    ABSTRACT Sixty female prawn seed collectors and 60 female control subjects from Sajenakhali and Sandeshkhali blocks of Sunderbans, West Bengal, India, were randomly selected to evaluate and compare musculoskeletal disorders and physiological stress. The control group was engaged in domestic work involving minimum handi ntensive activities. The modified Nordic musculoskeletal questionnaire and rapid entire body assessment were used. Most subjects suffered from discomfort in different body parts, especially in the lower back (98%),knees (88%), shoulders (75%), ankles (70%) and feet (67%). This study reveals that female prawn seed collectors suffer from significant physiological load and extreme physiological stress due to prolonged working hours in a standing posture and excessive work pressure. Consequently, all these factors affect female prawn seed collectors’ health and work performance.

    To estimate cost-effectiveness ratios and net costs of a training intervention to reduce morbidity among porters who carry loads without mechanical assistance in a developing country informal sector setting. Pre- and post-intervention survey data (n = 100) were collected in a prospective study: differences in physical/mental composite scores and pain scale scores were computed. Costs and economic benefits of the intervention were monetized with a net-cost model. Significant changes in physical composite scores (2.5), mental composite scores (3.2), and pain scale scores (-1.0) led to cost-effectiveness ratios of $6.97, $5.41, and $17.91, respectively. Multivariate analysis showed that program adherence enhanced effectiveness. The net cost of the intervention was -$5979.00 due to a reduction in absenteeism. Workplace ergonomic training is cost-effective and should be implemented wherein other engineering-control interventions are precluded due to infrastructural constraints.

    A school bus is used universally to transport school children to and from school. These are considered as the safest mode of transport in city areas in spite of poor ergonomic interior design and predominant unsafe condition. The study was performed among the students to find out their concept of safety regarding the interior design of school buses. For this study a questionnaire was developed and performed among three hundred urban students of eight different schools of Kolkata. The students were divided into three groups of equal strength: primary, middle and high. Each group had fifty male and fifty female students. From the analysis of questionnaire it was observed that all students were facing many unsafe and poor ergonomics related problems that lead to injuries while traveling in the school buses. There is dearth of data regarding the ergonomic studies of the teenage girls working in the manual brick manufacturing units of the unorganized sectors in India. Therefore, a field study was conducted in about four brick manufacturing units of West Bengal, India. Their job in these fields was to carry 8-10 unbaked and sun-dried bricks (49.11 +/- 2.13 Kg) from the brick field to the kiln-top and again carry the baked bricks back from the kiln to some other corner of the brick field, under the hot sun.     Background: Handloom is one of the oldest industries in India, particularly in West Bengal, where a considerable number of rural people are engaged in weaving. Objectives: The purpose of this study was to evaluate the prevalence of low back pain among the handloom weavers in India. Methods: A modified Nordic Musculoskeletal Disorder Questionnaire and Oswestry Low Back Pain Disability Questionnaire along with a body part discomfort scale were administered to handloom weavers (n = 175). Working posture of the participants was assessed using the Ovako Working Posture Analysis System (OWAS). Results: Sixty eight per cent of the participants reported suffering from low back pain, making it the most prevalent disorder in our sample. Analysis of the Oswestry Low Back Pain Disability Questionnaire data revealed that among those with low back pain (n = 119), 2% had severe disabilities, 46% had moderate disabilities, and 52% had minimal disabilities.     The study was undertaken to determine the prevalence of musculoskeletal discomfort among the sawmill workers of Kolkata and to identify the causative factors behind the development of such discomfort. For this study 110 male workers were randomly selected. For the symptom survey, modified Nordic questionnaire on detail discomfort feelings was performed. Analysis of working posture, repetitiveness of work, measurement of handgrip strength, measurement of pre and post working heart rate, measurement of lower back muscle flexibility were performed on the selected sawmill workers suffering from musculoskeletal discomfort. The results revealed that musculoskeletal discomfort was a major problem among the sawmill workers, primarily involving the lower back (100%), neck (95.96%), wrist (87.78%), and shoulder (84.44%).     It is a well recognized fact that manual material handling (MMH) entails enormous physical labor and associated discomfort feeling, which invariably culminate into musculoskeletal disorders. However mental fatigue, in such instances, becomes an unexplored parameter that needs definite attention during assessment of work status of MMH workers. The present study is being performed among 75 porters of a central market in Kolkata to ascertain the extent of physical and mental exhaustion among them. SF-12 health status questionnaire followed by another questionnaire involving psychosocial questions were performed among them. The analysis of both the questionnaires clearly revealed that most of the porters are under severe physical exertion as they alone carry about 120 kg load at a time and such activity is repeated at least 40 times throughout the day.     Occupational driving has often been associated with a high prevalence of back pain. Buses constitute the backbone of the local transportation; so many people are engaged in the profession. The present study was to investigate the prevalence of low back pain (LBP) and due to that problem whether their social and professional life hampers or not. The present study was undertaken among 160 government undertaking male bus drivers. Modified Nordic musculoskeletal questionnaire was performed with physical assessment. Then Oswestry Low Back Pain Disability Questionnaire (QLBPDQ) was performed to evaluate the extent to which social and professional life is restricted by back pain. Afterwards, the sit-and-reach test and investigation of sitting postures was done. From the analysis of questionnaire it was revealed that drivers had to spend 8-10 hours daily for 6 days in a shift.     Duchenne muscular dystrophy (DMD) gene transcripts are most abundant in normal skeletal and cardiac muscle and accumulate as normal myoblasts differentiate into multinucleated myotubes. In this report we describe our initial studies aimed at defining the cis-acting sequences and trans-acting factors involved in the myogenic regulation of DMD gene transcription. A cosmid clone containing the first exon of the DMD gene has been isolated, and sequences lying upstream of exon 1 were analyzed for homologies to other muscle-specific gene promoters and for their ability to direct muscle-specific transcription of chimeric chloramphenicol acetyltransferase (CAT) gene constructs. The results indicate that the transcriptional start site for this gene lies 37 base pairs (bp) upstream of the 5' end of the published cDNA sequence and that 850 bp of upstream sequence can direct CAT gene expression in a muscle-specific manner.     ABSTRACT Occupational driving has often been associated with a high prevalence of neck pain. The aim of this study was to evaluate the prevalence of upper body musculoskeletal disorders among professional non-government city male bus drivers. One hundred ten (110) bus drivers were consecutively enrolled in the study. The modified Nordic questionnaire was used as a basis for data collection during 12 months. The associations between individual characteristics, workstation and organizational risk factors for neck pain and the associations between 12-month prevalence of neck pain and prevalence of pain in adjacent regions were examined. The 12-month prevalence of neck pain was the second highest, followed by: lower back, upper back, hand, shoulder, wrist and elbow pain. The main cases of neck pain were: Strenuous and monotonous job, time pressure, prolonged working hours, low income and excessive work pressure. Consequently these factors affected bus drivers' health and work performance.

