Eligibility

You, your dependents and your spouse are eligible for health, vision, dental, Short/Long Term Disability (employees
only) and other insurance benefits starting your first day of employment, provided:

• You are regularly scheduled to work 30 hours or more each week (or participating in a RampBack program
following a maternity or parental leave of absence).

• Your spouse is your legal spouse or if domestic partner has been living with you for at least six months.

• Your dependent children are under age 21 or full-time students under age 25; mentally or physically disabled
children of any age may also be eligible. If you live in Quebec, student coverage is available until age 26.

Generally, all full-time Amazon employees are eligible to participate in Amazon’s employee stock plan.

Employees regularly scheduled to work less than 30 hours: please see “Canada Benefits at a Glance: Part-time,
Seasonal, Flex-time, Temporary and Interns”.

Health
Insurer • Canada Life – Plan #169366
Cost • 100% paid by Amazon
Prescription drugs • 90% reimbursement of generic drugs
• Prescription plan deductible equal to dispensing fee
• Purchase with your Canada Life benefits card
• $1,000 annual out-of-pocket maximum per family (or per employee, if you have single coverage)
for prescription drugs per plan year (September 1 to August 31)
Paramedical • 90% reimbursement of the care and services of:
practitioners
- Acupuncturists
- Chiropodists/podiatrists
- Chiropractors
- Naturopaths
- Osteopaths
- Physiotherapists
- Licensed massage therapists (including shiatsu massage)

Orthotics • 90% reimbursement, to a maximum of $350 per calendar year
Orthopedic shoes • 90% reimbursement, to a maximum of $500 per calendar year
Vision care • 90% reimbursement
• Eye examinations, to a maximum of $50 per person every 24 consecutive months

• Glasses, contact lenses, laser eye surgery, to a maximum of $250 per person every 24

Hearing aids • 90% reimbursement, to a maximum of $500 every 5 calendar years
Hospitalization • 90% reimbursement for a semi-private room
Private duty nursing • 90% reimbursement, to a maximum of $25,000 per 3 calendar years

Out-of-Country
Emergency Care and • 100% reimbursement, to a $5,000,000 lifetime maximum
Global Medical • Out-of-country referral 50% reimbursement up to a maximum of $3,000 every 3 calendar years
Assistance • Emergency medical expenses incurred outside Canada (unlimited days out of country)

*Reasonable and customary refers to the maximum allowable amount Canada Life will reimburse on a paramedical
expense and are the commonly charged fees within a geographic area. To avoid surprises, contact Canada Life before
your treatment to find out how much you would be reimbursed.
Dental

• Hourly: 70% of weekly base pay‡; waiting period: 3 continuous days. Maximum benefit period:
