Check all that apply at your work site. Read & Heed
DO YOU NEED A SPOTTER? Obey all operating instructions & warnings
DO YOU NEED BARRICADES? Only trained personnel are
Pedestrians or Others in Work Area allowed to operate mobile equipment
Debris on Ground Only trained personnel are allowed to enter
Wind Speed/Weather Conditions aerial lift platforms
Parked or Moving Vehicles ALWAYS be aware of overhead lines
Grating/Surface Approved by Engineer NEVER exceed the Working Load Limit
Jobsite Ruts/Rough Spots. NEVER utilize an aerial lift as material hoisting
Jobsite Pot Holes/Speed Bumps/RR Tracks equipment
Deep Ditches/ Excavations ALWAYS make sure the access gate is closed,
Drop Offs safety chain is latched or seat belt is fastened.
Obstacles/Bump Posts ALWAYS tie off to the manufacturer's desig-

Steep Slopes ALWAYS keep both feet planted firmly on the

Culverts Keep hoses, electrical cords, welding leads
Trench Drains clear of moving parts when raising or lowering
Edges of Loading Docks ALWAYS use three point contact when
Building Projections entering or exiting equipment
Overhead Obstructions Keep all body parts inside platform railings or
Overhead Power or Phone Lines cab
Underground Power or Gas Lines ALWAYS check your surroundings and look in
Above Ground Fuel Tanks the direction of travel
Hazardous Locations - Restricted Areas ALWAYS barricade your work area to keep
Fire Hydrant/Equipment unwanted personnel out
Wooden Pallets and/or Crates Good Condition Use cribbing for outriggers if necessary
Forklift/RTFL Loads Secured DO NOT alter or modify equipment
