1. AIM

WILL CARS COOPERATIVE 23165, RL Establishes the steps to follow to execute the work
"ADEQUACY OF INFRASTRUCTURE OF ELECTRICAL SUBSTATIONS OF THE ANACO
DISTRICT YEAR 2012.", safely, identifying, examining and notifying the personnel involved of the
possible risks associated with the activity, fully complying with the regulations. and sequential order of
this procedure, to guarantee the physical integrity of the workers, protect the facilities and the
environment and ensure that the different activities are carried out in compliance with all quality control
requirements, both our own and those of the client.

2. SCOPE

This procedure consists of carrying out works of

work.

4. ORGANIZATION AND RESPONSIBILITIES.

4.1. PROJECT ORGANIZATION CHART

ROLES AND RESPONSIBILITIES.

PRESIDENT

Support and promote all activities related to INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE
AND ENVIRONMENT, this is done through the approval and monitoring of SIHO plans and
programs, presented to management by the INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE
AND ENVIRONMENT department. .
Register the Company in the RACDA (RASDA) for toxic waste generators
Carry out field audits on SIHO matters every two weeks. and leave a record.
Present to the contracting management any event that occurred on the work.
Ensure compliance with the Rules, Regulations, Laws and Ordinances regarding INDUSTRIAL
SAFETY, OCCUPATIONAL HYGIENE AND ENVIRONMENT.
Guarantee an adequate budget to carry out all activities of the SIHO Program.
Provide resources for the training of personnel in SIHO matters.
Comply with management visits.
Review Engineering (plans, procedures)
Check regulations
Verify operation of the equipment and materials to be installed.

RESIDENT ENGINEER

Support and promote all activities related to INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE
AND ENVIRONMENT, this is done through the approval and monitoring of SIHO plans and

This will prepare its work plan, establish the guidelines and safe work procedures for which it will
have the support of the area supervisors and inspector of INDUSTRIAL SAFETY,
OCCUPATIONAL HYGIENE AND ENVIRONMENT, maintain direct contact with the area
supervisors and Hygiene Safety and Environment in order to discuss the achievements made during
the execution of the safety work and also take corrective measures when necessary.
Provide all necessary support to all staff and supervisors regarding inventory of implements and other
important aspects for carrying out the project in safe conditions.
Carry out constant audits with the SIHO inspector.
Investigate any events with the SIHO inspector.
Attend talks and meetings on Industrial safety, Occupational hygiene and Environments.
Identify the risks adjacent to the work area that could affect the activities of the WILL CARS

Monitor personnel, equipment and field procedures to ensure that all activities are carried out safely.
Maintain a Preventive Maintenance Program for equipment and vehicles and keep a record of it.
Together with the workers and with advice from the SIHO inspector, prepare the ART
Give talks to workers before starting work, based on the information from the ART.
Prepare and place all work materials on site, prepare herbicide mixtures.
Guide workers in safe work procedures and practices, correct them if there is any deviation.
Keep work sites clean, with good lighting and adequate ventilation.
Inform the immediate supervisor if any accident or incident occurs and make the corresponding
report.
Take disciplinary measures with those workers who violate the established rules.

project.

the project.
Control and verify compliance with the WORKPLACE HEALTH AND SAFETY PROGRAM.

Advise supervisors in the preparation of the ART

Prepare monitoring reports for the specific project plan. As established in the plan
Prepare and deliver statistics of the SIHO indexes of the work. As established in the plan

Maintain permanent communication with the project leader, the contractor company and the Client's
SIHO personnel.
Verify compliance with recommendations arising from accident/incident investigations.

management in their presentation at the SIHO committee meetings of the Contracting Management
as well as their dissemination on the construction site.
Verify compliance with the entry and exit requirements of personnel. Through medical verification as

the complaints they make in writing.
Verify the preparation and execution procedure of the ART.

Verify whether workers have knowledge of work standards and procedures for the activities to be
carried out on the site.

QUALIFIED PERSONNEL.

Comply with all provisions and recommendations issued by supervisory personnel, in the same way
report unsafe acts and conditions that may cause any unwanted event.
Mandatory use of the personal protective equipment provided by COOPERATIVA WILL CARS
23165, RL keep it in good condition and request its replacement when it deteriorates.
Maintain order and cleanliness in all work areas before, during and after activities.

