JOB DESCRIPTION : Male and female toilet renovation.

If Work Plan cannot be followed exactly as sequenced then must stop work, contact Project Coordinator, Area and System Owner, Project Manager and the
EHS to revise and update the JHA.

Potential Hazards
Hazards
1. To inform the Project Coordinator or person in charge
Miscommunication
Inform PM before start the job in daily basis.

Unsafe work area
Only start work after receives verbal approval from PM.
PM need to buy off the work area for GC to start work in time.
To conduct toolbox talk before enter to site.
2. To identify the final location of the job area according to
Wrong Location
GCs PM instruction and drawing.
GC shall survey and confirm the job area before start the work.
Unsafe work area
GC EHS site team with collaboration off PM shall perform Hazard
identification and risk assessment prior to start work.

To park the vehicle at designated parking lot.

To unload tools and materials arrived from the lorry to work

area using trolley and manpower.

5. To barricade the work area accordingly before start work.

To communicate with the Area Owner about the location
where the work is going to be done.
6. To hack the existing brick wall partition using hacker and
hammer and to dismantle the ceiling boards and ceiling T
Worker shall placed suitable size of choke at the vehicles tire

before start the unloading task.
If the unloading task taking a long period workers shall
temporarily barricade the unloading spot with safety cone. To
have a full time spotter to control the traffic flow if necessary.
Workers shall wear hand glove to prevent hand cut and injuries
during the mobilization works.
Worker shall perform work with buddy system when lifting the
long materials and ladders.
GC Supervisor/safety supervisor to explain the emergency
evacuation plan to the workers.
Unsafe work area
GC site team and workers need to cordon off the work area with
sufficient space to work and store the material and tools.
Relevant signage need to be posted at the cordoned area.

To use ELCB when tapping power from points and to maintain

hang the power cords higher and do not lay on the floor.
To use dusk mask , ear plugs and safety goggle while hacking.
To use inspected ladder and to comply buddy system and three
point contact while working on ladder.
7. To clear all the hacked bricks and debris using sand bags
and trolley.
8. To install 1 PVC water pipes for sinks and sanitary fittings

10. To install ceiling T and ceiling board using cutter, plier and
hand drill.

hand tools.
13. To install sanitary fitting (Toilet bowl, urinal bowl and sink )
using hand drill , hand tool and cement.

14. To install timber doors for the male and female toilet using
hand drill.

To use hand gloves and relevant PPE related to the task.

To work in buddy system if material handling is too big or too
heavy.
To check the hand tools prior to start the installation job.
To confirm with GC and PM before installation and to refer to
approved drawing.
To wear proper rubber gloves to prevent from skin exposed to
cement that can cause skin irritation.
To maintain proper body posture while tiles installation and to
comply buddy system if material handling is too large or too
heavy.
To use inspected ladder and to comply with buddy system and
three point contact.
To check all the hand tools and power tools before use and to
maintain proper cord management system.
To wear proper hand glove while performing the task.
To wear proper rubber gloves to prevent from skin irritation and
dermatitis.
To use inspected ladder and to comply with buddy system and

To use dust mask or respirator to prevent from inhaling odour
caused from paint.
To wear safety goggle and all relevant PPE related to the job.

area. To clear all unused items and waste material immediately
from site after the job completion.
To use proper tray or container to clear the tools, material and
debris.
