Flexible Part-Time Employees

Benefits Summary (July 1, 2021 – June 30, 2022)


Lands' End provides several programs and benefits for our employees. If you have any questions regarding your benefit

ENROLLMENT for dental, vision, and life insurance plans

Initial Enrollment (for new flexible part-time employees)

• Enrollment forms must be completed within 31 days of your hire or rehire date.
• Enroll as soon as possible so ID cards and deductions are processed in a timely fashion.
• Benefits are effective the beginning of the pay period following your hire or rehire date.
• If you are currently covered under another plan, you may elect to decline coverage during Initial Enrollment and
then enroll in Lands' End coverage as a Special Enrollment when your other coverage ends or at Open
Enrollment.
• To complete the enrollment process:
o If you are a new hire or rehire, go to LEnet > UltiPro > Menu > Manage My Benefits > New Hire

o If you are a transfer to part-time, go to LEnet > UltiPro > Menu > Manage My Benefits > Life Event
Enrollment
• Enrollment is complete when you can download, email or print your confirmation statement.

Open Enrollment (for current flexible part-time employees)

• Open Enrollment occurs in early June


• Open Enrollment allows you to update your benefit elections (add, drop, change). Changes are effective July 1.
• If you do not make changes to your benefits, your current benefit elections for dental and vision will carry over
to the next year.
• To make changes to dental and vision coverage, go to the LEnet > UltiPro > Menu > Manage My Benefits and
click on the Open Enrollment link.
• Enrollment is complete when you can download, email or print your confirmation statement.

Special Enrollment/Qualified Life Events (for current flexible part-time employees)

• Generally, you may only change your benefit elections during the Open Enrollment period. However, you can
change your benefit elections mid-year if you experience a Qualified Life Event. These include:
o Marriage
o Birth, adoption, or placement for adoption
o Loss of other coverage
o A significant change in your or your spouse's coverage
o Entitlement to Medicare or Medicaid
• Changes must be made within 31 days of the event.
• To request changes to your benefits due to a life event, go to LEnet > UltiPro > Menu > Manage My Benefits >
Life Event.
• Complete the enrollment process, you will need to submit documentation of the life event via UltiPro.
• Once you have submitted your enrollment, you will be notified when your enrollment is approved.

ELIGIBILITY

Employee Eligibility – all ongoing flexible part-time employees are eligible for the benefits included in this summary.

Dependent Eligibility – You can enroll your eligible dependents (spouse and dependent children) for dental, vision and
life insurance.

Spouse is your legal spouse.

Your dependent children are eligible:

• Through the end of the month once age 26 for dental and life insurance.
• Through the end of the month once age 27 for vision insurance.
• Disabled dependent children

Dependent child definition includes a natural child, stepchild, legally adopted child, a child placed for adoption, a child
for whom legal guardianship has been awarded to you or your spouse.

If Both Spouses Are Ongoing Flexible Part-Time Employees at Lands’ End


If both you and your spouse are eligible for benefits as an ongoing flexible part-time Lands’ End employee, only one of
you can choose dental or vision coverage as an employee; the other person must be covered as a dependent.

When Benefits Begin

• Open Enrollment: Benefits are effective July 1 each year.


• New Hires/Rehires: Benefits are effective the first day of the pay period following your hire date (if enrollment
is completed within first 31 days of employment).
• Qualified Life Event: New benefits are effective on the date of event, such as birth (if required information is
completed/received within 31 days of the qualified life event).

When Benefits End


Dental, vision, and life insurance benefits will end for terminating employees and transfers to full-time on the last day of
the pay period in which the event occurs. Overage dependents coverage will end on the last day of the month. For
qualified life events, benefits end as of the date of the event.
Dental Plan (administered by Delta Dental)

Good dental health is important to your overall health and well-being.

• In-network providers offers claims processing services and discounts on services. If you choose an out-of-
network provider, you are responsible for charges above usual and customary.
• To find an in-network provider, visit www.deltadentalwi.com. Select the Provider Search tab; and click Find a
Dental Provider. You can choose either Delta Dental PPO or Delta Dental Premier providers. Delta Dental PPO
providers offers the greatest discounts.

• Superior Vision offers an online in-network site (DITTO.com) that features brand-name frames.
• Favorite brand contact lenses are available at ContactsDirect.com/Superiorvision.

Life Insurance (administered by Voya)


Should something happen to you, would your family have enough financial protection? Life and Accidental Death and
Dismemberment (AD&D) insurance can help provide that kind of security for you and your family. You may choose
coverage from one of the two plans shown in the table below.