    A cross-sectional study of 150 adult Bengalee Hindu male jute mill workers of Belur, a suburb of Kolkata, West Bengal, India, was undertaken to study the relationship of age, body mass index (BMI) and waist circumference (WC) with systolic (SBP), diastolic (DBP) and mean arterial (MAP) blood pressure. The mean age and the BMI of the subjects were 40.7 years (S.D. = 15.2) and 23.2 kg/m2 (S.D. = 3.2), respectively. The mean SBP, DBP and MAP were 124.7 mmHg (S.D. = 7.8), 81.5 mmHg (S.D. = 5.7) and 95.9 mmHg (S.D. = 6.1), respectively. Age had similar significant (p < 0.001) correlations with BMI and WC. Age and WC were significantly correlated (p < 0.001) with all the three blood pressure variables. In general, the correlations of BMI with SBP (r = 0.24, p < 0.01), DBP (r = 0.15, n.s.) and MAP (r = 0.19, p < 0.05) were weaker. Study population comprises of 300 child workers, 100 in each group (agricultural workers, constructional workers, control group or domestic workers with equal sex, age and socioeconomic distribution).     Stonecutting and setting are important tasks in a construction site. A field study was conducted to assess occupational-related disorders and to conduct ergonomics assessment among stonecutters. The study focused on (a) the duration of work per day, (b) the working environment and working activities and (c) the feeling of discomfort in different parts of the body. A detailed posture analysis was performed with the Ovako working posture analysis system (OWAS). It was observed that stonecutters worked continuously in awkward postures during stonecutting and setting. Consequently, they suffered from discomfort in different parts of their body, specifically in the lower back, knees and shoulders, which mainly prevented them from continuing their work. This study also revealed that stonecutters had to work in congested work areas with a poor level of illumination. The noise level and dust particles emitted during stonecutting activities could affect stonecutters.

