Orthodontics
A facemask (also referred to as a protraction facemask, orthopedic

facemask, or reverse-pull headgear) is a type of an orthodontic headgear

used to treat underbite and other malocclusions where the upper jaw is

too far backwards.

The Face Mask orthodontic appliance or Reverse Pull Headgear consists of

a wire face frame with forehead and chin guards as well as removable
elastics.

There are foam pads that attach inside the guards for a soft cushion.

This Face Mask is used when the upper jaw is too far back in comparison
to the lower jaw.

This can be referred to as an “underbite”.

The Reverse Pull Headgear or Face Mask will help

pull the upper jaw forward to correct the skeletal imbalance in

addition to correcting the bite.

Typically, the Face Mask is used

in younger patients that are still growing.

The Face Mask pads rest on the forehead and chin with a frame (like a

catcher’s mask).
Braces or bands on the upper molar teeth are placed.

The patient attaches orthodontic elastic bands from these braces or

bands inside the mouth to the external Face Mask. This pulls the teeth

and/or jaw forward.

• The Face Mask is to be worn 12-14 hours minimum per day, preferably at
home

and while sleeping

• Avoid hard or sticky foods: gum, toffee, licorice, hard candy

• Regularly check that the bands on the teeth are secure at all times

• Remember to bring your Face Mask to each appointment. Elastics may be

replenished at this time.

Biomechanical considerations :

1. Amount of force- the amount of force required to bring about skeletal

changes is about 1 pound or 450 gms per side.

2. Direction of force- 15 – 20 degree downward pull to the occlusal

plane to produce a pure forward translatory motion of the maxilla. If the

line

of force is parallel to the occlusal plane, a forward translation as well as an

upward rotation takes place.

3. Duration of force- time taken to achieve desired results is

proportional to the amount of force utilized. Low forces (250 gm/ side) take
13

months to produce desired results. High forces ( 1600- 3000 gms) reduced

treatment time to 4 – 21 days.

4. Frequency of use- 12 to 14 hours of wear a day.

ADVANTAGES OF FACEMASK:
1)ls only extraoral traction device for correction of maxillary defiecny with

rapid improvement and early result

2)Improved complaince and patient confidence.

EFFECTS OF FACEMASK:
1)Forward movement of maxilla

2)Forward movement of maxillary teeth

3) Proclination of maxillary incisors

4) Correction of A crossbite

5) Downward and Backward rotation of mandible
