Visual Evoked Potentials

Polygraphic recordings were collected for 11 normal subjects during sleep and wakefulness in order to investigate characteristics of the rapid eye movement (REM) associated potentials. EEGs were averaged using 5 different triggering points: (1) saccade onset under the normal ambient illumination, (2) saccade onset in the total darkness, (3) onset of REMs during REM sleep, (4) flash during REM sleep, and (5) flash during stage 2 sleep. In the central area, positive potentials appeared with waking saccades under the normal ambient illumination (P240L) and REMs (P185R). The latency of P185R associated with REMs was significantly shorter than that of P240L associated with waking saccades. These findings suggest that P185R is evoked by PGO waves occurring just before the REM. A small positive potential appeared in the occipital area with waking saccade under the normal ambient illumination (P260L) and REMs in the total darkness (P250R). ObjectiveTo establish whether people exposed to drinking water contaminated with 20 tonnes of aluminium sulphate in the Camelford area of Cornwall in the south west of England in July 1988 had suffered organic brain damage as opposed to psychological trauma only.DesignRetrospective study of affected people.Participants55 affected people and 15 siblings nearest in age to one of the group but who had not been exposed to the contaminated water were studied.Main outcome measuresVarious clinical and psychological tests to determine medical condition and anxiety levels in affected people. Assessment of premorbid IQ (pFSIQ) with the national adult reading test, a computerised battery of psychomotor testing, and measurement of the difference in latencies between the flash and pattern visual evoked potentials in all participants.ResultsThe mean (SE) pFSIQ was above average at 114.4 (1.1). The most sensitive of the psychomotor tests for organic brain disease was the symbol digit coding (SDC) test (normal score 100, abnormal <85). Participants performed less well on this test (54.5 (6.0)) than expected from their pFSIQ (P<0.0001) and a little less poorly on the averaged less discriminating tests within the battery (86.1 (2.5), P<0.0001). In a comparison with the 15 sibling pairs (affected people’s age 41.0 (3.3) years v sibling age of 42.7 (3.1) years (P=0.36) the exposed people had similar pFSIQ (114.7 (2.1)) to their siblings (116.3 (2.1), (P=0.59) but performed badly on the symbol digit coding test (51.8 (16.6)) v (87.5 (4.9) for siblings, P=0.03). The flash-pattern differences in exposed people were greater than in 42 unrelated control subjects of similar age (27.33 (1.64) ms v 18.57 (1.47) ms, P=0.0002). The 15 unexposed siblings had significantly better flash-pattern differences than their affected siblings (13.4 (2.4) ms v 29.6 (2.9) ms, P=0.0002). No effect of anxiety could be shown on these measurements from the analysis of the anxiety scores of exposed people.ConclusionPeople who were exposed to the contaminated water at Camelford suffered considerable damage to cerebral function, which was not related to anxiety. Follow up studies would be required to determine the longer term prognosis for affected individuals. The visual mismatch negativity (vMMN) response is an event-related potential (ERP) component, which is automatically elicited by events that violate predictions based on prior events. VMMN experiments use visual stimulus repetition to induce predictions, and vMMN is obtained by subtracting the response to rare unpredicted stimuli from those to frequent stimuli. One increasingly popular interpretation of the mismatch response postulates that vMMN, similar to its auditory counterpart (aMMN), represents a prediction error response generated by cortical mechanisms forming probabilistic representations of sensory signals. Visual and auditory reaction times (RTs) have been reported to decrease during moderate aerobic exercise, and this has been interpreted as reflecting an exercise-induced activation (EIA) of cognitive information processing. In the present study we examined changes in several independent measures of information processing (RT, accuracy, P300 latency and amplitude) during exercise, and their relationship to visual or auditory modalities and to gender. P300 latencies offer independent measures of cognitive speed that are unrelated to motor output, and P300 amplitudes have been used as measures of attentional allocation. The aim of the present study was to investigate baseline neurophysiological characteristics of the central and autonomous regulation and their reactivity to different tests in a group of persons with so-called &amp;#39;electrical hypersensitivity&amp;#39;, which is often considered as a form of psychosomatic disorders. Twenty patients with combinations of neuroasthenic symptoms (general fatigue, weakness, dizziness, headache) and facial skin (itching, tingling, redness) have been investigated. An equal number of symptom-free persons served as a control group. The examination comprised self-reported measures, testing of visual functions, measurements of blood pressure, heart rate and its variability, electrodermal activity, respiration, EEG and visual evoked potentials (VEP). Several variables were found to differ between the patient and the control groups. The mean value of heart rate in rest condition was higher in the patient group compared to the controls (mean value of inter-beat intervals were 0.80 and 0.90 s, respectively). Heart rate variability and response to standing test were decreased in the patient group compared to the controls. Patients had faster onset, higher amplitudes, and left-right hand asymmetry of the sympathetic skin responses. They had a higher critical fusion frequency (43 vs. 40 Hz), and a trend to increased amplitude of steady-state VEPs at stimulation frequencies of 30-70 Hz. The data indicated that the observed group of patients had a trend to hyper sympathotone, hyperresponsiveness to sensor stimulation and heightened arousal.

Visual Evoked Potentials (VEPs) are evoked potentials generated in response to visual stimuli. The flash VEP (FVEP) is used less frequently than pattern-reversal VEP (PR-VEP) because; it shows great variations in both latency and amplitude in normal subjects. The advantage of FVEP is its feasibility in non-cooperative subjects, which circumvents the major limitation of PR-VEP. The present study was undertaken to assess the effect of change of color of flashlight on variability of FVEP latencies. Healthy subjects in the age group of 18-30 years underwent the standard stimulus using white light, followed by altered stimuli done with red and blue light. 2 trials were given for each eye, for each type of stimulus. We present the case of a patient having dissociative identity disorder (DID) who-after 15 years of misdiagnosed cortical blindness-step-by-step regained sight during psychotherapeutic treatment. At first only a few personality states regained vision whereas others remained blind. This could be confirmed by electrophysiological measurement, in which visual evoked potentials (VEPs) were absent in the blind personality states but were normal and stable in the seeing states. A switch between these states could happen within seconds. We assume a top-down modulation of activity in the primary visual pathway as a neural basis of such psychogenic blindness, possibly at the level of the thalamus. VEPs therefore do not allow separating psychogenic blindness from organic disruption of the visual pathway. In summary, psychogenic blindness seems to suppress visual information at an early neural stage.

PURPOSE. To assess whether there are any advantages of binocular over monocular vision under blur conditions.
METHODS. We measured the effect of defocus, induced by positive lenses, on the pattern reversal Visual Evoked Potential (VEP) and on visual acuity (VA). Monocular (dominant eye) and binocular VEPs were recorded from thirteen volunteers (average age: 28±5 years, average spherical equivalent: -0.25±0.73D) for defocus up to 2.00D using positive powered lenses. VEPs were elicited using reversing 10 arcmin checks (4 reversals/sec). The stimulus subtended a circular field of 7 degrees with 100% contrast and mean luminance 30 cd/m2. VA was measured under the same conditions using ETDRS charts. All measurements were performed at 1m viewing distance with best spectacle sphero-cylindrical correction and natural pupils.
RESULTS. With binocular stimulation, amplitudes and implicit times of the P100 component of the VEPs were greater and shorter, respectively, in all cases than for monocular stimulation. Mean binocular enhancement ratio in the P100 amplitude was 2.1 in-focus, increasing linearly with defocus to be 3.1 at +2.00D defocus. Mean peak latency was 2.9 ms shorter in-focus with binocular than for monocular stimulation, with the difference increasing with defocus to 8.8 ms at +2.00 D. As for the VEP amplitude, VA was always better with binocular than with monocular vision, with the difference being greater for higher retinal blur.
CONCLUSIONS. Both subjective and electrophysiological results show that binocular vision ameliorates the effect of defocus. The increased binocular facilitation observed with retinal blur may be due to the activation of a larger population of neurons at close-to-threshold detection under binocular stimulation.
