Background: The Neurodegenerative diseases are increasingly affecting the elderly with a severe impact on their brain health. There is a wide gap in supplementation based studies for increasing the cognition levels of the geriatric population especially in the developing countries like India, which are at extreme risk of developing neurological disorders. Vitamin B12 herein has caught much attention lately for improving the cognitive status. Literature has linked the possibility of alleviating neurological disorders in the elderly with effective vitamin B12 management. Abundant animal and human models have proved that supplementation of vitamin B12 is beneficial for the restoration of cognitive functions. Objective: To supplement vitamin B12 deficient mild cognitively impaired geriatric patients with injectable doses of vitamin B12 followed by impact evaluation.

Methods: Screening of the mild cognitively impaired patients was carried out using the Mini-Mental State Examination and Yamaguchi Fox Pigeon Imitation test. Baseline information was elicited from the patients residing in urban Vadodara (a district in the state of Gujarat), India. This included socio-demographic, medical and drug history, anthropometric and physical activity pattern as well as biochemical parameters comprising of serum vitamin B12 and glycated haemoglobin profile. A sub-sample of 60 patients with mild cognitive impairment (MCI) demonstrating severe vitamin B12 deficiency were conveniently enrolled for injectable doses of Vitamin B12 in the dosage of 1,000 μg every day for one week, followed by 1,000 μg every week for 4 weeks & finishing with 1,000 μg for the remaining 4 months. Post six months intervention all the parameters were elicited.

Results: Vitamin B12 supplementation resulted in a significant (p<0.001) improvement in the MMSE scores of the patients with a rise of 9.63% in the total patients. Gender-wise division also highlighted a significant increase (p<0.001) in the scores by 6.79% and 12.46% in overall males and females and a 10.20% and 8.24% rise for young-old (60-69 yrs) and old- old (70-85 yrs) categories, respectively. As a result, 27 patients progressed towards the category from the MCI state being assessed by MMSE scores. In the same manner, YGFPIT too demonstrated a 38% increase in normal with 35% males, 42% females, 41% young–old and 31% old-old moving to normal status. Thus, a total number of 28 patients progressed to the normal condition as per YGFPIT.

Conclusion: Hence, vitamin B12 supplementation was found significantly effective in placing the serum vitamin B12 of MCI patients from the deficiency state to sufficient levels and in turn increased their performance in MMSE and YFPIT scores.

Folic acid and cobalamin are B-group vitamins that play an essential role in many cellular processes. Deficiency in one or both of these vitamins causes megaloblastic anaemia, a disease characterized by the presence of megaloblasts. Megaloblasts occur when inhibition of DNA synthesis causes asynchronous maturation between the nucleus and the cytoplasm. Clinical manifestations are similar to those of other types of anaemia, with the exception of cobalamin deficiency megaloblastic anaemia, which presents distinctive neurological symptoms. An understanding of the metabolism of these vitamins will enable clinicians to make the best use and interpretation of laboratory studies and monitor therapeutic strategies, which consist mainly of administering supplements to restore body reserves.

Studies have shown that nutritional intake and metabolism may play an important role in the causes and treatment of several female reproductive health issues. The water soluble vitamins B6, B12 and C have been suggested to play important roles in maternal and reproductive health in general. This study is therefore aimed at investigating the direct effects of the water soluble vitamins on uterine contractility using ex-vivo assays and on female reproductive function using non-pregnant mice models. The effect of the vitamins ascorbic acid (ASA), cobalamin (CBL) and vitamin B complex (VBCo) on spontaneous, oxytocin (OT)-induced uterine contractility and high KCl-induced (80 mM) tonic contractions were examined. ASA (0.1 – 444.40 µg/ml), CBL (0.0005 – 2.22 µg/ml) and VBCo (0.03-124.43 µg/ml) were observed to produce concentration-dependent inhibition of spontaneous uterine contractility. On OT-induced uterine contractility, ASA (0.07 – 777.78 µg/ml), CBL (0.004 – 0.89 µg/ml) and VBCo (0.19 – 217.76 µg/ml) were observed to inhibit OT-induced uterine contractility with CBL and VBCo showing a significant (p < 0.05) reduction in frequency. No change was however observed on the effect of ASA, CBL or VBCo on tonic uterine contraction produced by high KCl. On effect of reproductive hormone levels, CBL was observed to have significantly decreased (p < 0.0001) plasma levels of oestradiol (E3) and progesterone (P4). On reproductive tissues in the presence of CBL (1.0 mg/kg), hypertrophy of the cervix, a reduction in the number and size of the uterine glands and matured follicles in the ovaries were observed. These observations show a significant role for the water soluble vitamins in female reproductive function.

