation must be integrated within the clinical evaluations to determine economic (allocative) efficiency. According to the literature dental implants will show better results. The treatment is, however, more expensive. Assessment of costs and benefits can lead to more efficiency in dental care. The possibilities to evaluate the outcome of treatment are discussed and essential aspects of the economic evaluation are described.

In this article the cost-effectiveness of dental implants (rootform and transmandibular) is compared with making complete dentures combined with vestibuloplasty and just a set of new dentures. The cost components of labour and technique are analysed at the individual patient level. Effectiveness was measured using a ratio scale. Several assumptions for the long run effectiveness of the treatment options are necessary due to lack of information about this in the literature. Assumptions were made regarding: survival rate, life span of dental implants and complete dentures, costs of aftercare and development of oral health in the long run. The main conclusion is that overdentures on rootform implants are superior to complete dentures combined with preprosthetic surgery or overdentures on a transmandibular implant. The choice between treatment with rootform implants or just a new set of dentures is a more difficult matter.

Vanaf januari 1999 wordt door de gezamenlijke inspanning van de verpleeghuizen Reggersoord-De Schiphorst en Thuiszorg Drenthe verpleeghuisverplaatste thuiszorg geleverd aan somatisch zieke patiënten en/of patiënten met psychogeriatrische problematiek. De doelstelling van het project is om opname in het verpleeghuis te voorkomen of uit te stellen door tijdelijk extra hulp te bieden. Het gaat om patiënten met een verpleeghuisindicatie (somatisch en psychogeriatrisch) die vooralsnog in hun eigen omgeving wonen. De zorgbehoefte bij deze patiënten is op een zeker moment groter dan de gecombineerde ondersteuningsmogelijkheden van mantelzorg en basiszorg vanuit de thuiszorg. De extra hulp wordt verleend door Thuiszorg Drenthe en verschillende verpleeghuisdisciplines.

The effectiveness of dental implants is widely studied, especially in terms of their clinical outcomes. However, from the policymaker's point of view, variables other than safety and efficacy, such as the costs and effectiveness of dental implants as compared to other treatment alternatives, are vital in decision making. This paper compares the costs of different treatment strategies in a randomized clinical trial in patients with resorbed mandibles and persistent problems with their conventional dentures: treatment with a mandibular overdenture on permucosal dental implants, an overdenture on a transmandibular implant, new dentures after preprosthetic surgery, and new dentures only. To describe pre-hospital delay times in patients with acute myocardial infarction (AMI) in two regions in the Netherlands: Groningen (a region with high mortality for coronary heart disease (CHD)) and Rotterdam (a region with low CHD mortality). Descriptive. The pre-hospital treatment delay of AMI patients in Rotterdam in 1990-1991 versus 1993-1995 was compared and also compared between Groningen en Rotterdam (1993-1995). In each region 3 hospitals participated (1 academic, 2 regional). The data were collected with a structured interview within 7 days after onset of symptoms in hospitalized AMI patients (n = 924) or by interviewing relatives of deceased patients (n = 40). Objective: The aim of this study was to determine whether diabetologists recognise patients’ needs for additional intensive multidisciplinary care due to glycaemic and diabetes-related psychosocial difficulties. Research design and methods: We compared 114 participants in a diabetes intervention programme with 201 as yet non-referred outpatients, of whom 54 outpatients were considered eligible for the intervention by their diabetologists; thus, 147 outpatients were considered non-eligible. Results: Analysis revealed that the eligible patients had poorer glycaemic control but all other parameters were similar to non-eligible patients. Significantly, 22 (15%) of the 147 non-eligible patients clearly had diabetes distress and could potentially benefit from the intervention. Conclusion: The results suggest that in regular care, patients’ needs with respect to glycaemic control are recognised by their diabetologists, but patients with high psychosocial diabetes-related distress are often overlooked, though they also may be in need of additional care. Integrated monitoring of diabetes-related distress in outpatients could improve this area of diabetes care.

The aim of this study was to compare implant-retained mandibular overdentures and two conventional treatments for their effects on functional ability (chewing, speaking, etc), patient satisfaction, and quality of life (psychosocial functioning). Assignment of 90 patients was executed by means of a balancing allocation computer program to ensure the pretreatment comparability of the three groups. Twelve months after treatment, the average scores for almost all specific quality-of-life measures had improved significantly in all three groups. On average, all patients experienced fewer restrictions in their social activities and had fewer psychological problems because of their full dentures. No impact on the general quality of life was established. One year after treatment, all three dental treatment modalities had a comparably positive effect on dental health-related quality of life.

The objective of the present report was to study the effect of implant treatment on subjective parameters in edentulous patients with an extremely resorbed mandible. Three different treatment modalities to support an overdenture were compared: transmandibular implant according to Bosker, augmentation of the mandible followed by four endosseous implants, and the insertion of four short endosseous implants. Sixty patients [50 women and 10 men, mean (+/- SD) age 59 (+/- 11) years] met the inclusion criteria and were assigned in one of the three treatment groups. Before treatment and 12 months after placement of the new overdentures, denture satisfaction, psychosocial aspects and experiences during the surgical phase were assessed with a battery of questionnaires. After 1 year, 58 patients were available for evaluation: one patient had died, and one patient had moved out of the region. Hostility has been studied mainly in relation to coronary heart disease (CHD). However, given the pathways linking hostility to CHD, it might be expected that hostility also relates to non-CHD. Therefore, the relation between the expression and the experience of hostility and various health outcomes was examined in a cross-sectional design. The data were collected among male patients with a myocardial infarction in the age range of 30-70 years (N = 279) and a population sample of men in the same age group (N = 2663). Based on checklist of the most frequent disorders, the subjects from the latter group were divided into subsamples according to their disease status. Three components of hostility, i.e., resentment, suspicion, and aggression, were measured by the Buss Durkee Hostility Inventory (Buss & Durkee, 1957). The overall finding was that all components of hostility were related to non-CHD disease but not to CHD.
