Rezumat
Summary
TIBOLA (tick-borne lymphadenopathy) este o
rickettsiozã transmisã la om printr-o cãpuºã din genul
Dermacentor, agentul patogen cel mai frecvent incriminat
fiind Rickettsia slovaca.
Pacientã în vârstã de 55 ani a fost consultatã pentru o
placã inflamatorie roºie violacee, infiltratã, rotundã având
diametrul de 2 cm, localizatã la nivelul vertexului ºi
apãrutã în urmã cu 45 de zile, la aproximativ 10 zile dupã
o înþepãturã de cãpuºã survenitã în luna februarie 2015, în
timpul unei plimbãri într-o pãdure. Înþepãtura a fost
însoþitã iniþial de un eritem edematos pruriginos al regiunii
cefalice. Placa inflamatorie era însoþitã de adenopatie
laterocervicalã dreaptã dureroasã. Pacienta nu prezenta
febrã ºi nici alterarea stãrii generale. Investigaþiile de
laborator hematologice ºi biochimice uzuale au fost
normale. Serologia pentru Borrelia burgdorferi a fost
negativã. Tratamentul a fost efectuat cu doxiciclinã 200
mg/zi timp de 3 sãptãmâni cu dispariþia leziunii
inflamatorii ºi a adenopatiei laterocervicale ºi persistenþa
doar a unei zone alopecice reziduale. Diagnosticul de
TIBOLA a fost stabilit pe baza criteriilor clinice,
anamnestice ºi evoluþiei bune sub tratamentul cu
Doxiciclinã.
Caracteristicile epidemiologice de TIBOLA sunt:
distribuirea sezonierã în lunile reci ale anului (septembriemai) ºi localizarea geograficã în þãri mediteraneene ºi ale
Europei de Est. Incubaþia medie este de 7 zile. Clinic,
diagnosticul de TIBOLA se bazeazã pe urmãtoarele criterii
obligatorii specifice: înþepãtura unei cãpuºe de mari
dimensiuni pe partea superioarã a corpului (90% pe pielea
pãroasã a capului), escarã necroticã de inoculare
înconjuratã de un halou eritematos fix ºi adenopatie

rickettsiosis transmitted to humans through a tick of the
Dermacentor genus,
most commonly incriminated
pathogen being Rickettsia slovaca.
A 55-year-old female patient was consulted for an
inflammatory red purple plaque, infiltrated, round with a
diameter of 2 cm, located in the upper part of the scalp and
emerged after 45 days, about 10 days after a tick bite
occurred in February 2015, during a walk in a forest. The
sting was initially accompanied by an itchy edematous
erythema of the cephalic region. The inflammatory plaque
was accompanied by painful right laterocervical
lymphadenopathy. The patient did not have fever or
impaired general condition. Common blood and biochemical
laboratory investigations were normal. Antibody testing
for Borrelia burgdorferi was negative. She was treated with
doxycycline 200 mg/day for 3 weeks with the disappearance
of the inflammatory lesion and the laterocervical
lymphadenopathy and the persistence of a residual alopecic
area. TIBOLA diagnosis was established based on clinical,
anamnestic criteria and improvement under treatment with
doxycycline.
The epidemiological characteristics of TIBOLA are:
seasonal distribution in the cold months of the year
(September to May) and geographical location in the
Mediterranean and Eastern Europe. The average
incubation is 7 days. Clinical diagnosis of TIBOLA is based
on the following binding specific criteria: the bite of a large
tick on the upper body (90% on the hairy skin of the head),
necrotic eschar at the site of the tick bite surrounded by
fixed erythematous halo and painful regional
lymphadenopathy (occipital , cervical) and nonspecific

regionalã dureroasã (occipitalã, cervicalã) ºi pe criterii
minore nespecifice: cefalee, febrã, artromialgii, astenie,
erupþie cutanatã. Diagnosticul de certitudine se bazeazã pe
examinãrile microbiologice dintre care cea mai utilizatã este
imunofluorescenþa indirectã. Evoluþia afecþiunii este rapid
favorabilã sub tratament cu dispariþia febrei în câteva zile ºi
a adenopatiei într-o sãptãmânã cu persistenþa unei alopecii
cicatriciale pe locul escarei. Tratamentul de elecþie este
doxiciclina la adulþi ºi macrolidele de ultimã generaþie
(azitromicina ºi claritromicina) la copii ºi femeile gravide.
TIBOLA este o rickettsiozã cu o emergenþã în creºtere
în România, în parte cauzatã rãspândirii cãpuºelor din
genul Dermacentor.
minor criteria: headache, fever, arthromyalgia, asthenia,
rash. A certainty diagnosis is based on microbiological
examinations of which indirect immunofluorescence is the
most widely used. The evolution of the disease is rapidly
improving under treatment with the disappearance of the
fever in a few days and the lymphadenopathy in a week with
a persistent cicatricial alopecia on the eschar. The treatment
of choice is doxycycline for adults and new-generation
macrolides (azithromycin and clarithromycin) in children
and pregnant women.
TIBOLA is a rickettsiosis with a growing emergence in
Romania, in part because of the spread of ticks of the genus
Dermacentor.

Introducere
TIBOLA (tick-borne lymphadenopathy) este
o rickettsiozã transmisã la om printr-o cãpuºã din
genul Dermacentor, agentul cauzal cel mai
frecvent incriminat fiind Rickettsia slovaca.
Tabloul clinic asociazã prezenþa înþepãturii de
cãpuºã, escarei inflamatorii la locul de inoculare
ºi adenopatiei locoregionale. Au fost propuse ºi
alte denumiri pentru acest sindrom precum
DEBONEL (DErmacentor - BOrne Necrosis Erythema Lymphadenopathy)1 ºi SENLAT (Scalp
Eschar and Neck LymphAdenopathy after Tickbite)2.
Caz clinic
Pacientã în vârstã de 55 ani a fost consultatã
pentru o placã inflamatorie, pruriginoasã,
localizatã la nivelul vertexului afirmativ apãrutã
în urmã cu aproximativ 45 de zile. Leziunea a
debutat la aproximativ 10 zile dupã o înþepãturã
de cãpuºã în timpul unei plimbãri într-o pãdure
din zona montanã în luna februarie a anului 2015.
Pacienta nu a prezentat febrã ºi nici alterarea
stãrii generale, iar tratamentul cu betalactamine
nu a fost eficient. La examenul dermatologic a
prezentat o placã inflamatorie roºie-violacee,
infiltratã, rotundã cu diametrul de 2 cm,
localizatã la nivelul vertexului însoþitã de
adenopatie laterocervicalã dreaptã dureroasã,
ganglionii având diametrul de pânã la 2 cm
(Fig.1, Fig.2). Investigaþiile de laborator

Introduction
TIBOLA (tick borne lymphadenopathy) is a
rickettsiosis transmitted to humans through a
tick of the Dermacentor genus, the most common
causative agent being Rickettsia Slovaca. The
clinical presentation associate the presence of a
tick bite, an inflammatory eschar at the
inoculation site and regional lymphadenopathy.
Other names have been proposed for this
syndrome such as DEBONEL (DErmacentor BOrne Necrosis - Erythema Lymphadenopathy)1
ºi SENLAT (Scalp Eschar and Neck
LymphAdenopathy after Tick-bite)2.
Clinical case
A 55-year-old female patient was consulted
for an itchy inflammatory plaque, located in the
upper part of the scalp and emerged 45 days ago.
The lesion appeared about 10 days after a tick
bite occurred in February 2015, during a walk in
a mountain forest. The patient did not have fever
or impaired general condition and the treatment
with beta-lactam antibiotics was inefficient. The
dermatological exam describes a red purple,
inflamatory plaque, infiltrated and round with a
diameter of 2 cm, located in the upper part of the
scalp, accompanied by painful right laterocervical
lymphadenopathy, with lymph nodes up to 2 cm
in diameter (Fig.1, Fig.2). Common blood and
biochemical laboratory investigations were

hematologice ºi biochimice uzuale au fost
normale. Serologia pentru Borrelia burgdorferi a
fost negativã. Pacienta a urmat tratament cu
doxiciclinã 200 mg/zi timp de 3 sãptãmâni cu
dispariþia atât a leziunii inflamatorii cât ºi a
adenopatiei laterocervicale dupã douã sãptãmâni
de tratament, la finalul tratamentului persistând
o zonã alopecicã rezidualã pe locul plãcii
eritematoase iniþiale. Diagnosticul de TIBOLA a
fost stabilit pe baza criteriilor anamnestice, clinice
ºi a evoluþiei favorabile sub tratamentul cu
Doxiciclinã.

diagnosis was established based on clinical,
anamnestic criteria and improvement under

Discuþii
Agentul patogen cel mai frecvent incriminat
în TIBOLA este R. slovaca, izolatã în 1968 în
Republica Cehã ºi având ca vector o cãpuºã mare
din genul Dermacentor. Raoult ºi colab. (1997)
descriu cazul unui pacient care prezenta o
leziune unicã de inoculare pe pielea pãroasã a
capului însoþitã de adenopatie cervicalã3. Un
tablou clinic similar este gãsit în acelaºi an de
Lakos la 27 pacienþi descriindu-l sub numele de
TIBOLA (TIck BOrne LymphAdenopathy), un
sindrom constând din asocierea unei escare
inflamatorii de inoculare, adenitã locoregionalã ºi
înþepãturã de insectã4. Alãturi de R. slovaca, în
apariþia TIBOLA sunt incriminate ºi alte
rickettioze sau bacterii intracelulare precum R.
raoultii5, Bartonella henselae6, Francisella
tularensis7.
Discutions
The most frequent incriminated pathogen in
TIBOLA is R. slovaca, isolated in 1968 in the
Czech Republic transmitted by large Dermacentor
ticks. Raoult et al. (1997) describe a patient who
shows a single inoculation lesion on hairy skin of
the head accompanied by cervical lymphadenopathy3. A similar clinical picture is
found in the same year by Lakos in 27 patients
describing it as TIBOLA (TIck BOrne
LymphAdenopathy), a syndrome consisting of a
combination of an inflammatory inoculation
eschar, regional adenopathy and insect bite4.
Other than R. slovaca, there are other rickettiosis
or intracellular bacteria that can be etiologic
agents for TIBOLA such as
R. raoultii5,

Bartonella henselae , Francisella tularensis7.
The infection has two epidemiological
characteristic features: seasonal distribution in
the cold months of the year (September to May,
with two peaks of incidence between September-

Infecþia are douã aspecte caracteristice
epidemiologice: distribuirea sezonierã pe
perioadele reci ale anului (din septembrie în mai,
având douã vârfuri ale incidenþei între septembrie-noiembrie ºi martie-mai) ºi localizarea
geograficã mediteraneanã (Franþa, Spania,
Portugalia, Italia) ºi Europa de Est (Ungaria,
Austria, România, Croaþia, Germania)5, aceste
particularitãþi fiind legate de distribuþia
Dermacentorului în Europa prin pãduri ºi stepe,
fiind atraºi de animale cu pãr ºi de temperatura
cuprinsã între 4 ºi 16oC la care acesta este activ.
Incubaþia este în medie de 7 zile (variind între
1 ºi 55 zile). Clinic se disting criterii clinice
obligatorii specifice: înþepãtura unei cãpuºe de
talie mare pe partea superioarã a corpului (90%
pe pielea pãroasã a capului), prezenþa la locul
inoculãrii a unei escare necrotice înconjuratã de
un halou eritematos fix (15% mai mari de 5cm) ºi
adenopatia regionalã dureroasã (occipitalã,
cervicalã) precum ºi criterii clinice minore
nespecifice: cefalee, febrã, artromialgie, astenie, ºi
rar erupþie cutanatã8.
Sub tratament, evoluþia este rapid favorabilã
cu dispariþia febrei în câteva zile ºi a adenopatiei
într-o sãptãmânã. În schimb, vindecarea escarei
este lentã durând sãptãmâni sau luni. Sunt notate
ºi sechele precum alopecia cicatricialã localã
tranzitorie sau definitivã la locul inoculãrii la
52,7% dintre pacienþi, astenie persistentã timp de
luni de zile la 36,8% dintre pacienþi9.
Spre deosebire de boala Lyme ºi febra
butonoasã
mediteraneanã,
TIBOLA
se
diferenþiazã prin vârstã mai tânãrã de apariþie,
localizarea leziunii de inoculare îndeosebi pe cap,
perioada de incubaþie mai scurtã ºi apariþia în
timpul lunilor reci ale anului10.
Diagnosticul de TIBOLA se bazeazã pe date
epidemiologice ºi pe aspectul clinic. Prezentarea
la consultaþie de multe ori târzie face ca semnele
inflamatorii iniþiale ºi locul înþepãturii de cãpuºã
sa fie cãutate cu grijã. Se pot observa o serie de
anomalii
biologice
precum
leucopenie,
trombocitopenie, citolizã hepaticã moderatã,
prezente ºi în alte rickettsioze.
Diagnosticul de certitudine se bazeazã pe
examinarea microbiologicã. Astfel, serologia prin
IFI reprezintã modul de diagnostic uºor ºi rapid
al unei infecþii recente (pânã la 3 luni dupã
inoculare), putând însã da reacþii încruciºate cu

