    Hemangiomas are the most common vascular tumors and von Willebrand disease is the most common inherited bleeding disorder. In this article, we report a 21-year-old female who had von Willebrand disease presenting with 1x1 cm lesion of the tongue. The lesion was located at the anterior one-third in midline tongue throughout full-thickness. The patient was administered cryoprecipitate preoperatively. Then the lesion was excised under local anesthesia. Histopathological examination revealed a hemangioma. No complication such as bleeding or infection after the operation was observed.

    We evaluated the factors that have considerable significance on the outcome of open cavity procedures in the treatment of cholesteatoma. The study included 66 patients (43 males, 23 females; mean age 47 years; range 21 to 69 years) who underwent surgery for cholesteatoma. Of these, 56 patients completed at least a five-year follow-up. Mucosal infections and granulation tissue formation occurred in seven patients (10%) in the early postoperative period. Retraction pockets developed in three patients (4%); of these, only one patient required excision because of deep localization. Drum perforations that occurred in two patients (3%) were repaired by myringoplasty. Revision surgery was performed in four patients (7.1%) due to residual cholesteatoma within a five-year follow-up.     Objective: The purpose of this study is to evaluate the relation between poor oral hygiene and tonsillar hypertrophy. Methods: 80 children of ages between 3 and12 who consulted with Pedodontics at Baskent University Istanbul Hospital, were included in this study. For data analysis of dental caries, dft index for primary teeth (d = decayed, f = filled, t = teeth) and DMFT index for permanent teeth (D = decayed, M = Missing due to caries, F = filled, T = Teeth) were used in accordance with WHO (World Health Organization) criteria for oral health surveys. The oral hygiene status was determined by using Simplified Oral Hygiene Index of Greene and Vermillion.     Follicular dendritic cell sarcomas are unusual, and extranodal origin is extremely rare. The English literature contains only eight cases in which this sarcoma has been presented as a tonsillar mass. We report a new case of follicular dendritic cell sarcoma of the tonsil in a 76 year old woman. The patient underwent diagnostic tonsillectomy for a left tonsillar mass, and follicular dendritic cell sarcoma was diagnosed based on histopathological and immunoperoxidase findings. Postoperative radiotherapy was performed. The patient is alive and disease-free at 4 years of follow-up. Without a high index of suspicion, this entity can easily be missed. We believe that follicular dendritic cell sarcoma should be included in the differential diagnosis for any tonsillar mass.

    Objective: To determine the relation between hypoxia and physical parameters in patients who had different levels of severity of obstructive sleep apnea (OSA). Methods: This was a retrospective, cross-sectional study of 259 men who were evaluated with overnight polysomnography. Severity of OSA was graded based on the apnea-hypopnea index (AHI): normal/simple snoring (n=31); mild OSA (n=70); moderate OSA (n=63); severe OSA (n=95). Patients with different severity were divided into subgroups, based on having the lowest or highest values of the total sleep time with oxygen saturation <90% (ST90) or minimum oxygen saturation (min SaO2). Results: Median AHI was 20.4 events/hour. Univariate analysis showed that ST90 was correlated with AHI (r=0.772; p≤0.001) and Epworth sleepiness scale (ESS) (r=0.344; p≤0.001), and min SaO2 was inversely correlated with AHI (r=-0.748; p≤0.001) and ESS (r=-0.319; p≤0.001). Aynı çalışmada bilateral tutulum ise yalnızca bir vakada bildirilmiştir. Bu yazımızda, her iki kulağında uzun yıllardır kulak ağrısı dışında, ek yakınması olmayan, ve bilateral superior semisirküler kanal tutulumu tespit edilen erişkin hasta sunulmaktadır. Bilateral kanal dehisansı nadir görülen bir patolojidir ve bu vakalarda kulak ağrısı beklenmeyen bir semptomdur. Literatür taramamızda benzer bir vakaya rastlanılmamıştır.

