1. Scope Of Work
This procedure covers the requirements of PT Linde and PT Asuka Engineering Indonesia Jawa
LG – TSF Project and Civil Construction Works regarding the work of the high concrete wall
casting method in several facilities, namely:

1. Mac Blow Off Silincer ( 3,5 m x 3,5 m ; Height 17,95 m )


2. Gan Gox Off Silincer ( 3,55 m x 4,05 m ; Height 14,1 m )
3. Molsieve ( 2,5 m x 3 m ; Height 14,5 m)
4. Transformer Fire Wall ( 24 m x 12 m ; Height 6,6 m)

3. Definitions
COMPANY : PT. Linde Indonesia
CONTRACTOR : PT. Asuka Engineering Indonesia
SUB-CONTRACTOR : The company that manufactures or supplies equipment and services
to perform duties specified by CONTRACTOR. "VENDOR" covers
Manufacturer/ Supplier/Sub-vendor.

4. Responsibilities

Group / person who will be responsible in the site is listed bellow :

4.1. Project Manager

1. Ensuring that the project works in his zone are carried out in accordance with
Company policies and in accordance with the requirements of this Quality plan.
2. Ensuring the full compliance of subcontractors with Company Quality policies and
with the requirements of this Quality plan.

3. To ensure that all the equipment required to execute the works according to the
construction programme are available, in good condition, and provide any
additional equipment that might be required.
4. To co-ordinate with the Construction Manager, Project Engineer, Safety Engineer,
Foreman and Surveyor for a safe and proper execution of the works.
5. To guide specific attention to all safety measures in co-ordination with the safety
officer/engineer.

4.2. Construction Manager

1. Ensure area is ready and safe to start the works.


2. Set up required equipment and plant through discussion with the Project Manager
and Project Engineer/Works Supervisor.
3. Ensure the works are carried out according to the specification, quality and
approved shop drawings.
4. Liaise and co-ordinate with the Project Manager for the agreed sequence of works
with respect to the construction programme.
5. Allocation of required manpower through co-ordination with the Project Manager.
6. Provide the risk assessments for the works in hand.
7. Provide sufficient and safe access for operatives, trucks and pumps.
8. Take precautionary measures with regards to protecting works from hot weather,
wind, rain and sun.

4.3. Project Engineer

1. The engineer shall carry out his duties in a manner that shall be co-ordinated by
the project manager on a daily basis, and shall ensure proper distribution of the
workforce and equipment at required locations.
2. To be aware of test frequencies related to the formation level.
3. To control disposal of waste excavated material according to the instructions
received from the Project Manager.
4. To co-ordinate with the Safety Officer to maintain safe working and proper
housekeeping of the site. To comply with the safety measures and ensure that all
Safety Officer Teams are aware of the same to prevent accident and loss.
5. To monitor and check all activities and ensure that works shall be carried out
according to specifications, quality and approved drawings.
6. To inform the QC Inspector of the areas ready for inspection.

4.4. QA/QC Inspector

1. Ensuring that Consultant/Client inspection requests are implemented.


2. Compilation of all necessary quality control checklists
3. Assisting Consultants during the Inspections.
4. Coordinating with the third party lab regarding tests and results.
5. The control of work performance by means of checking the work before
consultants’ inspection and issuing RFIs & punch lists as necessary.
6. Completion of documentation to verify the work performed.
7. Controlling all inspection activities on-site in line with ITPs.
8. Ensuring that all test equipment including surveying equipment is calibrated and is
suitable for use on-site.

4.5. Surveyor

1. To establish benchmarks from agreed reference points, provide required setting


out and level markings and follow up with regular checks.
2. Co-ordinate with the Project Engineer / Foreman and ensure the approved shop
drawings/construction drawings shall be implemented properly.
3. Maintain survey details and reports, periodically check the progressing works and
advise the project manager of any deviation from the drawings.
4. To Control Alignment / Verticality, etc During Vertical Section Assembly and After
Dismantle the Formwork

4.6. HSE Manager

1. To ensure implementation of all safety measures related to the nature of works


being carried out, and in accordance with the Project Safety Plan.
2. To ensure that all the persons involved in the works are aware of their
responsibilities, and that they have enough understanding of the safety procedures.
3. The safety officer in co-ordination with the Project Manager shall ensure that all the
implemented safety measures are effective enough to maintain safe working on the
site.
4. To maintain continuous inspections of the site activities, advise and train persons
on a daily basis to prevent accidents and personnel injury.