Life Insurance includes AD&D coverage for the employee only. This coverage provides an additional benefit for loss of
life due to an accident. The coverage is equal to the life benefit. Reduced amounts are payable for dismemberment.

If you do not enroll during the Initial Enrollment period, you may enroll later but your enrollment is subject to
evidence of insurability. For those that enroll in Option 1, you can elect Option 2 during the Open Enrollment
period without evidence of insurability.

Family & Medical Leave (FMLA)
FMLA allows you time away from work for certain qualifying situations such as:

• To care for your child after birth, or placement for adoption or foster care
• To care for a family member with a serious health condition
• For a serious health condition that makes you unable to perform your job
• If your family member has received a call to support a qualifying military operation
• To care for a family member who is an injured military service member
• Eligibility is based on hours worked and length of service
• To determine if you are eligible for FMLA, contact Matrix at 877.202.0055

Retirement Plan
You are eligible to participate in the plan at the end of the calendar quarter if you are at least 18 years of age and meet
one of the following:

• 1,000 hours of work within the 1st anniversary of your hire/rehire date; or,
• 1,000 hours of work in any plan year thereafter.

This plan offers immediate rollovers from other qualified plans and 100% vesting. As a participant, you will be able to
contribute and save 1-75% of your base pay with pre-tax and/or Roth 401(k) contributions. The quarter following one
year of service, if you are a participant, the company will match 50 cents for every $1 you save up to 6% of your salary
provided you work 1,000 hours each payroll year. Participants are immediately 100% vested in all contributions. If you
are rehired and were a previous participant, you are immediately eligible to participate upon rehire.

Personal Hours
As an active employee, you will receive personal hours each February based on the number of hours worked in the
previous fiscal year. These hours can be used anytime to cover scheduled hours you are unable to work. Personal hours
do not need to be scheduled in advance. If you work in a jurisdiction that requires paid sick leave, you will earn personal
hours each pay period based on the requirements in your jurisdiction in lieu of the personal hours award on February 1.

Unpaid Hours
Unpaid hours are available to help manage the times you need to be away from work. If you are a new hire or a rehire
employee, you will be given a balance of unpaid hours based upon your month of hire. In addition, you will accrue one
hour for every 30 hours paid.

Comer Center
Our onsite fitness facility is available for use by employees, their spouse and dependent children. The Center has a
swimming pool, 1/8-mile indoor track, exercise equipment, gymnasium, aerobics room and racquetball courts. Guests
and dependent children need to be accompanied by you or your spouse. Currently, the Comer Center is closed due to
COVID.

Jury Pay
If you are legally obligated to be absent on a scheduled day of work for jury selection or for service as a member of a
jury, you will be paid for the scheduled hours on the day(s) you serve.

Registered Dietitian
Nutrition counseling with a registered dietitian is a unique opportunity to receive reliable nutrition information designed
to meet your individual needs. This service is free to all employees, spouses, and dependent children by appointment.
Personal Trainer
A personal trainer can help you get started on an exercise program and help you stick with it. A free consultation is
available by calling the Comer Center.

Discounts
As a perk of being a Lands’ End employee you can receive a variety of discounts, including:
• Employee Merchandise Discount - You may purchase Lands’ End product for you, your spouse, and your
dependent children up to 50% off the original catalog/internet price. At retail stores, you receive up to
50% off full-price merchandise and an additional 20% off overstock and other items on promotion.
• Perks at Work – Perks at Work allows you to shop with over 30,000 merchants and receive discounts and
points that you can use towards future purchases.
• Cell Phone Plan Discounts – Discounts on services, phones and accessories for various providers
• Entertainment Discounts – Discounts on area golf, waterparks, theaters, and other local attractions.

Pay
Lands’ End issues pay via an Electronic Fund Transfer (EFT), often called direct deposit, for your pay. You can make up to
five electronic fund transfers each pay period to financial institutions of your choice. Direct deposit is a secure and
convenient method for you to receive your pay. You can view your pay statement on-line.

Referral Bonus
• Peak Season - Refer someone to work, and you will receive a $50 bonus if that person stays through the peak
season. Your name is also entered in drawings for additional money or gifts.
• Regular Referrals - If you refer someone for a regular full-time hourly or salaried position, you may be eligible
for between $500 and $1,000 for your referral, depending on the position.

This is a summary. Be sure to request more detailed information before making decisions on any of these benefits. Plan
documents will govern for any conflict.
