The prevalence of intellectual disability is an estimated to be 1-4%. Etiological factors such as malnutrition, lack of perinatal care, and exposure to toxic and infectious agents, which are more common in low- and middle-income (LAMI) countries, possibly contribute to a higher prevalence of intellectual disabilities in these countries.1-5 People with intellectual disabilities have a greater risk of experiencing physical and mental health problems and are vulnerable to chronic disease at an earlier age.2,6 While the disease burden due to intellectual disabilities has not been estimated, it is known that 1% of the global burden of diseases is due to lead exposure derived intellectual disability and cardiovascular diseases alone.7 In addition, persons with intellectual disabilities are frequently the most vulnerable group of the society and, on many occasions, are exposed to human rights violations and deprived of minimum services and dignity. Last year was the European Year of People with Disabilities with many activities in Europe in order to focus on this part of the population. The first author participated in the historic event in late 2003 at the graduation of ten physicians, who became the first specialists in intellectual disability medicine in the world after three years of studies under the guidance of professor Heleen Evenhuis, MD, who in the year 2000 became the first professor of intellectual disability at the Erasmus University Medical Center in Rotterdam at the Department of Family Medicine.
Objective. This study aims to generate evidence on intellectual development disorders (IDD) in Mexico. Materials and methods. IDD disease burden will be estimated with a probabilistic model, using population-based surveys. Direct and indirect costs of catastrophic expenses of families with a member with an IDD will be evaluated. Genomic characterization of IDD will include: sequencing participant exomes and performing bioinformatics analyses to identify de novo or inherited variants through trio analysis; identifying genetic variants associated with IDD, and validating randomly selected variants by polymerase chain reaction (PCR) and sequencing or real-time quantitative PCR (qPCR). Delphi surveys will be done on best practices for IDD diagnosis and management. An external evaluation will employ qualitative case studies of two social and labor inclusion programs for people with IDD. Conclusions. The results will constitute scientific evidence for the design, promotion and evaluation of public policies, which are currently absent on IDD.

I have been a clinical psychologist working in what we now call intellectual disability (ID) for the past 25 years. Over this time I have become increasingly concerned about how the concept of ID is defined and understood by others. It seems to me that ID is often seen as a discrete entity that can be easily identified. This, together with an increasing trend towards specifying criteria for services, has led to many people who are in need of a service being refused one on the grounds that they do not have an ID.
Intellectual disability is an interdisciplinary and complex issue. It is a developmental disability that occurred due to hereditary, socioeconomic and environmental reasons. The incidence of intellectual disability took place around 3% of populations irrespective of the socioeconomic status but its prevalence rate was found more among lower class and lower middle class than the upper class. It is important to note that this developmental disability occurred at any time before the age of eighteen years. The Persons with intellectual disability is the most vulnerable group among all other disabilities. It is also most difficult to understand and identify them due to its multiplicity of causes-the known and also the unknown. Therefore it is a challenging task to intervene intellectual disability especially with its treatment and prevention. In India, for the first time Disability got due attention and full coverage in the National Census Survey, 2001 with the crucial authentic data about disability.