    The sand core making process is performed manually in West Bengal involving a large number of workers of lowest economic strata. The core making workers most often work for a prolonged period of time and they are forced to handle various amounts of heavy load during the entire period of work. In this study an attempt was made to identify the work related musculoskeletal disorders among the sand core-making workers. Fifty male workers engaged in carbon dioxide and chemical core making work at an unorganized sector at Baruipur, Calcutta were randomly selected for this study.     An investigation of 150 adult Bengalee Hindu male jute mill workers in Belur, a suburb of Kolkata, West Bengal, India, was conducted to study the relationship between central obesity and blood pressure. In accordance with their waist circumference measurement, the subjects were divided into two categories: centrally non-obese (CNO) and centrally obese (CO). The participants were classified as the CO group if they had a WC of 80 cm or more. Results showed that none of the CNO subjects was mild hypertensive (SBP>/=140 mmHg and/or DBP>/=90 mmHg) while 85 of the CO subjects (82.5%) were mild hypertensives, the difference being statistically significant (chi-square=9.33; p<0.0025). Moreover, the data also revealed that the CO subjects had much (p<0.001) greater mean weight, body mass index (BMI), systolic (SBP), diastolic (DBP) and mean arterial (MAP) blood pressure than the CNO group members. The significant difference in blood pressure was found even after correcting the confounding effects of age and BMI variables. The results of this study showed that, the Bengalee male jute mill workers in the CO group had significantly higher blood pressure irrespective of age and overall adiposity (BMI). Therefore, the presence of central obesity is deemed a risk factor, for hypertension regardless of age and BMI. Thus, a WC cut-off point of 80 cm could be employed for health promotion among Bengalee men so as to prevent and manage hypertension effectively.

    Background: Chikan embroidery is a popular handicraft in India that involves hand-intensive stitching while seated in a static posture with the upper back curved and the head bent over the fabric. Women perform most Chikan embroidery. Objectives: The aim of this study was to analyze the repetitive nature of this work among female Chikan embroiderers by measuring the prevalence of upper extremity discomfort and carpal tunnel syndrome (CTS). Methods: The Nordic musculoskeletal questionnaire was used to analyze the extent of upper extremity pain symptomology. The repetitive nature of Chikan embroidery work was evaluated using the Assessment of Repetitive Tasks of the upper limbs tool (ART). Motor nerve conduction studies of median and ulnar nerves were performed with embroidery workers and a control group to determine the risk of CTS. Results: Among embroidery workers, the prevalence of wrist pain was 68% and forearm pain was 60%. The embroiderers also commonly reported Tingling and numbness in the hands and fingertips. The ART analysis found that Chikan embroidery is a highly repetitive task and nerve conduction studies showed that the embroidery workers were more likely to experience CTS than women in the control group. Conclusions: Chikan embroidery is a hand-intensive occupation involving repetitive use of hands and wrists and this study population is at risk of experiencing CTS. Future research should explore the potential benefits of ergonomics measures including incorporating breaks, stretching exercises, and the use of wrist splints to reduce repetitive strain and the probability of developing CTS.

    One of the main activities of the goldsmiths is Blowing Pipe. The blowpipe is to expand and shape the gold beads by means of positive pressure produced by careful, controlled oral expiration. Objective of this study was to assess the effect of ergonomic intervention on facial muscle fatigue and respiratory stress of goldsmiths to continue their work. For this current study 100 male goldsmiths were selected from the Davangere District of Karnataka. This study used the questionnaire, Examination of Pulmonary Function, Measurement of peak expiratory flow rate and electromyography of three major facial muscles. Three primary types of ergonomic interventions were provided to the goldsmiths. Subjects were interviewed at the end of each week to ascertain intervention acceptance. From the analysis of subject's preference, the hand air pipe was selected by. A large number of goldsmiths complained of respiratory symptoms in this industry. Reduced lung volumes and peak expiratory flow rates of goldsmiths was found, presumably from heavy pressure generated by using blow pipe. This work habit also increases the fatigue of facial muscles, at the end of the day. The purpose of this study was to evaluate the prevalence of low back pain among the handloom weavers in India. Improving upon the weaver's work-posture could improve their quality of life.

