Splints

The purpose of this study was to investigate the effectiveness of Johnstone pressure splints (JPSs) on spasticity and cutaneous sensory inputs in children with spastic cerebral palsy (CP). Thirty-four children with spastic diplegic CP participated in this study. Children whose motor development levels were similar were divided into a treatment and a control group. Each group consisted of 17 participants (six females and 11 males). Mean age of the treatment group was 48.82 months (SEM 4.42), and the control group, 47.52 months (SEM 5.27). The treatment group underwent Bobath's neurodevelopmental therapy (NDT) combined with JPSs. The control group underwent NDT alone five days a week for three months. Before and after treatments, lower-extremity passive range of motion (ROM) by goniometric measurements, spasticity by Modified Ashworth Scale (MAS), and somatosensory evoked potentials (SEPs) were measured. Passive ROM showed significant improvements in both groups (p<0.01). In the treatment group, all MAS scores increased. In the control group, the difference was significant except for values of internal rotator muscles. Improvements in passive ROM in the treatment group were significantly higher than the control group except in hip abduction and external rotation (p<0.05). MAS scores of the treatment group were significantly higher than the control group (p<0.05). SEP values increased in both groups but values of the treatment group were significantly higher than the control group (p<0.05).

To compare the efficacy of acupuncture with night splinting for carpal tunnel syndrome (CTS). Sixty one mild-to-moderate degree CTS patients, aged 27-67 were randomly assigned to acupuncture (Acu) and night splinting (NS) groups. The Acu group received 10 sessions of electro-acupuncture twice a week. The NS group received prefabricated volar neutral wrist splint during the night for 5 weeks. Outcomes were assessed at baseline and the end of treatment protocol by Boston Carpal Tunnel Scale comprising symptom severity scale (SSS) and functional status scale (FSS). Pain was measured by 100 mm visual analog scale (VAS). VAS decreased more in Acu than in the NS group (p = 0.028) whereas improvements in SSS and FSS were not significant between both groups. Electro-acupuncture was as effective as night splinting in respect of overall symptoms and functions in mild-to-moderate degree CTS. However, pain was reduced more by electro-acupuncture than night splinting.

Introducción: Los traumatismos dentoalveolares en la población pediátrica son frecuentes por lo cual el odontólogo debe estar capacitado para brindarle atención y un oportuno tratamiento. Las lesiones traumáticas pueden ser intencionadas, resultado de traumatismos directos o indirectos. Entre las etiologías predominantes encontramos las caídas, siendo el diente más afectado el incisivo central superior con un 80 %. El objetivo principal es la reposición y estabilización del segmento óseo mediante férulas para limitar su movimiento y permitir la curación del ligamento periodontal y cicatriz ósea. El pronóstico varía de acuerdo a la condición de cierre apical, tipo de traumatismo y el tiempo transcurrido. Se debe valorar de manera constante, clínica y sintomatológica, los cambios de sensibilidad y de coloración. Reporte de caso: En este trabajo se expone el diagnóstico, tratamiento y progreso, de un niño de 8 años de edad que presenta una luxación palatina dentaria de la pieza 1.1, para lo cual se realizó una reducción mecánica y fijación rígida. Conclusión: Aunque el medio de fijación suele presentar alteración a nivel del tejido periodontal se obtuvo una evolución satisfactoria.

Whether a patient with burn injury is an adult or child, contracture management should be the primary focus of burn rehabilitation throughout the continuum of care. Positioning and splinting are crucial components of a comprehensive burn rehabilitation program that emphasizes contracture prevention. The emphasis of these devices throughout the phases of rehabilitation fluctuates to meet the changing needs of patients with burn injury. Early, effective, and consistent use of positioning devices and splints is recommended for successful management of burn scar contracture.

Management of a wide variety of musculoskeletal conditions requires the use of a cast or splint. Splints are noncircumferential immobilizers that accommodate swelling. This quality makes splints ideal for the management of a variety of acute musculoskeletal conditions in which swelling is anticipated, such as acute fractures or sprains, or for initial stabilization of reduced, displaced, or unstable fractures before orthopedic intervention. Casts are circumferential immobilizers. Because of this, casts provide superior immobilization but are less forgiving, have higher complication rates, and are generally reserved for complex and/or definitive fracture management. To maximize benefits while minimizing complications, the use of casts and splints is generally limited to the short term.
