    SCHOLARSHIP AGREEMENT

    KNOW ALL MEN BY THESE PRESENTS:

    This agreement executed by , a bonafide student of the Polytechnic


    University of the Philippines, hereinafter called the AWARDEE, residing at
    _____________________________________________________________with the written consent of
    _________________________________________________the student’s PARENT/GUARDIAN, residing at
    _______________________________________________________both hereinafter referred to as the Party of
    the First Part, and the Polytechnic University of the Philippines, with the Principal office at Sta. Mesa,
    Manila, hereinafter referred to as the Party of the Second Part.

    - WITNESSETH -

    WHEREAS, the University recognizes and encourages academic excellence and its scholarship program
    is designed to support those who strive toward each excellence.

    WHEREAS, the University has approved to grant the AWARDEE a scholarship consisting of
    [ ] full or [ ] partial tuition fees remission for the _____ semester of school year 20___ /20___ :

    WHEREFORE, the Parties of the First and Second Parts hereby agree to be bound by the following
    conditions:
    1. That it shall be the responsibility of the AWARDEE to:
    a. Fullfill the requirement specified in Office Memo No. 2, s. 1980;
    b. Report to the respective trainer/coach/supervisor regularly;
    c. Show exemplary conduct at all times;
    d. Maintain the required scholarship performance;
    e. Extend cooperation and support in the prosecution and/or implementation
    of the University projects, its rules and regulations.

    2. That it shall be the responsibility of the University to:


    a. Maintain the herein stipulated scholarship support under the aforementioned conditions;
    b. Cancel the scholarship grant anytime on the basis of :
    b.1 Misrepresentation of the AWARDEE in his/her application for scholarship;
    b.2 Willful violation by the AWARDEE of the rules, regulations, and guidelines of the
    University
    b.3 Behavior unbecoming of the scholar determined by the competent authorities
    of the University;
    b.4 Conduct inimical to the interest of the University.

    3. That the parties of this contract further agree:


    a. That, upon cancellation of the scholarship grant, the AWARDEE shall pay the equivalent tuition
    fee; provided, further, that the AWARDEE’s grades for the semester shall be withheld until
    his/her obligation is settled;
    b. That the PARENT/GUARDIAN shall assist the AWARDEE in complying with the
    aforementioned terms and conditions.

    In witness whereof, we have hereunto set our hands in Manila this __________ day of
    __________________ 20 ___________.
