Purpose: To compare thickness measurements of the central 6.0 mm of the cornea obtained with the Orbscan® II topography system and topographical ultrasound pachymetry. Setting: School of Optometry, University of Santiago de Compostela, Galicia, Spain. Methods: In 24 right eyes, pachymetric measurements were taken at the center and 1.2 mm and 3.0 mm on the superior and inferior hemimeridians. A 1‐sample t test was applied to assess the significance of the relationship between Orbscan II and ultrasound methods. This study examines the functional morphology of the olecranon process in hominoids and fossil hominins. The length of the bony lever of the triceps brachii muscle (TBM) is measured as the distance between the trochlear articular center and the most distant insertion site of the TBM, and olecranon orientation is measured as the angle that this bony lever makes with the long axis of the ulna. Results show that Homo, Pan, Gorilla, most monkeys, and the Australopithecus fossils studied have similar relative olecranon lengths. Suspensory hominoids and Ateles have shorter olecranons, suggesting, in some instances, selection for greater speed in extension. The orientation that the lever arm of the TBM makes with the long axis of the ulna varies with preferred locomotor mode. Terrestrial primates have olecranons that are more posteriorly oriented as body size increases, fitting general models of terrestrial mammalian posture. Arboreal quadrupeds have more proximally oriented lever arms than any terrestrial quadrupeds, which suggests use of the TBM with the elbow in a more flexed position. Olecranon orientation is not consistent in suspensory hominoids, although they are all characterized by orientations that are either similar or more posterior than those observed in quadrupeds. Homo and the fossils have olecranons that are clearly more proximally oriented than expected for a quadruped of their size. Résumé À défaut d’un champ littéraire francophone proprement dit, il existe un système francophone dont le fonctionnement peut être analysé à partir de la relation réciproque de centre à périphérie. Au sein de ce système, on peut distinguer, par rapport au centre franco-parisien, différentes positions qui déterminent certaines stratégies rhétoriques de la part des auteurs de la périphérie, lesquelles agissent à leur tour sur le mode de réception de ces oeuvres par le centre (assimilation ou exotisation). Parmi toutes les stratégies rhétoriques employées, le discours identitaire apparaît à la fois comme un vecteur privilégié et comme une condition de reconnaissance par le centre, dont la demande pour l’exotisme est fort importante à l’égard de certaines zones périphériques.

Adrenalectomy continues to play an important role in the management of Cushing’s syndrome (CS). Untreated CS causes considerable physical and mental morbidity and mortality. However, little information is available on the effect of adrenalectomy in ameliorating functional disabilities in CS patients. Our study assesses the long-term outcome of adrenalectomy in patients with CS. This is a retrospective analysis of CS patients managed during 1990–2005 at a tertiary care center. We analyzed the clinical presentation, endocrine evaluation, and surgical management preoperatively and following adrenalectomy. The subjects were 37 patients with CS (age 24.5 ± 15 years, range 1–60 years; male:female 1.0:1.2). There were various etiologies—unilateral adrenocortical adenoma (n = 11), adrenocortical carcinoma (n = 13), pituitary ACTH-secreting adenoma with failed transsphenoidal surgery (n = 4), ectopic unidentified ACTH source (n = 7), bilateral adrenal macronodular hyperplasia (n = 1), primary pigmented nodular adrenal hyperplasia (n = 1) —for which the patients underwent adrenalectomy: unilateral (n = 22), bilateral (n = 13), or adrenonephrectomy (n = 2). Two patients died during the perioperative period owing to chest infection and sepsis. At the median follow-up of 60 months (range 6–144 months), the patients exhibit significant persistence of obesity (41%), proximal muscle weakness (44%), menstrual irregularity (8%), hypertension (31%), and insulin-dependent diabetes (29%). Hirsutism and psychological abnormalities persisted to a lesser extent. All patients had biochemical cure of CS following surgery evidenced by the 8 a.m. basal cortisol ≤ 5 μg/dl. The hypothalamic-pituitary-adrenal axis recovered as shown by normalization of the short synacthen-stimulated cortisol level (peak level ≥ 20 μg/dl) after a median follow-up of 9 months (range 6–18 months). Incomplete clinical recovery following adrenalectomy emphasizes the need of early recognition and prompt treatment of CS. Surgery for adrenocortical adenoma is safe and effective; however, survival of patients with CS due to adrenocortical carcinoma remains poor. Bilateral adrenalectomy provides early control of hypercortisolism in selected cases of unlocalized ectopic ACTH syndrome or failed transsphenoidal surgery. Even though functional recovery is incomplete after adrenalectomy, quality of life improves considerably.