    We aimed to clarify the relation between allergic rhinitis and the serum levels of 25-hydroxivitamin D in the adult population. The study group consisted of 86 patients with allergic rhinitis who were diagnosed with the help of history of allergy, positive signs for allergy, blood samples, and positive skin prick tests; while the control group included 43 age- and sex-matched healthy volunteers with negative skin prick tests. The demographic data, medical history, findings in the physical examinations, serum levels of total immunoglobulin E (IgE) and 25-hydroxyvitamin D, and skin prick test results of the groups were noted. A total of 129 patients fulfilling the necessary criteria were enrolled. The median serum 25-hydroxyvitamin D levels in the study group were significantly lower compared to the control group ( P = .014).     Orbital floor blow-out fractures occur due to blunt trauma causing displacement of orbital contents to the maxillary sinus while the orbital rim is intact. Surgical repair of these fractures includes transantral, transorbital, or endoscopic endonasal approaches with or without implant use. We report a 12-year-old boy who presented with diplopia after blunt trauma to the head while playing football. Computed tomography revealed a left isolated orbital blow-out fracture. The patient was treated by a combined endoscopic endonasal-transantral approach and stability was restored with a urethral balloon catheter following endoscopic reduction of the fracture. Healing of the orbital floor was confirmed by an early computed tomography scan. This technique restores eye volume and function without the use of external incisions or implants.

    Objectives: Percutaneous tracheotomy is a relatively new technique which is considered as an alternative to conventional tracheotomy. While both techniques have their own advantages and disadvantages, the data related to long-term follow-up after percutaneous dilational tracheotomy (PDT) and its late complications are limited in the literature.     Mucinous cystadenocarcinoma of the salivary gland is a rare entity that has only recently been described. This lesion was included in the 1991 World Health Organization Classification as "papillary cystadenocarcinoma," and that same year the United States Armed Forces Institute of Pathology classified it as "cystadenocarcinoma with or without a papillary component." Only a small proportion of salivary gland tumors are adenocarcinomas. Most mucinous cystadenocarcinomas affect the major salivary glands, particularly the parotid. These are low-grade malignancies, and most that occur in the minor salivary glands show very little aggressive behavior. These tumors are histologically similar to adenocarcinomas of the gastrointestinal tract. It is often difficult to differentiate them from other neoplasms. The differential diagnosis includes mucoepidermoid carcinoma, acinic cell carcinoma, salivary duct carcinoma, nasal adenocarcinoma, and metastatic carcinoma. This report describes the case of an 80-year-old man who presented with a painless swelling in his right upper lip that had been present for 1 month. Fine-needle aspiration biopsy of the lesion revealed inflammatory cells without an epithelial component. The treatment was wide excision, and there was no recurrence during 6 months of follow-up. The pathological diagnosis was low-grade mucinous cystadenocarcinoma of a minor salivary gland.

    Dysphagia is a common presenting complaint in otolaryngology practice, and there are many causes. Forestier syndrome is a rare cause of dysphagia. It is also known as diffuse idiopathic skeletal hyperostosis (DISH) syndrome or vertebral ankylosing hyperostosis. Forestier syndrome consists of anterolateral perivertebral ligament calcification. It was first described by Forestier and Rotes-Querol in 1950; diagnosis is primarily radiological and the etiology is unknown. In addition to dysphagia Forestier syndrome has been reported to cause laryngeal stridor, dyspnea, snoring and hoarseness. Other important symptoms associated with Forestier syndrome are stiffness and pain in the back, pain related to tendinitis, myelopathy related to core compression associated with the ossification of the posterior longitudinal ligament, and pain related to vertebral complications such as fracture or subluxation. We report six cases of Forestier syndrome as an etiologic factor in dysphagia and present clinical and radiological findings.

