    This is in connection with the request letter of the Contractor, INSERT NAME OF CONTRACTOR,
    dated INSERT DATE OF LETTER, for the Time Extension of the INSERT SCOPE OF WORK under PO No.
    INSERT PO NUMBER. As of INSERT DATE OF LATEST ACCOMPLISHMENT EVALUATION, the project is
    at INSERT PERCENTAGE OF WORK Completion. Upon evaluation and verification by Construction
    Management, we conform that the reasons of delay stated on the attached request letter is
    valid and we recommend extending their contract until INSERT REVISED DATE.

    INSERT ADDITIONAL DETAILS IF NEEDED

    This requested time extension will be FINAL. Failure to comply on the date granted under the
    normal site condition after this request, shall result to the imposition of liquidated damages on
    the amount stated on the Contract under General Conditions, Article IV.1 Failure to Complete
    Work.