Attend the scheduled talks on INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE AND
ENVIRONMENT and follow the instructions and/or recommendations emanating from it.
Report any deviation in behavior and any deviation in the work environment.
Exercise the specific functions derived from the employment contract in relation to the risks linked to
it, not only in defense of their own health and safety, but also with respect to other workers.
Comply with the instructions, warnings and teachings given regarding INDUSTRIAL SAFETY,

necessary.
Notify the RESIDENT ENGINEER and the SIHO-A Inspector of all acts and actions related to
the care of injured people.
Ensure that a minimum stock of medications is maintained at the workplace.
Have current certifications.
Stay updated in your work area (The company guarantees compliance as established in the specific

Maintain a work team together with the ambulance operator and the INSP. SIHO-A

7. CONDITIONS EXISTING BEFORE AND DURING THE WORK

For the purposes of the protection of workers, the work must be carried out in conditions appropriate to
the physical and mental capacity of the workers and to facilitate their execution, a daily supply of
drinking water, ice, glasses, and all those elements necessary to fully comply with basic sanitation, as
indicated by LOPCYMAT.

TO ENSURE THE WELL-BEING OF WORKERS, THE FOLLOWING WORKING


CONDITIONS MUST BE COMPLIED WITH:

a) Recognition and evaluation of the work area: that the work area is clean and tidy, that no
chemical, physical, biological, ergonomic or climatic agents are found that affect the workers'
work day.
b) Preparation and signing of the ART: that the ART is signed and approved by the executing and
PDVSA personnel.
c) Carry out an audit of the ART to verify if it complies with the regulations: the ART must comply
with the established parameters, in addition to there being a change in the process it must be
identified.
d) Disclosure of the operational risk analysis system (ART): there must be a 5-minute chat record.
Provision and/or supply of personal protective equipment (gloves, masks, helmet, glasses, boots
with iron toes, protective belts for working at height for each worker.) and all those PPE that are
necessary at the time of the activity. .
e) Visual verification of the physical condition of workers: they must be in good physical condition
to carry out their activities
f) Placement of materials and tools on site: they must be placed strategically and orderly in order
not to obstruct the passage of workers.
g) All equipment to be used will be reviewed under the supervision WILL CARS COOPERATIVE
23165, RL and their correct operation will be checked: to prevent damaged equipment from
causing an accident, for this reason the equipment must be in good condition.

k) Verify that workers have sufficient experience in performing their duties: the worker must have a
certain level of experience in the activity being carried out.
l) Allow access to the work area only to the personnel and vehicles involved: there should not be
any personnel who do not belong to the project since this can generate a risk agent for them or
the personnel working.
m) Use transportation routes indicated by the PDVSA installation custodian: the routes must be
made known in case of contingency or eviction of personnel.
n) Examine that escape routes: escape routes must be free of obstacles that prevent evacuation of
the area in case of emergency.
o) Check the conditions of the tools (picks, shovels, machetes, etc.) to be used in the activity: the
tools must be in good condition.
p) Review and notification of irregular terrain where the activity will be carried out: those areas that
may cause falls to the working personnel must be indicated.
q) Check the weather: workers should not work if there are unfavorable weather conditions (rain,
thunderstorms)
r) Avoid the accumulation of waste: maintain control of waste management.
s) Check for the presence of poisonous insects or animals: personnel should not be subjected to
dangerous biological conditions.
t) Keep vehicles in the work area: personnel transport, ambulance, tool transport, along with their
drivers must remain in the work area.

The personnel executing the work must know the types of risks associated with the activities to be
carried out daily, which will be notified through a Risk induction and notification program to personnel
throughout the project.

The Notification program will consist of informing the executing staff through induction talks with
topics related to the scope of work, procedures, Risks present on site, Risk Analysis, preventive
measures, emergency plans, reading of ART, etc. in order to comply with Title IV, Chapter II, Article
56, numeral 4 of the LOPCYMAT. All induction talks and risk notifications to personnel must be signed
by the Supervisor and the construction staff.

