Nausea & Vomiting


 Pregnancy: Diclectin (Pyridoxine + Doxylamine) / Dimenhydrinate (OTC) / Ginger
 Post chemotherapy: Metoclopramide (D Rs Atg), Prochlorperazine (D Rs Atg),
Dronabinol, Ondansetron (5-HT Rs Atg) / Palonosetron / Dexamethasone /
Promethazine
 Opioid induces: Metoclopramide, Prochlorperazine
 For pilots: Meclizine (long t1/2)

Constipation: tt by laxative
 Bulk-forming (Psyllium, bran, Methylcellulose): with 250 ml water to prevent
esophageal obstruction
C.I: GIT mechanical obstruction or fluid restriction / 2hrs away from food
 Osmotic Laxative: Glycerin, Lactulose
C.I: pts on galactose free diet .
 Saline laxatives:
Mg hydroxide (Milk of magnesia)
C.I: pts with cardiac or renal dis.
Interfere with absorption of Fe & Folic acid
Mg sulfate (Epsom salt) / Na phosphate
C.I: pregnant & lactating women
 Enema: not for < 2yrs, Na restricted pts, renal or cardiac diseases
 Stimulant laxatives:
Senna: DOC Opioid-induced, Excreted in breast milk
Bisacodyl: for long-term use for pts on H2Rs atg
Cascara sagrada: C.I: Children, Excreted in breast milk
Castor oil: C.I: Pregnancy, Used in ER
 Stool softener: Docusate Na
 Lubricant/Emollient: Mineral oil

BEST USED FOR:


 Infants: Glycerin, Lactulose or sorbitol
 Children: Mg hydroxide, Mineral oil, Lactulose or sorbitol
 Pregnancy: Bulk-forming agents, stool softener, Osmotic, Mg hydroxide
Avoid: stimulant laxative, Mineral oil, Castor oil
 Breast feeding: Bulk-foaming agents, Osmotic / Mg sulfate, Cascara, Senna (2nd
choice)
 Cancer: stimulant laxative with enema /
 Elderly: Glycerin, Lactulose