    Congenital arhinia is an extremely rare anomaly consisting of an absence of external nasal structures and nasal passages. Fewer than 30 cases have been reported. Patients with a familial absence of the nose have been reported, but the effects of genetic and maternal factors are unknown. Midface hypoplasia may accompany arhinia. Accompanying malformations are thought to be caused by an absent or rudimentary nose. A patient with partial congenital arhinia is presented and the embryology and literature review are discussed.

    Vardenafil, a selective phosphodiesterase 5 (PDE5) inhibitor, may affect nasal patency because of its adverse-effect profile. This double-blind, placebo-controlled, randomized clinical study sought to assess the effect of vardenafil on nasal patency in patients at a university hospital. Nasal patency was assessed using a visual analog score and by measuring the minimum cross-sectional areas (MCAs) and nasal cavity volumes with acoustic rhinometry in 14 subjects before and after administration of vardenafil. Measurements were repeated after administration of a local decongestant spray. There was no statistically significant difference between the nasal cavity volumes, MCA, and visual analog scale (VAS) scores before and after the administration of placebo. However, there was a significant increase in the nasal cavity volumes, MCAs, and VAS scores after application of the local decongestant. A significant correlation was found between MCAs and VAS scores (r = 0.96; p < 0.001). After administration of vardenafil, there was a significant increase in the degree of subjective sense of nasal obstruction as measured by VAS scores. Total nasal volumes showed a significant decrease (p < 0.05). The congestion effect induced by the vardenafil was reversed after application of the local decongestant spray, and a significant increase in cross-sectional areas was noted. In the vardenafil group, a significant increase in MCA, total volume, and VAS scores was observed after application of the local decongestant (p < 0.05). Objective and subjective nasal obstruction after administration of vardenafil was significantly higher in this study than in previously reported studies. The effect of congestion can be reversed by local decongestants. The role of PDE5 inhibitors in nasal physiology merits additional investigation.

    Papillary thyroid carcinoma (PTC) is the most common thyroid malignancy. Radioactive iodine (RAI) therapy is used for remnant ablation following thyroidectomy. Side effects such as dysphagia, xerostomia, and sialoadenitis may occur. We aimed to determine the differences in the parotid and submandibulary glands between healthy patients and patients with PTC who had undergone RAI therapy and have dry mouth symptoms using both shear wave elastography and ultrasonography. We enrolled 30 patients with PTC who had undergone RAI therapy following surgery and 30 healthy controls. In this study, we aimed to evaluated shrinkage of nasal mucosa and cartilage tissue and compared differences in shrinkage after resection, after formalin fixation, and during microscopic examination to understand differences in rate of shrinkage with different tissue types.     Ganglioneuroblastoma is a rare tumor with moderate malignancy, which is composed of mature ganglion cells and seen in sympathetic ganglia and adrenal medulla. The diagnosis is possible after cytological and immunohistochemical studies following a needle biopsy or surgical excision. There is no consensus regarding the need for chemo- or radiotherapy after surgery. In this case report, clinical behavior and diagnosis and treatment of the rare tumor cervical ganglioneuroblastoma were discussed.

    An 81-year-old male patient presented with a painful mass in the tongue that showed a progressive growth within the past two months. Examination revealed a polypoid mass in the midline of the tongue, purple in color, and 1.5x1.5 cm in size. There was no palpable lymph node on neck examination. An excisional biopsy was performed with adequate surgical margins. The histopathological diagnosis was Kaposi's sarcoma. Clinical and radiological evaluations did not show any systemic involvement. An HIV test was negative. He was referred to the medical oncology department. No recurrence was detected during a year follow-up.

    Osteomas are the most common benign tumors of the paranasal sinuses. They may be seen at all ages and show a male preponderance. A 54-year-old male patient presented with a complaint of nasal obstruction. Computed tomography of the paranasal sinuses showed three osteomas, nearly 5 mm in size, in both ethmoid sinuses and the right frontal sinus. Inquiry into the Gardner's syndrome was negative. No surgical treatment was performed at the patient's discretion. To our knowledge, the presence of multiple osteomas in the frontal and ethmoid sinuses has not been reported in the English literature.