5. To give special concern to housekeeping, and ensure that the site is maintained
clean and tidy.
6. To ensure all the relevant safety sign boards for different works are in place.

4.7. Foremen/ Works Supervisor

1. Ensure the works are progressed in the sequence as agreed with the Project
Manager.
2. Liaise with the Project / Construction Manager for the allocation of the work force,
ensuring adequate manpower is available.
3. Liaise with the site manager to ensure all the required plant / materials are available
to construct the works.
4. Full time supervision shall be required to ensure the works are carried out in
accordance to specifications, quality and approved drawings

4.8. Skilled And Unskilled Labourers

1. Participated in Toolbox Meetings


2. Doing House Keeping
3. Prepare all the required plant / materials are avaible to construct the work
4. Follow the directions of the foreman / supervisor

4.9. Lifting Supervisor

1. Make a lifting plan and toolbox meeting before activity


2. Make sure the paper permit to work is in the acc

4. Make sure equipment such as body harness is ready and tagged
5. Make sure heavy equipment such as crane is ready and tagged

5.1. Admistration Prepration

- Have Valid work Permit include attachment (JSA-Work-Method-Drawing-Certificate


competent if needed )

- Worker has pass working at height training and have sticker WAH training in safety Helmet

- Job Safety Analysis ( JSA ) has been Approved

- Full body Harness Double lanyard must be use ( if the height more than 6.25 m should have
absorber)

- Drawing has been IFC and Marking Stamp by Asuka

- Worker who involve at working at height should be conduct Dailiy check up (DCU) by Medic
( nurse)

- Work Method has been approved

5.2. Scaffolding Installation

In the installation of scaffolding, the work is divided into several sections, each section
having a height of 3 - 4 m according to the height of the building. Installation of scaffolding
around the building as much as 3 scaffolding shafts. for the distance between the building
and scaffolding as wide as 1/2 meter

Section A-A ( First Section )


- on the inner platform (scaffolding) only as a side installation platform in the formwork
- Access to the casting is carried out from the outer side of the platform
- The support in the first section was removed when working on the sections so as not to
interfere with access

- As a substitute for support as in the first section, in the second section and so on, use
wire rope which is attached to the concrete section below
- for the base stand point formwork using a tie rod that is embedded in the concrete
section wall below it

5.3. Formwork Installation

At this stage the installation follows the size of the plywood, which is 2000 mm x 1120 mm
x 18 mm which will be arranged vertically. the installation is arranged in a maximum of 2
plywood in a vertical direction and a horizontal direction following according to the width of
the building.

1. Rebar is installed with a 6m persection and then given a spacer to maintain the distance
between the 2 main bars and installed concrete decking and formwork to maintain
verticality
2. The formwork is given a tierod to hold the panel but before being assisted using a
support that is plugged into the ground, for a height of above 3m, the formwork is
supported using iron that is plugged into the previous section.
3. In each section, the tie rod that is at the top is given a wire rope to maintain the
stability of the formwork and its verticality
4. For manhole holes use the opening listed on the Drawing forcon for access, a
minimum of 60 cm x 60 cm according to the opening of the building. for the location
adjust the opening point (according to the Drawing forcon ). if there is no opening in
the forcon drawing then we make a manhole and it will be closed with grouting after
casting at the highest elevation and finishing has been done.
5. The process of lifting the formwork is divided into 2, namely manually and using a
crane. if manual lifting, formwork will be assembled one by one on top of the building.
if using a crane, the formwork has been assembled below and then lifted to be
installed.

5.4. Casting Concrete

At the casting stage using a concrete pump to support the work and can reach all areas.
During the concrete filling stage, the entire area must be filled to a maximum height of 1
meter before continuing at the next height so that the results of casting can produce
maximum results. The concrete pump must pour the concrete gradually in one pouring. Use
an external vibrator for each layer pouring to assist the internal vibrator. Concrete is poured
over the top of the formwork using a 3'' dim tremy pipe and placed at the pour point so that
the concrete spreads evenly.

5.5. Dismantle The Formwork

Disassembling the formwork, approximately waiting 3 - 7 days according to quality control


instructions for the formwork to be dismantled and dismantled sequentially from top to
bottom. As per result of compressive strength test sample concrete.

5.6. Work Description

We have several scope for our Concrete Wall Section Facility, therefore;