The history of Castanea sativa (sweet chestnut) cultivation since medieval times has been well described on the basis of the very rich documentation available. Far fewer attempts have been made to give a historical synthesis of the events that led to the cultivation of sweet chestnut in much earlier times. In this article we attempt to reconstruct this part of the European history of chestnut cultivation and its early diffusion by use of different sources of information, such as pollen studies, archaeology, history and literature. Using this multidisciplinary approach, we have tried to identify the roles of the Greek and Roman civilizations in the dissemination of chestnut cultivation on a European scale. In particular, we show that use of the chestnut for food was not the primary driving force behind the introduction of the tree into Europe by the Romans. Apart from the Insubrian Region in the north of the Italian peninsula, no other centre of chestnut cultivation existed in Europe during the Roman period. The Romans may have introduced the idea of systematically cultivating and using chestnut. In certain cases they introduced the species itself; however no evidence of systematic planting of chestnut exists. The greatest interest in the management of chestnut for fruit production most probably developed after the Roman period and can be associated with the socio-economic structures of medieval times. It was then that self-sufficient cultures based on the cultivation of chestnut as a source of subsistence were formed.

Debates on the subject of cultural complexity and its material manifestations are situated at the centre of research on prehistoric pastoralism in North Africa. Employing already published databases, this article integrates raw data from archaeological sites across the Sahara with ethnography to generate a framework of analysis in which changes in material culture can be interpreted. It attempts to establish a relationship between the analysis of human and cattle remains in order to study (a) the relations between modes of interment of animals and of humans, and social changes, and (b) the processes responsible for the appearance of a symbolism of power in the mid- and late Holocene funerary rituals. Their integration with landscape systems results in a conclusion of complex patterns of cultural diversity which question previous dismissals of early Saharan pastoral-is is as the progenitors of social complexity.

Two processes characterize the later precontact history (twelfth-fourteenth centuries) of the northern part of the American Southwest: aggregation of people into large towns and depopulation of large regions. These processes have been explained as the result of environmental, economic, and social factors, including drought and warfare. Using a theoretical perspective based on Pauketat’s “historical processualism,” we argue that aggregation and depopulation are partly the result of historical developments surrounding the expansion and collapse of the Chaco regional system. We present our understanding of the Chaco regional system from the perspective of historical processualism; then, historical developments in the northern San Juan and Cibola regions-northern and southern frontiers of the Chaco world-are compared. Atrial fibrillation (AF) has a deleterious impact on health-related quality-of-life (HRQoL), but measuring this outcome is difficult. A comprehensive, validated, disease-specific questionnaire to measure the spectrum of QoL domains affected by AF and its treatment is not available. We developed and validated a 20-item questionnaire, Atrial Fibrillation Effect on QualiTy-of-life (AFEQT), in a 6-center, prospective, observational study. Factor analyses established 4 conceptual domains (Symptoms, Daily Activities, Treatment Concern, and Treatment Satisfaction) from which individual domain and global scores were calculated. Medieval archaeological findings made of pietra ollare (basic and ultrabasic metamorphic rocks belonging to the greenschist facies) and coming from central–eastern Italy have been characterized through a petrological study (modal mineralogy, whole-rock geochemistry, XRD and SEM–EDS analyses). The pietra ollare artefacts considered in this work consist of fine-grained, grey to pale-grey, magnesite-bearing talc-schists (i.e., soapstones). In order to determine their production centres, Alpine soapstones quarried in ancient times were selected on the basis of their mineralogical and textural compatibility with the archaeological findings. The mineralogy and chemistry (major and trace elements) contributed to establishing the ancient quarries of the Valchiavenna (central Alps) as the probable provenance area. This archaeometric investigation proves that Alpine artefacts made of pietra ollare spread to the south of the Po Plain during the Middle Ages. It can be inferred that the Alpine soapstone trade towards the Marche and Abruzzo regions was addressed through the main waterways: Lake Como, the Adda and Po Rivers and finally the Adriatic Sea.