steppes, being attracted to animals with hair and
temperature between 4 and 16oC temperature at
which it is active.

between 1 and 55 days). Clinical diagnosis of
TIBOLA is based on the following binding
specific criteria: the bite of a large tick on the
upper body (90% on the hairy skin of the head),
necrotic eschar at the site of the tick bite
surrounded by fixed erythematous halo(15%
larger than 5 cm) and painful regional
lymphadenopathy (occipital , cervical) and
nonspecific minor criteria: headache, fever,
arthromyalgia, asthenia, rash8.
Under treatment, evolution is rapidly
favorable with the disappearance of fever in a
few days and adenopathy in a week. Instead,
eschar healing is slow, lasting weeks or months.
Sequelae are noted such as local transient or
permanent cicatricial alopecia at the inoculation
site in 52.7% of patients, persistent asthenia for
months in 36.8% of patients9.
Unlike Lyme disease and Mediterranean
spotted fever, TIBOLA distinguishes by the
appearance at a younger age, the inoculation
lesion found especially on the head, shorter
incubation period and the emergence during the
cold months of the year10.
TIBOLA diagnosis is based on epidemiological data and clinical aspect. The medical
consultations are usually late, making the initial
inflammatory signs and tick-bite site to be
searched thoroughly. It can be observed a series
of biological abnormalities such as leukopenia,
thrombocytopenia, moderate hepatic cytolysis,
also present in other rickettsiosis.
A certainty diagnosis is based on
microbiological examination. Thus, serology by
indirect immunofluorescence represents a quick
and easy diagnosis of a recent infection (up to 3
months after inoculation), but may give crossreactions with other rickettsial species. Instead,
Western blot allows a diagnosis of species. It can
be performed a culture or PCR from the biopsy of
DermatoVenerol. (Buc.), 60: 249-254
alte specii de rickettsii. În schimb, Western blot
permite diagnosticul de specie. Se pot efectua
cultura sau PCR din biopsia zonei de inoculare,
din crustele escarei, din prelevãri recoltate pe
tampon prin frecarea escarei, din sângele
circulant al pacientului sau chiar din cãpuºa
incriminatã. Diagnosticul de certitudine rãmâne
totuºi dificil, în acest sens venind ºi rezultatele
obþinute de Ibarra ºi colab. în 2006 care nu a
obþinut confirmarea serologicã de rickettsioze
recente decât la 61% dintre pacienþi având tablou
clinic de TIBOLA8. PCR a fost pozitiv în 9 seruri
testate din 31 (29%), din 10/10 cãpuºe prelevate,
dar a fost negativ pe 31 de eºantioane de sânge ºi
biopsii cutanate. O explicaþie a testelor serologice
negative s-ar putea explica prin realizarea tardivã
a acestora de multe ori dupã începerea
antibioterapiei determinatã de prezenþa
simptomelor infecþioase (febrã, edem inflamator
al feþei), difuzarea strict locoregionalã a bacteriei
sau de multiplicitatea speciilor bacteriene
implicate în apariþia TIBOLA.
În privinþa tratamentului, literatura de
specialitate admite necesitatea tratãrii tuturor
speciilor de rickettsiozã acutã fãrã aºteptarea
confirmãrii diagnosticului datoritã posibilitãþii
apariþiei manifestãrilor clinice severe sau mortale
(ex. febra pãtatã a Munþilor Stâncoºi). Trebuie
þinut cont cã rickettsiile sunt bacterii intracelulare
rezistente la betalactamine, aminozide ºi
cotrimazol11. Ciclinele reprezintã tratamentul de
elecþie în TIBOLA, iar la copii ºi gravide
macrolidele de ultimã generaþie (azitromicinã
10mg/kgc/zi într-o singurã dozã pe zi ºi
claritromicinã 7.5 mg/kg de douã ori pe zi).
Durata tratamentului cu doxiciclinã variazã în
funcþie de autori: dozã unicã12, adaptarea duratei
în funcþie de evoluþia clinicã ºi încã 72 de ore
dupã obþinerea apirexiei11, 200 mg doxiciclinã
timp de 10,14 sau 21 zile sau încã 3 zile de
azitromicinã sau 5 zile de claritromicinã13.
Tratamentul aplicat rapid în faza inflamatorie a
afecþiunii conduce la dispariþia febrei în 48 ore,
iar a adenopatiei în 7 zile la 88% dintre pacienþi,
fãrã a preveni apariþia sechelelor (alopecia). În
schimb, în faza tardivã de TIBOLA cu prezenþa
alopeciei tratamentul nu este necesar.
În cazul prezentat, diagnosticul de TIBOLA
s-a bazat pe: contextul de apariþie al plãcii
inflamatorii de inoculare localizate la nivelul