As with the initial induction of each worker, they must be informed by their SIHO-A Inspector of
the activities, risks and prevention measures to be carried out on each work day (Reading of ART, Risk

the “CRIMINAL ENVIRONMENTAL LAW” in its decree No. 2216 on RULES FOR THE
MANAGEMENT OF SOLID WASTE OF DOMESTIC, COMMERCIAL, INDUSTRIAL ORIGIN,
OR OF ANY OTHER NATURE THAT IS NOT HAZARDOUS, which reads “Solid waste generated in
the works must be classified, stored, collected and transported until its final disposal.”

WORK PROCEDURE

The supervisor of the work must prepare (SIHO-A Inspector) the corresponding Risk Analysis,
identifying the existing Risks in the area and the sources or actions that degrade the environment,
pointing out the prevention and control actions.
Maintain and verify the existence of the corresponding permits for the execution of the work and
also the permits of the Ministry of the Environment in case of carrying out actions that could affect
the environment.
The RESIDENT ENGINEER will visually inspect the area where the work is carried out.
A specific area will be cordoned off for waste generated at the work.
Solid waste of domestic, commercial, industrial, or any other nature will be classified according to
its physical properties.
Daily cleaning of work sites will be implemented at the end of each task.
One day a week will be assigned for the general cleaning of all work fronts and for the transfer of
waste to a designated place.
The RESIDENT ENGINEER and INSPECTOR SIHO-A will give a talk on the management of
hazardous and non-hazardous solid waste.
All materials and waste generated will be collected in appropriate containers and taken from the
work site to an approved location for final disposal. (recycler via cantaura)
A record will be kept of the volume of solid waste managed.
The RESIDENT ENGINEER will verify that the area is orderly and clean.

DEFINITIONS

storage, transportation, characterization, treatment, final disposal and any other operation that involves

Establish control measures for the management of Liquid/solid Effluents through the development and
application of an operating procedure that allows us, thanks to its application, to have an occupational
health and environmental protection program for both the personnel executing the work, as well as for
the communities in the area where the work is carried out to achieve environmental conservation

All liquid effluents will be collected in portable toilets. These will be periodically cleaned by a company
specialized in this type of work. Said effluents will be properly disposed of outside the area. The
company in charge of this activity must present the cooperative with the permits. the competent
authorities to provide this type of services (MARNR, health, municipal permit, etc.) and the volume of
liquid waste handled periodically.
Note: the rental of portable toilets does not apply to the contract.

Continuous Noise: It is that whose time interval between 2 maximum levels has a duration less than or

Scope
 This plan contemplates general aspects, control points and actions to follow in the event of a
serious emergency, providing responses to the different circumstances that may arise, reflecting a
state of initial readiness that will serve as support in the event of an emergency.
Policies
 When a worker enters the company, a disclosure of the emergency plan is made, as well as
before starting any project, the respective emergency plan must be reviewed and approved.
 Several emergency drills will be carried out both in the projects in progress and in the company's
facilities in order to measure the effectiveness of the emergency plan.
Definitions and Abbreviations
 Contingency Plan : procedure that guides actions in order to solve an emergency, facilitate its
control and minimize associated losses. This is put into practice when it is likely to affect
communities and it is necessary to use external resources to control the situation.
 Emergency: Circumstances that occur suddenly and unexpectedly due to operational failures,
acts of third parties or natural phenomena; which endanger people's lives, damage property and
the environment. They are resolved with our own resources and do not affect the communities.
 Serious emergencies: any condition that endangers people's lives, irreversible damage to
property or the environment, which may be within the company's control capacity. In some cases
only limited external help is required for control.
 Means of Communication: The means to be used for emergency communications will be
through portable equipment (radios) .
 Personnel Evacuation: if rapid mobilization of personnel is required, the same daily work units
will be used.
 Evacuation routes: these are determined according to the geographical characteristics of the
project execution area.
 Eviction: When an emergency occurs, the construction supervisor will notify what happened
and, if necessary, will give the order to evacuate the workers, guiding them through the
emergency routes indicated above.
 Contact with the Company and Official Organizations. When any emergency is registered, the
INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE AND ENVIRONMENT INSPECTOR
must immediately establish precise and concise communication with the Operators and Guards
and other public organizations that are responsible for what happened and/or have participation
in solving the problem.
 Rescue procedure. In order to avoid aggravating the injuries due to poor treatment or
spontaneous reactions of an untrained person, the injured should be moved to a safe place and
proceed with the evacuation.

