This form is to be completed by interviewers in order to rate candidate’s performance and qualifications for the
applied position. The interviewer should give the candidate a numerical rating under each heading according to their
performance. The numerical rating system is based on the scale:

Prior Work Experience – Has the candidate acquired skills or


qualifications through past work experiences which are required for
this position?

Team building skills – Does the candidate show team building skills
which are necessary for this position?

Critical thinking skills – to what extent does the candidate was able
to demonstrate how they solve problem, build a process?

Knowledge of field - The candidate was knowledgeable about his


area/field of expertise

Interest in learning - How open did they appear to be when learning


new things?

Attitude towards this position – How much interest the candidate


showed for this position in their responses?

Overall Impression and Recommendation – Summary of your Proceed Proceed Do not


perceptions of the candidate’s strengths/weaknesses. Final with proceed
comments and recommendations for proceeding with the candidate. reservations
