A review of all underground drawings is to be conducted to


determine the location of any piping or electrical services

EQUIPMENT OPERATIONS
Operators will be certified for the equipment operated

A fire extinguisher shall be provided in the immediate area

and pogo stick type compactors

Hearing protection is required for all employees working in


the immediate area of high noise compacting equipment

Concrete Operations
All protruding reinforcing steel that an employee could fall
onto shall be protected

Employees shall not be allowed to ride concrete buckets nor


work under buckets while buckets are being raised, swung or
lowered into position

Proper working platforms and access shall be provided

Concrete trucks are to be equipped with an audible back up


alarm

Flaggers equipped with an orange vest shall direct and spot


backing concrete trucks

Concrete trucks are not permitted to travel project road ways


with chute gates attached

Pump Trucks
Are to be leveled with outriggers fully extended and down

Pumps and pressure gauges are to be manned and monitored


at all times during the pumping operation

All pipe supports (spouts) will be designed to 100% overload.


Compressed air hoses will utilize fail-safe joint connectors.
Tremies, elephant trunks, etc. sections will be secured with a
fail safe chain or cable

Pump and spouts are to be cleared if there is an extended


interruption of pumping operations

Formwork / Shoring

Form work and shoring will be designed, erected , supported
braced and maintained so as to safely support all vertical and
lateral loads

All shoring equipment will be inspected prior to erection

All base-plates, shore heads, extension devices and


adjustment screws will be in firm contact with the footing sill
and form material

Stripping

Forms and shores must not be removed until the concrete


gains sufficient strength to support its weight

Employees removing formwork or shoring at elevations in


excess of 6 ft. will use fall protection

Masonry saw blades must be covered with a semicircular


Masonry Tools enclosure to retain blade fragments

Power rotating type concrete troweling machines shall be


equipped with an automatic shut off switch

SCAFFOLDS
Scaffolds shall be designed by a qualified person and shall be
constructed and loaded in accordance with that design.

Scaffolds shall be erected, moved, dismantled, or altered only


under the supervision and direction of a competent person
qualified in scaffold erection, moving, dismantling or
alteration. Such activities shall be performed only by
experienced and trained

Scaffold and components shall be capable of supporting,


without failure, its own weight and at least 4 times the
maximum intended load applied or transmitted to it.

Footings shall be level, sound, rigid, and capable of


supporting the loaded scaffold without settling or
displacement

Each platform on all working levels of scaffolds shall be fully


planked or decked between the front uprights and the
guardrail supports.

Guys, ties, and braces shall be installed at the closest


horizontal member to the 4:1 height and be repeated
vertically at locations of horizontal members every 26 feet or
less thereafter.

The top guy, tie or brace of completed scaffolds shall be


placed no further than the 4:1 height from the top. Such guys,
ties and braces shall be installed at each end of the scaffold
and at horizontal intervals not to exceed 30 feet.

A safe means of access shall be provided for employees


erecting or dismantling scaffolds

Fall protection shall be provided for employees erecting or


dismantling scaffolds

Scaffolds and scaffold components shall be inspected for


visible defects by a competent person before each work shift,
and after any occurrence which could affect a scaffolds
structural integrity

Pile Driving

EQUIPMENT OPERATIONS
Operators will be certified for the equipment operated

Equipment will be inspected monthly and prior to use

Equipment will be equipped with a back-up-alarm as


applicable

Back swing of equipment will be protected by barricade or


identified spotter

All pressure vessels which are a part of, or used with, pile
driving equipment shall meet the requirements of ASME
Pressure Vessels (Section VIII).

Overhead protection , the equivalent of 2 inch planking, that


does not obscure the operators vision shall be provided.

Stop blocks shall be provided for the leads to prevent the


hammer from being raised against the head block.

A blocking device, capable of safely supporting the weight of


the hammer, shall be provided for placement in the leads
under the hammer at all times while employees are working
under the hammer.

Guards shall be provided across the top of the head block to


prevent the cable from jumping out of the sheaves.

When the leads must be inclined in the driving of batter piles,
provisions shall be made to stabilize the leads.

Fixed leads shall be provided with a ladder, and adequate


rings, or similar attachment points, so that the loft worker
may engage his body harness lanyard to the leads . If the
leads are provided with loft platforms, such platforms, shall
be protected by

Air or steam hose leading to a steam or air hammer or jet pipe


shall be securely attached to the hammer with adequate length
of at least 1/4 in. diameter chain or cable to prevent whipping
in the event the joint at the hammer is broken.

Safety chains, or equivalent means, shall be provided for each


hose connection to prevent the line from thrashing around in
case the coupling becomes disconnected.

Steam line controls shall consist of two shutoff valves, one of


which shall be a quick-acting lever type within easy reach of
the hammer operator

Guys, outriggers, thrustouts, or counterbalances, shall be


provided as necessary to maintain stability of pile driver rigs.

Engineers and winchmen shall accept signals only from the


designated signalman.

All Employees shall be kept clear when piling is being


hoisted into the leads.

When piles are being driven in an excavated pit, the walls of


the pit shall be sloped to the angle of repose or sheet piled
and braced.

When steel tube piles are being "blown out", employees shall
be kept well beyond the range of falling materials.

When it is necessary to cut off the tops of driven piles, pile


driving operations shall be suspended except where the
cutting operations are located at least twice the length of the
longest pile from the driver.

or alteration. Such activities shall be performed only by
experienced and trained

Scaffold and components shall be capable of supporting,


without failure, its own weight and at least 4 times the
maximum intended load applied or transmitted to it.

Each platform on all working levels of scaffolds shall be

The Limited Access Zone shall be restricted to entry by


employees actively engaged in the construction of the
wall. No other employees shall be permitted to enter.

Masonry walls over 8 feet in height shall be adequately


braced to prevent overturning and prevent collapse until
permanent supporting elements of the structure are in
place.

Controlled Access
A controlled access area will be created to keep all
unauthorized personnel out of the lift area.

Only employees with assigned responsibilities to the lift will


be permitted in side the lift area. Names of authorized
employees are to be included as part of the prelift meeting

Cameras will require prior approval. Photographers will not


be permitted inside the controlled lift area.

A walk down of the area will be conducted prior to lift to


verify the area is clear of unauthorized personnel.

Communications
A pre-lift meeting will be conducted reviewing the lift plan,
identifying roles and responsibilities, emergency procedures

A clear radio frequency will be established and radio


communications maintained for the duration of the lift

Load Control
Tag line placement, maximum permissible wind speed and
rigging placement are to be included in the lift plan and