A nutritional deficiency in younger age group includes both

Dementia is a worldwide health problem which affects millions of patients; Alzheimer's disease (AD) and subcortical vascular dementia (sVAD) are the two most frequent forms of its presentation. As no definite therapeutic options have been discovered, different risk factors for cognitive impairment have been searched for potential therapies. This report focuses on the possible evidence that vitamin D deficiency and hyper-homocysteinemia can be considered as two important factors for the development or the progression of neurodegenerative or vascular pathologies. To this end, we assessed: the difference in vascular risk factors and vitamin D-OH25 levels among groups of sVAD, AD, and healthy age-matched controls; the association of folate, B12, homocysteine, and vitamin D with sVAD/AD and whether a deficiency of vitamin D and an increment in homocysteine levels may be related to neurodegenerative or vessel damages. The commonly-considered vascular risk factors were collected in 543 patients and compared with those obtained from a healthy old volunteer population. ANOVA group comparison showed that vitamin D deficiency was present in demented cases, as well as low levels of folate and high levels of homocysteine, more pronounced in sVAD cases. The statistical models we employed, with regression models built, and adjustments for biochemical, demographic and neuropsychiatric scores, confirmed the association between the three measures (folate decrease, hyperhomocysteinemia and vitamin D decrease) and dementia, more pronounced in sVAD than in AD.

Background: Reticulocytes are the youngest erythrocytes released from the bone marrow into the blood and they circulate for 1-2 days before becoming mature erythrocytes. In literature, there were studies about reticulocyte parameters that could help in differentiation of iron deficiency anemia (IDA) from vitamin B12 deficiency anemia. However, in those studies there were no data about differentiation of mixed anemia (vitamin B12 deficiency and IDA). The purpose of this study is to explore a response to 'could reticulocyte parameters help in differential diagnosis of mixed anemia?' in 6-12 years old children.

Methods: The study enrolled 26 patients with IDA, 22 patients with mixed anemia, 32 patients with vitamin B12 deficiency, and 32 age and gender matched healthy controls. Blood for hematological parameters such as complete blood count, reticulocyte count, CHr, MCVr, CHCMr were collected into standard tubes containing EDTA.

Results: There is a statistically significant difference of both MCV (mean corpuscular volume)/MCVr (Reticulo-cyte mean corpuscular volume) ratio and MCVr between IDA and controls; in controls and vitamin B12 deficiency anemia; in controls and mixed anemia; in IDA and vitamin B12 deficiency anemia; in IDA and mixed anemia. Also in terms of both CHr and CHCMr (Corpuscular mean hemoglobin concentration of reticulocyte), there is a statistically significant difference between controls and IDA; controls and mixed anemia; IDA and mixed anemia; IDA and vitamin B12 deficiency anemia.

Conclusions: In mixed anemia, MCV could be normal or decreased, and in peripheral blood smear erythrocytes cells could be morphologically normal. For this reason diagnosis of mixed anemia is not easy and needs additional laboratory investigations. Our results suggest that in a differential diagnosis of mixed anemia from vitamin B12 deficiency, IDA, and healthy controls, CHr, CHCMr, and MCVr (together with MCV and individually) could be useful. So, with a simple and cheap laboratory parameter, differentiation of mixed anemia could be done.

Vitamin B12 defiiency causes haematological, gastrointestinal, psychiatric and neurological diseases. Subacute combined degeneration (SCD) of the spinal cord, characterised by
degeneration of the lateral and posterior columns, is often found due to vitamin B12 defiiency. We report SCD occurring in a 57-year-old man who presented with a 2.5–month history of gradually progressing tingling in the figers and toes and neck ache. Laboratory data revealed vitamin B12 defiiency and magnetic resonance (MR) imaging of the cervical spinal cord demonstrated abnormal hyperintense signal changes on T2-
weighted imaging of the posterior columns. In our case, follow-up MR imaging fidings correlated well with clinical outcome after treatment with vitamin B12 supplements. Neurological
symptoms in vitamin B12 defiiency are frequent. Early spinal MR imaging assists in the early diagnosis and treatment of the disease.