from samples harvested on a swab by eschar
rubbing, from the patient’s bloodstream or even
from the criminalized tick. The certainty
diagnosis is still difficult, also in this regard came
the results of Ibarra et al. in 2006 who has
obtained serological confirmation of recent
rickettsioses only in 61% of patients with clinical
aspects of TIBOLA8. PCR was positive in 9 out of
31 serums tested (29%), 10 out of 10 collected
ticks, but was negative in 31 blood samples and
skin biopsies. An explanation of negative
serological tests could be explained by the their
delayed testing, often after starting the treatment
with antibiotics determined by the presence of
symptoms of infection (fever, inflammatory
edema of the face), the strictly regional
dissemination of bacteria or by the multiplicity of
bacterial species involved in the etiology of
TIBOLA .
When it comes to the treatment, the literature
agrees the necessity of treating all the species of
acute rickettsiosis without awaiting confirmation
of clinical diagnosis due to the possibility of
severe or fatal (ex. Rocky Mountain Spotted
Fever) clinical manifestations. It should be
remembered that Rickettsioses are intracellular
bacteria resistant to beta-lactam antibiotics,
aminozides and cotrimazole11. Cyclins antibiotics
are the treatment of choice in TIBOLA and in
children and pregnant women the treatment is
made
with
new-generation
macrolides
(azithromycin 10 mg/kg/day in a single dose per
day and clarithromycin 7.5 mg/kg twice a day).
The duration of treatment with doxycycline
varies according to authors: single dose12,
adjusting the dose depending on clinical
evolution and another 72 hours after the
disappearance of fever11, 200 mg of doxycycline
for 10, 14, or 21 days, or an additional 3 days of
azithromycin or 5 days of clarithromycin13. A fast
treatment in the inflammatory phase of the
disease quickly leads to disappearance of fever
within 48 hours and the adenopathy in 7 days in
88% of patients, without the prevention of
sequelae (alopecia). In contrast, in the late phase
of TIBOLA with the presence of alopecia
treatment it is not necessary.
In the present case, TIBOLA diagnosis was
based on: the context of developing an

Concluzii
TIBOLA este o afecþiune puþin frecventã ºi
necunoscutã, putând fi întâlnitã ºi în România.
Cunoaºterea caracteristicilor clinice ºi epidemiologice face posibilã identificarea ºi tratarea
corectã a afecþiunii.
Conclusions
TIBOLA is a less common and unknown
condition, which may be encountered in Romania.
Knowledge of clinical and epidemiological
characteristics make it possible to identify the
condition and to treat it correctly.
