Studies suggest that the psychological disorder arising after long-term stress differs from depression and anxiety disorders. In Sweden this stress-related condition is called exhaustion disorder (ED). ED is assigned the ICD-10 diagnostic code F43.8. The Karolinska Exhaustion Disorder Scale (KEDS) has been developed in order to assist in diagnosing patients with ED. KEDS is a short (9 items) questionnaire that addresses the core symptoms of ED: Problems with cognitive functioning, physical and mental stamina, sleeping problems, ability to recover, sensitivity to sensory impressions, reactions to demands, and feelings of irritability and anger. KEDS has been validated in Swedish ED patients and healthy individuals and showed good discriminatory power (A. Besèr et al., Scandinavian Journal of Psychology, 2014, 55, 72–82). In response to the need to identify the health condition associated with long-term stress, we decided to translate KEDS into Danish, and to test it in a group of patients in occupational health clinics in Denmark. The purpose was to investigate to which extent KEDS is able to discriminate between stress-related disorders and other disorders. In the present paper we will present the translation process, the design of the test procedure, as well as preliminary test results.

Home care for the elderly constitutes a large and growing part of the social welfare system. Though, home care work is associated with a number of work environment-related challenges, including an increased risk for injuries, musculoskeletal disorders, high levels of sick leave and staff turnover, as well as stress and high workload, research is sparse. The present study explores local initiatives to improve the work environment initiated by the home care organizations themselves, and asks whether or not these initiatives affected gender equality. A qualitative web survey was sent to Swedish home care organizations, with open questions about change initiatives intended to have a positive effect on the work environment. There is an impressive amount of local work environment-related change initiatives going on. 80 categories of change initiatives were identified in answers from 178 units. However, these change initiatives were seldom evaluated or made accessible to stakeholders outsi...

While workplace wellness services are proactively established to improve well-being and reduce sickness absence, knowledge of reasons for using these services remains sparse. This study investigates which factors determine use of an in-house wellness service at a large organization (the Danish Police) with several departments in different geographical locations. All potential users of the Wellness service ( n = 15,284) were invited to respond to a cross-sectional questionnaire. Of 6060 eligible respondents, 58% had used the service at least once (any use) and 17% had used the service at least three times (frequent users). Two items assessed the frequency of statements of justifications for using or not using the Wellness service. Associations between 32 demographic and psychosocial variables and use of the Wellness service were evaluated with unadjusted bivariate logistic regression analyses. The two primary justifications for using the Wellness service were: to get a blood pressure assessment (37%) and to rehabilitate injury (26%). The two most common justifications for not using the Wellness service were: no perceived need (44%) and already physically active (34%). Of the 32 demographical and psychosocial variables included, 28 were associated with any use and 24 with frequent use. Use of the Wellness service appears to be driven by a complex configuration of factors that resist easy translation into practical advice. Non-participation was accounted for in terms of both positive and negative barriers. Use of the service for purposes of primary prevention and health promotion was, relatively speaking, lagging behind.

Background Few studies have assessed the mental health of principals, or studied associations with both organizational and social work environment factors and occupational balance. The purpose of the present study was therefore to investigate associations between supporting and demanding organizational and social work environment factors, occupational balance and stress symptoms in principals. Methods A total of 4309 surveys (2316 from the first round, 1992 from the second round), representing 2781 Swedish principals who had responded to at least one of two surveys, were included in the present study. The surveys include questions about socio-demographic factors, occupational balance, overtime work, and supporting and demanding organizational and social work environment factors, as well as questions about personal stress and exhaustion. Generalized Estimating Equations (GEE) models were used to specify a repeated measures model with a dichotomous outcome (binary logistic regression)...

The relationship between personality traits and cognitive performance was studied in two groups: men with symptoms and neuropsychological test results compatible with toxic encephalopathy (TE) and demographically similar healthy men (N=57 per group). Personality traits were assessed with the Karolinska Scales of Personality (KSP). The neuropsychological examination included 13 tests covering various functional domains. The TE group displayed elevated scores on all three KSP anxiety scales as well as an elevated impulsiveness score. Furthermore, the TE group had a lower score on the socialization scale than did the referent group. Different relationships between personality dispositions and cognitive functioning emerged in the two groups. Within the referent group the highest correlations were observed between KSP anxiety and socialization scale scores and reaction times measures. This pattern did not appear in the TE group; instead, divergent and a few weak relationships emerged. These relationships involved correlations between the KSP monotony avoidance score and some motor speed scores. By dividing the referent group into low anxiety and high anxiety subgroups on the basis of the multi-component anxiety scale score, it was shown that the test scores in the high anxiety subgroup mostly were indistinguishable from the scores in the TE group. In contrast, the low anxiety group had higher test scores than the TE group in 8 of the 13 tests. In conclusion, the expected relationship between anxiety and cognitive vigilance is absent in TE cases. This indicates that the neuropsychological performance decrement in TE cases is not primarily related to elevated mental distress, but is probably dominated by the effects of organic brain impairment. Thus, in TE cases low neuropsychological test scores should not be regarded as a consequence of emotional symptoms. Furthermore, personality traits may be considered as potential confounders even if traditional matching by demographic criteria has been successfully implemented.

Main Abstract Content: Purpose To investigate the association between conflict frequency and bullying at T1 and T2, moderation of individual conflict management styles and mediation of negative acts. Methodology Data from the Workplace Bullying and Harassment cohort (2006-2008) were analysed cross-sectionally (n=3363) and longitudinally (n=1664). Measures: conflict frequency; three individual conflict management styles (Active-Constructive, Active-Destructive and Passive-Avoiding); the Negative Acts Questionnaire; being a target or perpetrator of bullying. All analyses were adjusted for gender, age, workplace fairness and supervisor’s conflict solving abilities. Results Cross-sectional findings: Higher conflict frequency was significantly associated with more negative acts and a higher risk of being a target or perpetrator. Among those higher in Passive-Avoidant conflict management style, there was a stronger association of conflict frequency with both negative acts and being a target or perpetrator. Longitudinal findings: Higher conflict frequency (T1) was associated with more negative acts (β=0.23, p<.001) and higher risk of being a target (β=0.13, p<.001) or perpetrator (β=0.11, p<.001) (T2). Passive-Avoidant conflict management style enhanced the association between conflicts and being a target of bullying (β=0.18, p<.01). The association between conflicts (T1) and being target or perpetrator (T2) was mediated by negative acts (T1). Limitations The associations were significant only in the cross-sectional analyses and in longitudinal analyses not adjusted for baseline levels of the outcome: Inferences about causality should be cautious. Practical implications Implementation of conflict management strategies should focus on the Active-Constructive style. Value: This is the first study testing 2-year associations between conflicts and bullying with negative acts as a mediator. (Less)
