CONTRACT OF AFFILIATION
KNOW ALL MEN BY THESE PRESENTS:
This CONTRACT OF AFFILIATION made and entered into this ________________day of

________ 2024 at Iloilo City, Philippines by and between:
The ________________, a domestic corporation duly organized and existing institution of

higher learning under the laws of the Republic of the Philippines with principal address at
___________________, duly represented herein by ____________________, its Chief
Operating Officer, and hereinafter referred to as the “UNIVERSITY”,
And
The __________________________ duly represented in this agreement by its President,

_________________ herein after referred to as the “HOSPITAL’
WITNESSETH
WHEREAS, the HOSPITAL is basically equipped with facilities necessary to meet the demands
of the Nursing Program of the different Colleges of Nursing in the City of Iloilo;
WHEREAS, the UNIVERSITY, having a College of Nursing, is interested to avail the use of the
HOSPITAL training facilities;
WHEREAS, the UNIVERSITY, desires to give broader clinical exposure to its Nursing Students
by sending them to the HOSPITAL;
NOW THEREFORE, for and in consideration of the mutual agreement and covenant, the parties
herein entered into this Contract of Affiliation and agreed on the following terms and condition,
to wit;
1. That the UNIVERSITY shall pay the HOSPITAL, the affiliation fee at the start of the
training period as follows;
Nursing:
Php 120.00 per student for 50-80 hours
Php 60.00 per student for 10-29 hours -
2. That the Nursing students of the UNIVERSITY, while at the HOSPITAL, shall be subject
to the rules and regulation of the HOSPITAL;
3. That all breakage/s and damage/s that any Nursing student of the UNIVERSITY may
cause on the properties/equipment of the HOSPITAL shall be paid by the UNIVERSITY
(to be charged to the Nursing student/s by the Training Center or the UNIVERSITY);
4. That the HOSPITAL, shall before the beginning of each clinical year, specify the number
of Nursing students allowed to rotate per month and the UNIVERSITY shall send more or
less the same number of Nursing students each month but shall not exceed the number
of students specified by the HOSPITAL.
5. That in case of loss or damage to the HOSPITAL’S patient’s incurred due to the
negligence of the Nursing Students, the WVSU shall not be liable unless proven that the
negligence originated from its staff;
6. That the HOSPITAL reserves the right and has the duty to suspend or cancel the
affiliation privileges if the affiliated Nursing students have not complied with the
requirements of the CONTRACT OF AFFILIATION and/or have violated the rules and
regulations set herein; and or the affiliating Nursing students have repeatedly committed
errors detrimental to health, safety, and welfare of the institution’s clients. The final
decision however shall be made by the HOSPITAL DIRECTOR.
7. That the clinical exposure of the nursing student affiliates shall be undertaken by the
Hospital Director for Nursing Services of the HOSPITAL, who shall furnish the Dean
of the College of Nursing of the UNIVERSITY with such policies, rules, and regulations
applicable for the instant affiliation.
8. That this affiliation agreement shall be for a period of TWO (2) YEARS from the complete
signing herein of herein Contract of Affiliation, provided that should the HOSPITAL or the
UNIVERSITY decide to terminate this affiliation agreement, a notice in writing shall be
given to either party six (6) months before the actual termination.
9. The term of this contract is from November3, 2023 to November 3, 2025
10. That the parties hereby agree that any and all patient data gathered and processed in
relation to the implementation of this Memorandom of Agreement shall be strictly held
confidential in accordance with the provision of Data Privacy Act (RA 10173). The
trainees shall be required to sign a confidentiality/non-disclosure agreement prior to the
start of their training; and,
11. That the provisions stated under HOSPITAL affiliation rules shall be part and parcel of
this agreement.
IN WITNESS WHEREOF, the parties hereto have hereunder set their hands this ________day
of __________2024 at Iloilo City, Philippines.