In this study, we developed a selective and sensitive inner filter effect-based method by thioglycolic acid capping CdTe quantum dots (TGA-CdTe QDs) for the quantitative determination of Vitamin B12 (B12). The effect of all experimental parameters and possible interference agents for determination of B12 by TGA-CdTe, were individually investigated and optimized. In addition, the mechanism of this detection method was discussed in detailed. Under the optimum conditions, the fluorescence-quenching rate of the TGA-CdTe QDs showed a relative linear relationship against B12 concentration in the ranges of 0.02-0.4 and 1.5-70.0 µM B12 with a detection limit of 2.0×10-8 M. Furthermore, the accuracy, precision and the practical application of determination of B12 by this developed method, were studied in pharmaceutical formulations injection without any sample pretreatment process. The percentage recoveries for B12 determination in these real samples were obtained in the range of 95.0%-105.0%.

Cobalamins are ubiquitous cobalt-based corrinoid cofactors of various transferases, exemplified by methyltransferases and adenosyltransferases. The cobalt oxidation state in these cobalamins directly influences their coordination environment and the redox properties. Cobalt ion in these cobalamins prominently exist in three oxidation states (I, II and III). The main focus of this review is the lowest cobalt oxidation state, cob(I)alamin. Cob(I)alamin, also referred to as ''supernucleophile " , has broad reactivity properties. This review focuses on how recent advances in computational chemistry have contributed towards improving our understanding of the coordination environment, electronic structure, spectro-scopic properties of cob(I)alamin and its implications for the catalytic functioning of methyltransferases. Density functional theory and quantum mechanics/molecular mechanics approaches provide new insights into the coordination environment of cob(I)alamin bound inside methyltransferase and the molecular mechanism of methyl transfer to cob(I)alamin. In addition, the wavefunction-based multiref-erence approach has revealed the details of the ground state electronic structure and electronically excited S 1 state of cob(I)alamin. The occurrence of megaloblasts in the bone marrow characterizes megaloblastic anemia. Megaloblastic anemia is the result of folate and vitamin B12 deficiency in many cases. It is either caused due to nutritional deficiency of folates or due to its malabsorption. Usually accompanied by leukopenia or thrombocytopenia, this disease is characterized by megaloblastic bone marrow morphology along with myeloid, erythroid or platelet precursors. In this kind of anemia, the mature erythrocytes have abnormal shapes and are of various sizes. As a result, their Mean Cell Volume (MCV) is much greater than normal and Red Cell Distribution Width (RDW) is also deviated from normal level. Thus, in routine hematological procedure MCV, RDW needs to be thoroughly investigated, as it may be a useful indicator in diagnosis of suspected megaloblastic anemia with related oral manifestations, secondary to Vitamin B12 malabsorption, which thereby guides the further management.

The use of adsorption methods to recover vitamin B12 (VB12) from wastewater has been increasingly studied. However, there is a lack of knowledge on optimization of the methods. This study established a feedback network to evaluate alternatives regarding VB12 adsorption from wastewater. The network comprises environmental, economic and technological criteria and their feedbacks. Based on the network, the fuzzy matter-element theory was integrated with an analytical network process to rank the alternatives. Among five alternatives, activated carbon with KOH as activation media was suggested to be the optimal alternative for VB12 recycling from wastewater, while mesoporous activated carbon fibre was the least preferred alternative. Particularly, the adsorption technology reusing biomass ranked second to the optimal alternative, and has great application potential due to low costs and biological waste reuse. Sensitivity analysis does show that the ranking of alternatives was robust and was not subject to the change in weight.