    ABSTRACT PURPOSE To compare accuracy and usefulness of MRI combined with diffusion-weighted imaging (DWI) and fine-needle aspiration cytology (FNAC) for the diagnosis of common parotid masses. METHOD AND MATERIALS Twenty five patients (mean age, 61) with parotid masses were evaluated, prospectively. Twenty two patients were assessed with both MRI and FNAC methods. In nineteen of the these 22 patients, conventional MRI combined with DWI. Informed consent was obtained from all patients. Neck MRI was performed using 1.5 T MRI unit. T1, T2 and fat-suppressed T2-weighted images, DWI with ADC maps and fat-suppressed T1-weighted postcontrast images were obtained. MRI and FNAC diagnosis was compared with histopathology. Differences among the ADC values for Warthin tumors, pleomorphic adenomas, and carcinomas were statistically evaluated using by Kruskal-Wallis variance analysis and multiple comparisons test. Youden’s index was used for comparison the diagnostic performance of two methods. RESULTS The tumors comprised 8 Warthin tumors, 8 adenomas (6 pleomorphic adenomas, 2 basal cell adenomas), 5 carcinomas, 2 lipomas, 1 hemangioma, and 1 lympadenopathy. Technically, MRI was successful in 24 of 25 patients (96%) and FNAC was successful in 18 of 23 patients (78 %). T1-weighted postcontrast images did not provide any additional diagnostic information. The mean ADC value for the Warthin tumors was 1.02 ± 0.13 × 10-3 mm2/s, for the pleomorphic adenomas was 1.75 ± 0.40 × 10-3 mm2/s, and for the malignant tumors was 1.31 ± 0.16 × 10-3 mm2/s. The accuracy, sensitivity and specificity of MRI combined with DWI was 96%, 80% and 100%, respectively. The non-diagnostic rate for FNAC was 17%. The accuracy, sensitivity and specificity of FNAC was 94%, 75% and 100%, respectively, when FNAC had adequate material. Youden’s index was 0.80 for MRI and 0.75 for FNAC. CONCLUSION Both non-contrast MRI combined with DWI and FNAC may be valuable modalitity for the diagnosis of parotid tumors. Patient comfort and cost-effectivity should be considered in the method selection. CLINICAL RELEVANCE/APPLICATION N/A

    Ficolins are complement activating peptides that play a role in the initial host defense against infectious pathogens. In the present study, we investigated the relationship between single nucleotide polymorphisms (SNPs) in the ficolin 2 gene (FCN2) and chronic adenotonsillitis in pediatric cases. Case-control study. A total of 101 pediatric patients diagnosed with chronic adenotonsillitis and 100 healthy children were enrolled in the study. Genotypes of FCN2 promoter SNPs - 602 G>A and -4 A>G, and the exonic SNP c.772G>T were determined by light SNP assay after realtime PCR analysis using genomic DNA samples obtained from peripheral blood samples of all participants. Of the 101 chronic tonsillitis patients, 38 were girls and 63 were boys; the mean age was 5.2 ± 2.3 years. The c.772G>T SNP frequency was significantly higher in chronic adenotonsillitis cases compared to the control group (p = 0.00); however, no significant difference was determined at positions -602 G>A or -4 A>G (p > 0.05). The FCN2 c.772G>T genotype appears to be associated with predisposition to chronic adenotonsillitis in the pediatric age group. This nucleotide change is likely to influence the level of gene expression and contribute to the development of disease.

    A case of tortuous internal carotid artery bulging and partially obliterating the piriform sinus, causing a sensation of fullness and abnormal throat, accompanied by choking spells in a 57-year-old patient is presented. Endoscopic examination and computerized tomographic angiography were useful in establishing the diagnosis. This rare anomaly should be recognized in order to prevent possible fatal hemorrhages during further surgical interventions of the piriform sinus.
