What This Module is About

We have learned in the previous lesson about how to maintain hand tools, drawing
instruments, equipment, and paraphernalia. There are various activities that we encountered
such performing on safety procedures, following and identifying the procedures on cleaning
and identifying the malfunction hand tools, drawing instruments, equipment, and
paraphernalia. In this lesson we will discuss and give importance in following the standard
procedures in accomplishing and inspection forms such as Job order slip, Requisition slips,
and borrowers slip, and Inspection check list.

Onviewing the deferent samples of forms and slip, it caters with various
importance on creating views of ideas on making a set of familiarities through a
sample forms, and it is also a good venture knowing their procedures in
accomplishing and preparation of each forms. In connection with your new lesson,
you are going to learn and create a sample forms/slip by following a standard
procedures in accomplishing each, the lesson will make you enhance your
procedural skills while creating your output.

What is it

Drafting Materials and equipment are substantial’s for drafting instruction


therefor proper procedures in barrowing and retrieval is essential. As what you have
written in the activity above, it shows that instruction and procedures must be
equipped during and after the activity.
Procedures in accomplishing forms
A. Requisition Procedure

1. List down the materials and tools needed as per job requirement.
2. Check for the availability of the needed materials and tools in the stockroom.
3. Accomplish a requisition form
4. Let the shop teacher check the requisition form
5. Submit the requisition form to the materials
6. Unavailable requested materials and tools should be substituted without sacrificing the cost
and quality of work
7. The materials officer will prepare the requested materials for issuance to the students

B. Preparing a Job Order

Making a job order and successfully filling it are two different things. Here are a
few pointers on how a job order has to be like.

1. Good job title – Perhaps the most important part and the first thing seen is the job
title. Use specific titles like Facilities Manager.

2. Great job description – Create brief but compelling descriptions about the job and
help the candidates picture themselves in the position.

3. Applicant qualifications – Provide mandatory minimum requirements.

4. Compensation and benefits – Always state the obvious, what is in it for the applicant.

C. How to Write a Work Order Letter

Writing a work order letter is a simple process, but you need to make sure you include enough
information to let your maintenance team know exactly what they need to do. Here, we’ll go over
how to write a work order letter.

D. How to Make a Requisition Slip Form
Follow the steps below to create a general type of requisition slip form:
1. Prepare the sheet to be used for the form.
The areas to be prepared must include the margin and the required design preferences
of the organization or the company who will use the requisition slip form for their request
procedures within the company.

2. Make the header.

The header would be the area where the name of the company and its logo will be
indicated along with the title of the form. In addition, this is also where the address and the
contact information of the company can be stated, specifically below the organization or
the company’s name.

3. Incorporate an area for the details of the requestor.


The general information of the requestor should be documented in the form in cluding
his position in the company and the department that he is representing or the one who will
be needing the requested items. The date of when the requestor used the form can also be
included in this part of the form. Entry fields must be retained bla nk and with labels to
guide the requestor in filling out each field with the appropriate information.

4. Create a table for the requisition details.


The table should be able to collect the description of each item to be requested, its
quantity, price, and the overall price estimation to be considered.

5. Place a signature block.


The signature block must have lines for the signature, notes, and dates when the
decision of the approval or requisition committee has been made.

After the contents are arranged and finalized, the created form should be saved in a
location in the device which can be accessed by the intended users or distributors of the
form. Furthermore, its file name should also be the same as the title stated in the header.

E. A borrower’s slip.

Procedures

1. Fill out a Service Equipment Request Form (SERF)


2. Have it noted by either of the following:
 Teacher of the student
 Faculty for classroom use
 Department/Unit Heads
3. Submit the accomplished form to the in charge of the borrowed tools and equipment’s
Note: 3 days lead time is required. The request is still subject to approval
c. Lost/Damaged Equipment
a. Lost Equipment
i. Equipment lost while on loan must be reported immediately.

ii. Lost equipment must be replaced or paid within one (1) week after it was
reported lost. If the borrower fails to settle the transaction, computation of fine will
start after the one-week grace period until the account has been settled.
iii. The equipment must be paid or replaced according to the value determined by
the Accounting Office.
b. Damaged Equipment
i. Any damage or malfunction in any of the equipment should be reported
immediately to the IT Help Desk.
ii. For laptops or projectors returned that are damaged but still repairable, the Party
Responsible will be liable for the repair charges of the equipment and should fill
out a Material Requisition Form (MRF).
iii. For laptops or projectors returned that are damaged and are beyond repair the
ITS Office will charge the Party Responsible based on the depreciated cost as
determined by the Accounting Office.

Summary

After fulfilling all the activities you had encounter on the module, you had learned about the
different sample forms and slip, were you can use in your next venture of lesson the forms can led you
into proper paper tools secured as requirement of the different task you might encounter such as
borrowing of tools, materials and equipment’s to the property custodian and fulfilling borrowers slip
and also the making of fulfilling a job order form, for a request on making a request to a repair or
needed tools. There are also forms on making inventory of tools before and after retrieval that could
be use in checking of tools returned and identifying the status of each materials/equipment and tools,
