NEAR MISS REPORTING Prepared By: HSE Department

Introduction
A “near miss” or “minor incident” is a close call that has the potential for property loss or injury and
prevents a task from being completed as planned. It has been reported that 75 percent of on-the-job injuries
were preceded by minor incidents or near misses. Don’t miss the message of a near miss!

Tips on Near Misses


 Near misses are accidents waiting to happen. The only difference between a near miss and an
accident is a fraction of an inch or a second in time.
 Report all near misses right away. Most accidents are preceded by multiple near misses. Report them
to your supervisor so that he or she can track the patterns, pinpoint the problem, and take corrective
action.
 If possible, remove the hazard immediately. Do your part to protect your co-workers, patients, and
visitors from injury. But don’t forget to report the hazard even if you removed it.
 Report damaged equipment or property. Don’t wait for a near miss or accident to happen. Stay alert
for anything that could cause an accident. Injuries can often be traced back to equipment or property
damage that was never reported and repaired.

Look for and report damaged equipment, such as.


 Ladders with broken rungs or missing safety information.
 Electrical equipment that gives a light shock.
 Worn or torn PPE.
 Damaged working platform.
 Fall materials from height.
 Uneven surface.
 Poor illumination.
 Other similar problems.

Stay alert for and report property damage, such as.


 Loose handrails or midrail.
 Poor house keeping.
 Broken sidewalks o Crumbling stairs.
 Crumbling stairs
 Other damages to the facility.
Think of scenario not the event. Often we look at the event but not the scenario. Consider all the events that
lead up to the near miss is the best way to report.

FIRE EXINGUISHER JIP-506 Date- 06.11.2023

Fire is among the most deadly workplace hazards but is the most preventable of accidents. Due to the serious danger of fire, it is
beneficial to know what to do should a fire erupt. Most fires start small. Fires can usually be contained if they are attacked by
properly trained personnel with the right type and size of fire extinguisher within the first two minutes. Get out immediately if a
fire starts to spread or block exit routes.
Types of Fire Extinguishers
• Water - Filled with water and pressurized air extinguishing agent.
• Carbon Dioxide - Contains a non-flammable gas extinguishing agent.
• Dry Chemical – Multipurpose - contains a dry powder extinguishing agent.
Types of Fires
• CLASS A: Class a fires involve common combustibles such as wood, paper, cloth,
rubber, trash and plastics. They are common in typical commercial and home settings,
but can occur anywhere these types of materials are found.
• CLASS B: Class B fires involve flammable liquids, gases, solvents, oil, gasoline,
paint, lacquers, tars and other synthetic or oil-based products. Class B fires often
spread rapidly and, unless properly secured, can re-flash after the flames are
extinguished.
• CLASS C: Class C fires involve energized electrical equipment, such as wiring, controls, motors, data
processing panels or appliances. They can be caused by a spark, power surge or short circuit and typically occur
in locations that are difficult to reach and see.
• CLASS D: Class D fires involve combustible metals such as magnesium and sodium. Combustible metal fires
are unique industrial hazards which require special dry powder agents found in dry chemical fire extinguishers.
• CLASS K: Class K fires involve combustible cooking media such as oils and grease commonly found in
commercial kitchens. The new cooking media formulations used for commercial food preparation require a
special wet chemical extinguishing agent that is especially suited for extinguishing and suppressing these
extremely hot fires that have the ability to re-flash.
Fire Extinguishers Essentials
• All fire extinguishers should be inspected monthly by a responsible person.
• All fire extinguishers should be serviced by a qualified technician on a yearly basis.
• Fire extinguishers have operating instructions listed in the name plate; be familiar with these.
• Fire extinguishers of the proper type and size for the exposure shall be readily available.
• Be familiar with the location of fire extinguishers on the job site.
• When fueling equipment, always have a fire extinguisher close by. Most
fire extinguishers operate using the P.A.S.S. method:
P – Pull the locking pin.
A – Aim at the base of the flames.
S – Squeeze the handle.
S – Sweep back and forth over the burning area.
Does:
• Do know the locations of extinguishers on the job site.
• Do familiarize yourself with the types of fire and what to use for each.
• Do remember the P.A.S.S. method for fire extinguishers.
• Do identify a safe evacuation path before approaching the fire.
• Do evacuate immediately if you have any doubt about your ability to fight a fire.
Do Not:
• Do not use water agents on grease and oil fires or use dry chemical extinguishers on electrical devices or
anything with metal because it is corrosive.
• Do not aim at the flames of the fire. Do not block access to fire extinguishers.
• Do not stand too close to the fire or even plan to walk on those extinguished areas.
• Do not put extinguishers in areas where fire is most likely to happen making them unreachable.
• Do not leave extinguishers in corridors or on anything that is highly magnetic.
• Do not play with the device or even play with its substances because of the possibility of injuring yourself.

Conserving Water

Use of water should consider ways in order conserve and minimize water

1. Don’t let the tap running while washing your hands.


2. Report leaks in the pipe/hose connections as soon as possible.
3. Taps should be tightly closed at all times.
4. Don’t allow water tanks to overflow.

Conserving Electricity

All personnel are expected to contribute in ways to minimize unnecessary consumption of


electricity.
1. Turn off lights when not in use. Use natural lighting if possible.
2. Turn off AC and other appliances when not in use or while leaving for the sites
3. Regularly clean the surfaces of tube light achieve maximum illumination.
4. If possible always consider low power consuming apparatus/equipment when purchasing

Conserving resources at sites

All personnel are expected to contribute to save company expenditures by:


1. Maximizing the use of welding rods.
2. Taking good care of provided PPE.
3. Use work equipment in appropriately to avoid damage
4. Turn off machineries and other electrical appliances when not in use at sites. (I.e. welding
machines, blowers, fans, and computers.
5. Printing necessary documents in fast print mode to save on toner and inks.
6. Printing less important documents on used paper or printing double sided.
7. Use natural light wherever possible
8. Regular lubrication and maintenance of equipment and tools and regular checks for
leakages.
9. During Hydro testing don’t allow discharge water to drain directly to the ground, discharge
water should be collected into a tank and re-use.
10. Reuse of treated water in areas where low water quality is acceptable (e.g., floor area
wash downs or dust suppression).