The radical pair that results from photolysis of adenosylcob(III)alamin (AdoCblIII) undergoes primary geminate recombination with a first-order rate constant of 1 x l09 s-l. In contrast, methylcob(III)alamin (CH3CbIII) and aristeromicylcob(III)alamin (AriCblIII, the carbocyclic analogue of AdoCblIII in which the ribofuranose ring oxygen has been replaced with a methylene group) does not undergo primary geminate recombination. The ribofuranose group enables a high rate of geminate recombination in the [Ado• CblII] radical pair. This may be due to a stereoelectronic (-anomeric) effect that maintains a pyramidal geometry at the 5'-carbon of the 5'-deoxyadenosyl radical, or it may be due to hindered rotation about the C4’-C5’, bond such that  -elimination to the olefin is prevented. Recombination in the geminate singlet radical pair is in competition with diffusive escape to form a solvent-separated radical pair. Hyperfine coupling from CoII promotes intersystem crossing to the triplet radical pair (Chagovetz, A. M.; Grissom, C. B. J. Am. Chem. SOC. 1993, 115, 12152). Recombination of the [CH3• CbII] radical pair is not prevented by a lack of intersystem crossing, as neither unlabeled nor 13C-labeled CH3CblIII undergoes geminate recombination. There is only a small difference in the rate of diffusive recombination in the solvent cage for AdoCblIII, AriCblIII, and CH3CblIII following photolysis: 2.01 x 104 s-l, 2.20 x l04 s-l, and 1.16 x l04 s-l. The rate of diffusive recombination is limited by productive collisions and not by radical geometry or intersystem crossing. The CF3• radical that results from photolysis of (trifluoromethyl)cob(III)alamin (CF3CblIII) maintains its pyramidal geometry and undergoes faster diffusive recombination in the solvent cage at 51 x l04 s-l. The C-Co bond dissociation enthalpy in AriCblIII is 37 ± 1.4 kcal/mol. The profound difference in geminate recombination rates for AdoCblIII and CH3CblIII is consistent with their different biological roles as enzymatic cofactors: AdoCblIII is an initiator of radical chain chemistry in the active site, whereas CH3CblIII is a methyl group donor in an SN2-type process.

Cobalamins are stored in high concentrations in the human liver and thus are available to participate in the regulation of hepatotropic virus functions. We show that cyanocobalamin (vitamin B12) inhibited the HCV internal ribosome entry site (IRES)-dependent translation of a reporter gene in vitro in a dose-dependent manner without significantly affecting the cap-dependent mechanism. Vitamin B12 failed to inhibit translation by IRES elements from encephalomyocarditis virus (EMCV) or classical swine fever virus (CSFV). We also demonstrate a relationship between the total cobalamin concentration in human sera and HCV viral load (a measure of viral replication in the host). The mean viral load was two orders of magnitude greater when the serum cobalamin concentration was above 200 pM (P < 0.003), suggesting that the total cobalamin concentration in an HCV-infected liver is biologically significant in HCV replication.

Background: The relation between key enzymes in regulation of folate metabolism and male infertility is the subject
of numerous studies. We aimed to determine whether 5, 10-methylenetetrahydrofolate reductase (MTHFR) C677T
and methionine synthase reductase (MTRR) A66G genotypes are associated with male infertility in Iranian men and
to evaluate its effect on seminal levels of folate and vitamin B12.
Materials and Methods: In this retrospective study, semen and peripheral blood samples were collected from 254
men with oligoasthenoteratozoospermia (OAT) and 77 normozoospermic men who attended Avicenna infertility clinic.
Single nucleotide polymorphism (SNP) analysis was carried out in genomic DNA by polymerase chain reaction
(PCR)-restriction fragment length polymorphism (RFLP) method for MTHFR C677T and MTRR A66G gene polymorphisms.
Results: In MTHFR C677T, our founding showed that T carrier was conversely lower in OAT than normozoospermic men
(χ2-test=7.245, P=0.02) whereas in MTRR A66G, A and G carrier showed no significant difference between the two groups
(χ2-test=1.079, P=0.53). The concentration of seminal folate was not different between normozoospermic (18.83 ± 17.1 ng/
ml) and OAT (16.96 ± 14.2 ng/ml) men (P=0.47). The concentration of vitamin B12 was slightly higher in normozospermic
men (522.6 ± 388.1 pg/ml) compared to OAT men (412.9 ± 303.6 pg/ml, P=0.058).
Conclusion: The MTHFR C677T and MTRR A66G have no effect on the concentrations of seminal folate and vitamin
B12. The present study showed that two SNPs of MTRR A66G and MTHFR C677T cannot be seen as a risk factor for male factor subfertility.

