Nutrition and Cancer

The health promoting effect of various fruits and vegetables is due to the presence of the plethora of bioactive compounds which have preventive and anti-proliferative activity. However, not much has been documented about functional foods in prevention and treatment of cancer. In this chapter, we have reviewed plant based functional foods which have preventive and therapeutic potential against various cancers. Several natural bioactive compounds and their association with functional foods have been enlisted with potency to prevent and/or treat cancer. Some of the bioactive compounds along with the mode of action with anti-cancer activity are curcumin, resveratrol, ellagic acid, tannic acid, chlorogenic acid, ferulic acid, caffeic acid, and kaempferol. Their role in cancer prevention has been discussed in depth.

Emergingonthehorizonincancertherapyisanexpansion of the scope of treatment beyond cytotoxic approaches to in- clude molecular management of cancer physiopathology. The goal in these integrative approaches, which extends be- yond eradicating the affected cells, is to control the cancer phenotype. One key new approach appears to be modula- tion of the inflammatory cascade, as research is expanding that links cancer initiation, promotion, progression, angiogenesis, and metastasis to inflammatory events. This article presents a literature review of the emerging relation- ship between neoplasia and inflammatory eicosanoids (PGE2 and related prostaglandins), with a focus on how inhibition of their synthesizing oxidases, particularly cyclooxygenase (COX), offers anticancer actions in vitro and in vivo. Al- though a majority of this research emphasizes the pharma- ceutical applications of nonsteroidal anti-inflammatory drugs and selective COX-2 inhibitors, these agents fail to address alternate pathways available for the synthesis of pro- inflammatory eicosanoids. Evidence is presented that sug- gests the inhibition of lipoxygenase and its by-products— LTB4, 5-HETE, and 12-HETE—represents an overlooked but crucial component in complementary cancer therapies. Based on the hypothesis that natural agents capable of mod- ulating both lipoxygenase and COX may advance the effi- cacy of cancer therapy, an overview and discussion is presented of dietary modifications and selected nutritional and botanical agents (notably, omega-3 fatty acids, antioxi- dants, boswellia, bromelain, curcumin, and quercetin) that favorably influence eicosanoid production.

This review focuses on selected aspects of the relation between alcohol consumption and cancer risk. Heavy alcohol consumption (i.e., ≥4 drinks/day) is significantly associated with an increased risk of about 5-fold for oral and pharyngeal cancer and esophageal squamous cell carcinoma, 2.5-fold for laryngeal cancer, 50% for colorectal and breast cancers, and 30% for pancreatic cancer. These estimates are based on a large number of epidemiological studies and are generally consistent across strata of several covariates. The evidence suggests that at low doses of alcohol consumption (i.e., ≤1 drink/day) the risk is also increased by about 20% for oral and pharyngeal cancer and 30% for esophageal squamous cell carcinoma. Thus, for these sites there is little evidence of a threshold effect. While consumption of fewer than 3 alcoholic drinks/wk is not associated with an increased risk of breast cancer, an intake of 3 to 6 drinks/wk might already yield a (small) increase in risk. No studies have investigated dietary differences between head and neck squamous cell carcinoma (HNSCC) patients with human papillomavirus (HPV)-positive tumors and patients with HPV-negative tumors. This study was designed to investigate the relationship between diet and HPV status in HNSCC patients. Cases of HNSCC were recruited from 2 clinical centers participating in the University of Michigan Head and Neck Specialized Program of Research Excellence (SPORE). HPV tissue genotyping was performed, and epidemiological and dietary data collected. Multivariable logistic regression tested whether pretreatment consumption of 12 selected micronutrients was significantly associated with HPV-positive status in 143 patients newly diagnosed with cancer of the oral cavity or pharynx. Chlorophyllin (CHL), a water soluble semisynthetic derivative of the ubiquitous plant pigment chlorophyll used as a food additive, is recognized to confer a wide range of health benefits. CHL has been shown to exhibit potent antigenotoxic, anti-oxidant, and anticancer effects. Numerous experimental and epidemiological studies have demonstrated that dietary supple-mentation of CHL lowers the risk of cancer. CHL inhibits cancer initiation and progression by targeting multiple molecules and pathways involved in the metabolism of carcinogens, cell cycle progression, apoptosis evasion, invasion, and angiogenesis. The modulatory effects of CHL on the hallmark capabilities of cancer appear to be mediated via abrogation of key oncogenic signal transduction pathways such as nuclear factor kappa B, Wnt/β-catenin, and phosphatidylinositol-3-kinase/Akt signaling. This review provides insights into the molecular mechanisms of the anticancer effects of dietary CHL.