    The manufacture of surgical instruments is one of the leading small-scale industrial sectors in West Bengal, India. The present study was undertaken to assess the rate, type and cause of injury incidents among surgical blacksmiths and whether these incidents affected the work performance of the blacksmiths. A cluster of 216 skilled and 225 unskilled blacksmiths (male), engaged in the manufacture of surgical instruments, was selected from Baruipur subdivision as study subjects. The study included: 1) completion of a questionnaire; 2) measurement of physical parameters; 3) incident records; 4) statistical analysis of the data. The present study revealed that the blacksmiths suffered very frequently from work-related injuries. The number of injuries that occurred during 2004 - 2005 for skilled and unskilled blacksmiths was 1413 and 1610 respectively. Unskilled blacksmiths were disproportionately affected. These injuries resulted in a high rate of lost workdays, i.e. 517 and 742 workdays for skilled and unskilled workers respectively. The study thus indicated that surgical blacksmiths are highly prone to injuries in their occupation, mostly affecting the fingers (23% and 23%) and back region (21.7% and 22%) in both groups, which consequently affected their health, productivity and work performance.

    It is important to understand what motivates workers and the extent to which the organization and other contextual variables satisfy them. The aim of the study was to determine factors of dissatisfaction resulting from the organizational culture among the blacksmiths involved in the surgical instrument industry. Fifty male surgical blacksmiths each of the skilled and unskilled groups of the forging section were selected. Organizational Role Stress Scale was used to measure the individuals' role stress and several forms of conflict within an organization. Also, the organizational culture and personal involvement in an organization was measured among the surgical blacksmiths. The mean score for total role stress for Skilled was 71.7 and for unskilled was 77.2. The most frequent type of organizational culture was reported to be hierarchy, both by skilled and unskilled surgical blacksmiths, followed by market and clan culture. This study shows that the skilled surgical blacksmiths have lower level of stress and conflicts in comparison with unskilled surgical blacksmiths. Both skilled and unskilled surgical blacksmiths estimated their level of personal involvement as low and indicated insufficient involvement in work teams. The satisfaction of the employees with their status and role in the organizational culture was also poor for both skilled and unskilled surgical blacksmiths.

    Lung function tests have become an integral part of assessment of pulmonary disease. Diseases of the respiratory system induced by occupational dusts are influenced by the duration of exposure. The aim of the study is to investigate the impairment of lung function and prevalence of respiratory symptoms among the rice mill workers. A total of 120 rice mill workers from three districts of Karnataka were included in this study. Fifty urban dwellers from the same socio-economic level were selected as controls. The study included clinical examination, assessment of respiratory symptoms, pulmonary function test, measurement of peak expiratory flow rate, absolute eosinophil count, ESR estimation, total IgE estimation and radiographic test. The present study has shown that the rice mill workers complained of several types of respiratory disorders like phlegm (40.8 %), dyspnea (44.2 %), chest tightness (26.7 %), cough (21.7 %), and nose irritation (27.5 %). Rice mill workers exposed to dust presented significantly (p < 0.05) lower levels of FVC (3.44 ± 0.11), FEV1 (2.73 ± 0.15) and PEFR (304.95 ± 28.79) than the controls. The rice mill workers are having significantly higher absolute eosinophil counts, total IgE and ESR than control groups. The hematological findings suggest that the harmful effects may be linked to both non-specific irritation and allergic responses to rice husk dust among rice mill workers. Dust exposure in the working environment affects the lung function values and increased the respiratory symptoms among the rice mill workers.

    This study was undertaken among 100 randomly selected bus conductors from 2 routes. A questionnaire study based on the modified Nordic musculoskeletal questionnaire, assessment of physical and physiological parameters, analysis of working postures and a detailed work study were performed. The analysis revealed that conductors had a work schedule of 16-18 h each day; the duration of work could vary from 15 to 20 days at a stretch. Discomfort leading to musculoskeletal disorders mainly affecting the leg (93.3%), knee (83.3%), shoulder (80%) and back areas (56.7%) had the highest 12-month prevalence rates and increased day by day. The conductors also suffered from extreme physiological stress due to prolonged working hours in hazardous standing posture conditions, excessive work pressure and minimum rest between trips. Consequently, all those factors affected their health and work performance.
