FEEDBACK LOG for formative Assessment
Fall 2016 The feedback you provide to students on your scoring guides or verbally is important and critical in helping students progress in their learning. Feedback should ALWAYS provide them with information about their strengths and their weaknesses. Students also do not automatically use the feedback of what you write or tell them. You will need to have conversations with them each day to help them reflect on what you shared with them from the day before (metacognition). You can also think of concrete support that may help them to reflect on the feedback. (index card with a reminder, poster, a string on their finger). Though you are documenting your feedback for each individual lesson (whole class and 2 individual students) and providing a short reflection on how you will help students use this feedback to continue on their path the next day (see column 3 below), this is a practice that should be done for all effective teaching. So, make sure you remember this for your unit as well. However, it will not be written up for your unit due to time constraints in grading the eportfolio,.

Day 1- Language Arts Read
Aloud and Writing Activity
I provided verbal feedback to
the students based off of the students writing. Child 1- responds well to verbal feedback. Child 1 was given verbal feedback to continue to work on his writing and add more details/adjectives to his writing. Child 2- Child 2 struggles to write so I had him explain his writing and his illustration to me. The feedback I provided was for him to practice writing on the line and to try and sound out the words as he wrote them.
Day 2- Social Studies Voting

provided to all students for the social studies lesson. Each child was assessed through a checklist.
How would or will you
encourage students to use the feedback? I would conference with the students weekly to check on their progress and give continued feedback to the students. I would remind Child 1 to add more details to his writing and I would work with Child 2 on his writing and letter cognition. I think weekly conferences with the students would help them remember what they need to be working on and improve. When I conferenced with the students weekly, I could go over different writing with Child 1 and his progress. Then, I could practice letters with Child 2 to see how he is progressing. I made notes for all the students on the checklist. Every one received positive feedback for the lesson. I would encourage Child 1 to

Day 2: Math Pictograph Activity
Child 1- shared his answer to
the class even though he spoke very quietly. On Child 1s checklist, I gave him positive feedback for speaking out in front of the class. Even though he was quiet, he still chose to talk, which is not something he chooses to do all the time. Child 2- Struggled to glue his pumpkin in the right area. Overall these are both minor issues that were noted on the checklist that was filled out. On Child 2s checklist, he got 2/3 checks. I had to check no if he glued his pumpkin in the right spot. I think he struggled to connect the pumpkin he drew with the three different options. I would provide him verbal feedback regarding the checklist and how he can improve for in the future. The entire class completed exit slips. Afterwards, the students received feedback about the exit slips. The feedback I gave to the entire class was that they all did a really good job and I know that the exit slip was hard. I told the class that we would continue to work on activities such as this so the students will better understand the activity next time. For Child 1, I gave him feedback about his addition facts. Child 1 struggles with his addition facts. Problems like 5+6, and 8+1, we have been practicing for a long time and he struggles with them. I would

I would encourage him to use this feedback by giving him plenty of opportunities to speak in front of the class I would just tell him that it is important to double check our work and pay more attention because that is something Child 2 struggles with. I would encourage the child to pay more attention by frequently asking him questions and checking on him during lessons.

use this feedback by doing more lessons on pictographs so they have the opportunity to better understand them.

him with activities that he could do at home to work on his addition facts. I would write a letter to his parents asking them to help him practice so that he can improve his addition facts. I

Day 3- Science Lesson
The students were provided a
checklist after the lesson regarding how they did during the pumpkin planting activity, the life cycle cut and paste, and the three things pumpkin seeds need to grow. The feedback provided was mostly positive because almost all of the students completed everything correctly. I told the students that they all did a great job and thanked them for working so hard. Child 1: I provided child 1 with positive feedback about how he completed the cut and paste activity and about how he knew the three things pumpkin seeds need to grow. I told him that he did a great job paying attention during the read aloud. Child 2: I provided feedback to Child 2 that was mainly positive on the checklist. He only messed up one section of the life cycle cut and paste but he still

I would help the student use this feedback by providing with materials and games he can play at home and during free time for number recognition. I would teach the student to play the games and would play the games with him. Afterwards, I would send the games home so the student could continue to practice. I would encourage the students to use this feedback by asking them to work this hard during every lesson. I would remind students of the excellent work that they did and how much they learned. I would encourage students to try their best just like they did for this activity. Child 1-I would encourage child 1 to continue to work hard. I would tell him he did a great job with the activity.
Child 2- I would tell Child 2 that
he did a great job and worked very hard on the activity. I would encourage him to slow down when he is working and pay more attention. I would make a little checklist for him to glue to his desk reminding him to pay attention to his work and take his time. I would also conference with the student
