CCA SCRIPT
I. INTRODUCTION
Good morning candidates. My name is _______________, your competency assessor for
(qualification). I would like to acknowledge the presence of Mr. RENATO D. UBIÑA, our
Assessment Center Manager and Ms. LOVELY NOLASCO, our TESDA Representative. They came
here to oversee the conduct competency assessment and to check if the conduct is in
accordance with the prescribed assessment methodology and procedure.

Let me check the attendance, as I call your name please come in front and present to me
your admission slip. Ms. CARMIE DELA ROSA, please come in front and affix your signature
in the column provided (GET THE ADMISSION SLIP), you are the candidate number 1. Mr.
JAY PEE FURAGGANAN, come in front and follow the same instruction (GET THE
ADMISSION SLIP), and you are the candidate number 2.

Here with me are the accomplished APPLICATION FORM and SELF- ASSESSMENT GUIDE. It
is an indication that you know and can perform all the units of competency enumerated in this
qualification
V. PROJECT/COC BASED ASSESSMENT PACKAGED
Today, I’m going to administer the competency assessment for (QUALIFICATION). This
qualification has 3 competencies, namely: BASIC, COMMON and CORE COMPETENCIES.
Under CORE COMPETENCY we have 1________,2__________,3_____________.
THIS IS A PROJECT BASED ASSESSMENT PACKAGED AND THIS IS A FULL QUALIFICATION. You
must demonstrate competence in all competencies for you to have a NATIONAL
CERTIFICATE, if not all, YOU WILL BE RE-ASSESSED AND REPEAT ALL. NO CERTIFICATE OF
COMPETENCY IN THIS ASSESSMENT FOR THIS QUALIFICATION.
THIS IS A COC ASSESSMENT PACKAGED. You must demonstrate competence in all

competencies for you to have a NATIONAL CERTIFICATE, if not all, YOU WILL BE RE-
ASSESSED AND REPEAT THE COMPETENCY RATED NOT YET COMPETENT.
VI. PURPOSE OF ASSESSMENT

*The purpose of this assessment for (qualification) is to determine your level of
competency.
VII. ASSESSMENT METHODS
To determine your knowledge and skills, the assessment method to be used are
demonstration with oral questioning.
VII. ALLOWABLE ADJUSTMENT

Later on, if you encounter some terms wherein you are not familiar with, feel free to
approach me and I’m willing to translate it in your dialect. Same is true with the instruction,
I will translate it orally. Do not worry, you will be all treated equally.
VIII. DO’S AND DON’TS
1. During the assessment, you are not allowed to leave the premises of this assessment
center.
2. If you have questions or clarifications, you may raise it now. Once the assessment started,
please refrain from asking questions. It is no longer allowed.
IX. TASK TO BE PERFORMED

The different task you will perform in competency number 1_____________. You have 4 hours
to perform the task. I will gather evidences through observation with oral questioning.
ASSESSMENT AREA
*all the materials, supplies, equipment and PPE are located in the storage room

number 1 and candidate number 2, proceed to table number 2. Get the materials, supplies
and tools needed. Before you start, may I remind you the safety precautions and apply the
5S and do not forget to wear your PPE.

you to follow and your guide for this assessment. You only have 4 hours to finish the task.
The time now is 12:00 noon and ends at 4:00 PM. Do you have questions? You may raise it
now, once it is started, you are no longer allowed to ask questions. If none, you may start it
now.
CANDIDATE NUMBER 2, your task is____________. Here is your specific instructions for
you to follow and your guide for this assessment. You only have 4 hours to finish the task.
The time is 12:00 noon and ends at 4:00 PM. Do you have questions? You may raise it now,
once it is started, you are no longer allowed to ask questions. If none, you may start it now.

*with regards to the result of the assessment, we observe strict confidentiality. This is to
ensure your integrity. Please refrain from giving away any form of information took place
during the assessment process.
You may now claim your NATIONAL CERTIFICATE at TESDA PROVINCIAL OFFICE located at Carig
Norte, Tuguegarao City, Cagayan. Bring with you your Php50.00 for the processing fee and
passport size picture with collar and white background.

How do you feel now? As I observed, while you were performing the task, maganda naman ang
pamamaraang ginawa mo. May mga technique kang ginawa na nakaganda sa outcome ng
product mo. However, as all we know, following instruction is important. As a result, dahil sa
may sarili kang style na ginawa, hindi mo nasundan ng tama ang instructions and guidelines at
dahil diyan, naapektuhan ang performance mo. I am sorry to tell you but you are not yet
competent. Do not lose hope dahil pwede ka pang ma re-assess. You fail now but it doesn’t
mean that you are good. God bless to your re -assessment. Please sign here (CARS and RATING

AFTER YOU ARRANGE THE PAPERS OF EACH CANDIDATE, MAKE IT SURE TO STAPLE IT ON
THE UPPER LEFT SIDE. JOIN THE TWO STAPLED PAPERS TOGETHER , AND ON THE TOP OF IT,
ATTACH THE ATTENDANCE SHEET AND STAPLE IT AT THE CENTER(UPPER PART)