10.1 EMERGENCY RESPONSE AND CONTROL.

10.1.1 DEFINITION OF ROLES AND RESPONSIBILITIES.

It will be the responsibility of the Presidency of COOPERATIVA WILL CARS 23165, RL, that
all members of its team are familiar with the content of these procedures, the administration and direct
observation of this procedure in its execution. For emergency situations, this plan includes procedures

e) Clear the place where the injured person is to provide greater ventilation.

i) Report to PCP.
j) The RESIDENT ENGINEER and the INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE
AND ENVIRONMENT INSPECTOR will begin the investigation of the event.
k) The INDUSTRIAL SAFETY, OCCUPATIONAL HYGIENE AND ENVIRONMENT
INSPECTOR and the Supervisor of COOPERATIVE WILL CARS 23165, RL They must
prepare a report within the next 8 hours, detailing what happened and the Construction Manager
must make the formal presentation to PDVSA and present it to the Ministry of Labor. And

10.6 EMERGENCY PLAN IN CASE OF FIRE:

If a fire outbreak or a small fire occurs as a result of poor operation or failure of the equipment that
generates heat.
Inform supervisor
The same will be extinguished with the fire extinguishers, which will be operated mainly by the
supervisory personnel and, failing that, by any worker who has received the induction in the
handling of fire extinguishers at the beginning of the work.
When the fires are of large proportions and the origin and the agents involved in it are unknown, all
personnel must be evacuated from the affected area, maintaining the coordination of supervisory
personnel, in the same way as all workers are instructed in the initial safety briefing regarding
evacuation techniques.
If possible, disconnect electrical and internal combustion equipment that is being used.
Refrain from searching for briefcases, tools, or any other material that makes evacuation tasks
difficult.
Activate any previously established communication mechanism with which workers are alert to fires
(radio and/or verbal communication).
Report the fire by calling the Fire Station
Call the Fire Department,
Indicate exactly the location of the fire or emergency,
Indicate the nature and magnitude of the emergency.
Try to stay calm.
If the escape route is filled with smoke, stay at ground level for more air and visibility.
Do not remove the safety mask at any time until you are in an area with fresh air.

10.7 IN THE PRESENCE OF SMOKE:

Unless the smoke is very dense and contains a toxic substance, it is not very dangerous for people,
so the respiratory protection equipment recommended for the activity must be placed, failing that, a
handkerchief or wet clothing over the nose, this will facilitate consequently, normal breathing, since
it does not reduce the lack of oxygen and prevents the inhalation of gases.
If the smoke is very dense, put on your mask and move to ground level, crawling on your hands
and knees, as close to the ground as possible.
If you get lost in any task, immediately look for a wall and move along it until you reach an exit.

1. At the beginning of the activities, the Custodial Supervisor must define the escape route, which

10.10 CONTINGENCY PLAN

OBJECTIVE: This plan has been prepared to respond to any accident and/or incident during the
performance of this activity.

PREVIOUS ACTIVITIES:

The personnel involved will be informed of the existence of this plan to guarantee its faithful
compliance.
Crowds of vehicles around the work site will be avoided. They will be parked 350 m from the work
front so as to allow rapid evacuation of the site.

IMMEDIATE ACTIONS IN CASE OF CONTINGENCY

All equipment and vehicles operating in the work area will be turned off, and all personnel and
residents in the area will be evacuated from the area near the emergency site.
Traffic will be stopped in both directions of the emergency area.
The area will be cordoned off and access to the area by unauthorized persons will be prohibited,
unless they are personnel from the companies involved duly trained for this contingency.
The contractor company will provide all necessary support to the PDVSA company in order to
resolve the situation.
An exhaustive report of what happened will be prepared and presented to PDVSA and the PDVSA
Supervisors involved to evaluate the case and make the necessary recommendations.