Research shows that serum Vitamin B12 levels are reduced in diabetes including gestational diabetes. We sought to compare serum cobalamin between pregnant females diagnosed with gestational diabetes mellitus (GDM) and healthy controls and to propose a cut off value for serum B12 levels as a predictive biomarker for GDM. Study design was Quantitative cross-sectional study. This study was carried out at Lady Atchison Hospital/Mayo Hospital Lahore, Pakistan from January to December 2016. Cobalamin levels were measured in women diagnosed with GDM (n=59) and controls (n=41). GDM was diagnosed when fasting blood sugar levels were 95 mg/dL or above. The levels were compared using Mann Whitney U test. Multiple linear regression was used to investigate the association of cobalamin with fasting blood sugar level after adjusting for gestational age and gravidity. ROC curve analysis was conducted to assess the suitability cobalamin as a predictive biomarker for GDM. Median cobalamin were significantly reduced in cases (108.95±92 pg/mL) as compared to controls (173±90 pg/mL) at p<0.001. Low vitamin B12 levels (β =-0.29, p=0.004) significantly predicted high fasting blood sugar levels in pregnancy even after adjusting for gestational age and gravidity. ROC curve analysis revealed the optimal cutoff point of serum cobalamin for predicting gestational diabetes to be 113 pg/mL at which sensitivity and specificity were was 56.9% (95%CI 44.1-68.8%) and 80.5 % (95%CI 65.6-89.9%) at P<0.0001. It was concluded that low serum cobalamin levels may be a risk factor and may predict the development of GDM.

BACKGROUND:
An important biological factor suggested in the pathophysiology of (recurrent) Major Depressive Disorder (MDD) concerns a polymorphism in a gene encoding for the MTHFR-enzyme of the one-carbon (1-C)-metabolism. Integratively investigating key 1-C-components (folate, homocysteine, vitamin B6 and B12), including the possible effects of antidepressant medication and depressive state, could provide more insight in the possible association between the MTHFR-polymorphism and recurrent MDD.
METHODS:
We compared the MTHFR C677T-polymorphism together with the key 1-C-components in clinically ascertained patients with recurrent MDD (n=137) to age- and gender-matched healthy controls (n=73).
RESULTS:
First, patients had lower folate (t=2.25; p=.025) as compared to controls; a difference that resolved after correction for demographics (t=1.22; p=.223). Second, patients that were depressed during sampling had lower vitamin B6 (t=-2.070; p=.038) and higher homocysteine (t=2.404; p=.016) compared to those in remission. Finally, current use of antidepressants had no influence on the 1-C-components.
CONCLUSIONS:
Despite investigation of a specific recurrently depressed patient population, we found no clear associations with the 1-C-cycle, except for higher homocysteine and lower vitamin B6 during the depressed state. This suggests that 1-C-cycle alterations in MDD are state-associated, possibly resulting from high levels of acute (psychological) stress, and may provide a treatment target to reduce cardiovascular risk in this population.

The ability of enzymes to harness free-radical chemistry allows for some of the most amazing transformations in nature, including reduction of ribonucleotides and carbon skeletal rearrangements. Enzyme cofactors involved in this chemistry can be large and complex, such as adenosylcobalamin (coenzyme B12), simpler such as “poor man’s B12” (S-adenosylmethionine and an iron-sulfur cluster), or very small such as one iron atom coordinated by protein ligands. While the chemistry catalyzed by these enzyme-bound cofactors is unparalleled, it does come at a price. The enzyme must be able to control these radical reactions, preventing unwanted chemistry and protecting the enzyme active site from damage. Here we consider a set of “radical” folds: the (β/α)8 or TIM barrel, combined with a Rossmann domain for coenzyme B12 dependent chemistry. Using specific enzyme examples, we consider how nature employs the common TIM barrel fold and its Rossmann domain partner for radical-based chemistry.
