Healthcare's economic challenges, corporate mergers, and technological innovations marked the last decade of the 20th century. Both consumers and providers of maternity care faced dramatic changes in reimbursement, which threatened the quality and scope of care provided to childbearing women, children, and families. For nurses in some institutions, this meant decreases in the number of RNs caring for patients and challenges to meet patients' needs with the focus on a black bottom line, fiscal profitability rather than on the patient. New York's Mount Sinai Hospital adopted a philosophy of patient-focused care. This article describes the 5-year journey to redesign a traditional, provider-focused obstetric and pediatric program, into a new patient-focused, family-centered maternal-child healthcare center. This study examines the relationship between perceptions of structural empowerment and anticipated turnover among critical care nurses. The literature revealed that work environments that provide access to information, resources, support and opportunity create an environment of more satisfied employees, ultimately reducing turnover. A sample of 257 nurses completed a background data questionnaire, the Conditions of Work Effectiveness Questionnaire-II (CWEQ-II) and the Anticipated Turnover Scale (ATS). Nurses in five critical care units perceive themselves to be moderately empowered. Structural empowerment was inversely related to anticipated turnover; those who were more empowered had a lower anticipated turnover score. Nurses who perceive themselves as empowered have higher levels of organizational commitment. Organizations that support and recognize these characteristics may experience improved retention rates. This study is the first to demonstrate a relationship between structural empowerment and anticipated turnover among critical care nurses. Research has demonstrated that workplace behaviours and attitudes are determined by social structures within the workplace. Nurses are responsible for a large portion of health care provided in this country. A serious nursing shortage is expected in the future and will exert pressure on both the supply and the demand of nurses.

Although there has been previous research linking perceptions of empowerment to specialty certification among critical care nurses, no research in this area has been conducted among nationally certified perinatal nurses. A comparative quantitative survey design was used. Participants included 80 perinatal registered nurses from one community hospital in New York. Registered nurses who were nationally certified in a perinatal specialty had higher total empowerment scores and higher scores on five of the six subscales. This study supports the results of previous studies focused on differences in empowerment among nationally certified nurses and nurses without national certification. Because much of the preparation for certification is done within continuing education, nurse leaders must be cognizant of the value of certification and must develop and implement programs to support certification in the workplace.
The purpose of the study was to examine the effect of a holistic program, the Collaborative Care Model (CCM) Program, and the development of a self-care plan on health-promoting behaviors in hospital nurses.

Shared decision-making has become a hot topic in healthcare. This article describes a step-by-=step, 'how to,' approach to implementation of an interprofessional shared governance and relationship-based care model at a 300 bed tertiary academic medical centre in a large metropolitan area in the US and outlines the lessons learned.  The project, undtaken in 'waves,' encompassed inpatient and ambulatory services and has been sustained for over eight years.

Globalization has been the hallmark of the 21st century. This article focuses on developing the Transcultural Nursing Leadership Institute (TCNLI) in China. This project built a leadership program in Wenzhou, China, empowering and supporting nurses to solve problems in their own practices with evidence-based approaches and local resources using the Dreyfus International Health Foundation's method Problem Solving for Better Health (PSBH).The partnership began when I was a Visiting Professor in Wenzhou, China and established collegial relationships with the Dean of the School of Nursing and the Chief Nursing Officers of the affiliated hospitals. In contrast to previous visiting scholars who went to China to lecture on health issues, I sought to develop a sustainable program and make a lasting contribution to the nursing practice in Wenzhou. The purpose of this article is to explore the concept of perinatal grief and evidence-based healing interventions for it. The loss of a pregnancy or death of an infant causes profound grief, yet society has long minimized or ignored this grief, which is among the most painful of bereavement experiences. Throughout the last century, research on grief and the special needs of bereaved parents has changed the context of professional intervention from protective to supportive. The central focus of bereavement interventions is to assist families in healing by helping them make meaning of their losses. The use of symbols, spirituality, and rituals has been shown to help bring meaning. Research has shown that memories are key to healing, and that gender, age, and relationships bring different grief expressions and experiences. To describe and interpret the culture of an online perinatal loss group. This qualitative study used ethnography, the study of culture. Methods included participant-observation, review of 447 e-mails, and participants' feedback about the findings. The setting was online in a perinatal loss listserv consisting of mothers and one grandmother who had experienced a perinatal loss through miscarriage, stillbirth, or neonatal death. In this changing group, there were between 82 to 87 participants from North America, Europe, Asia, and Australia. The essence of the culture was Shared Metamorphosis. The Internet connected grieving women who otherwise would likely not have met. Participants shared virtual identities, created a community, and brought meaning to their perpetual losses. Their grief was a process of remembrance and memories, and they used symbols to represent the deceased babies; angels were a commonly discussed symbol. For most of history, the care of women, particularly childbearing women, has been delivered by healers and health care providers who were women. This article is an overview of the historical, social, political, economic, and philosophic forces that shaped the role of women, especially midwives, who care for childbearing families. As healers and health care providers, midwives (and later, nurse-midwives) have based their practice on the natural process of pregnancy and birth. Their holistic view dominated childbirth until the 18th century, when the existence of midwives began to be challenged by institutions. In the United States, the marriage of midwifery with nursing in the 1920s, followed by the consumer childbirth revolution of the 1960s, gave new strength to the profession of nurse-midwifery.
