Summary
The Supervisor’s Report contributes to the assessment of CHCAGE001 Facilitate the empowerment
of older people.

Your supervisor will observe you as you undertake Assessment Task 4: Plan and implement an
activity

Resource Requirements
 Access to an aged care workplace including:
o Access to an aged care client
o Access to workplace policies and procedures
o Access to aids and equipment
o Permission from your supervisor and the client to undertake this task
 Work Placement Agreement

You will do this task at your work placement at a time organised with your supervisor.

What happens if I get something wrong?


If your assessor identifies that your performance does not meet these requirements, you will be
given feedback and an opportunity to submit further evidence or to do the task again.

Student Instructions
You are to have a discussion with a client in an aged care facility and find out what they would like to
achieve (goals) and what they wish for.

You will need to respond to their statements by designing and putting in place an activity that will
contribute to their goals and wishes.

 You are expected to display the skills and knowledge listed in the supervisor report.

 Before commencing this task ensure your supervisor is aware of the Supervisors Report and
has a copy.
 Confirm with your supervisor when to collect the report as instructed by your Assessor.
 Once you have the necessary permission, you are to:
 Learn about your client’s goals/wishes:
o This can be done through discussions with your client (this is essential),
reviewing their individualised plan and discussing with other staff as
appropriate.
 Plan an activity that will contribute to your client’s goals and wishes.
 Show your plan to your supervisor and get their approval to run the activity.
 Implement the activity – remember you will need to be supervised during this activity.
 Seek your client’s feedback about the activity once it is over.
 Ask other people who were involved in the activity for their feedback.
 Write a report about your experiences using the Project template.
 Double check your Supervisor has signed and dated the report.

Instructions to the Supervisor

A supervisor’s report is used to collect supporting evidence of ‘everyday performance’ during the
student’s work placement, rather than performance carried out as part of formal assessments.

The supervisor does not make a decision regarding the student’s competence. Evidence provided by
the supervisor is used to contribute to a pool of evidence regarding the student’s knowledge and
skills. The VET Teacher will use this pool of evidence to make an assessment decision.

You may not be able to observe or comment on all the criteria in this checklist. Do not be
concerned. The VET Teacher will be conducting assessments on a number of occasions and contexts
with the student.

You will be required to comment on specific activities undertaken by the student, as well as their
overall contributions.

To be considered a supervisor you must have direct responsibility for supervising the student in the
workplace.

When completing the Supervisor Report:


 Read through the list of required skills. We ask that you give honest feedback on the
student’s skills and knowledge in relation to the questions.
 Be aware that the student will see the report
 Where possible, ensure that the student has been provided with the opportunity to perform
each task specified
 Record the period of time which your observations were made, by stating the start and end
date in the required section of this report. The timeframe in which you have made the
observation should be within a 14-day period minimum.
 Tick ‘Yes’ if the student satisfactorily demonstrated the skills and knowledge. Tick ‘No’ if the
student did not satisfactorily demonstrate the skills and knowledge and tick ‘N/A’ if you did
not observe the student demonstrating the required skills and knowledge.
 Where possible include add any additional comments or specific examples that inform on
the student’s ability to perform the role to industry standards.
 Record your name and position, sign and date the supervisor’s declaration.
 Complete the report and sign the declaration at the end of the checklist. Give the original
copy to the student. The student will collect the completed report on the negotiated date
and return it to their VET Teacher.
 The VET Teacher may make contact with you to authenticate the content of the report.
Keep a copy in case you are contracted to verify a part of this report at a later time.
