Central line–associated blood stream infections (CLABSI) are among the most common, lethal, and costly health care–associated infections. Recent large collaborative quality improvement efforts have achieved unprecedented and sustained reductions in CLABSI rates and demonstrate that these infections are largely preventable, even for exceedingly ill patients. The broad acceptance that zero CLABSI rates are an achievable goal has motivated and stimulated

From just about all accounts, Americans are driving more than ever, not just to work but to shopping, to school, to soccer practice and band practice, to visit family and friends, and so on. Americans also seem to be complaining more than ever about how much they drive - or, more accurately, how much everyone else drives. However, the available

Following partly the lines as defined by Benvenuti, Slee states that endogenous development indeed emerged as a powerful idea in development thinking, but that a thorough theoretical underpinning is still lacking. The problems as related with the required elaboration of new theoretical models are discussed.

The purpose of this paper is to examine the effects of job preference, unpaid overtime, importance of earnings, and stress in retaining nurses in their employing hospitals and in the profession. Data come from our survey of 1396 nurses employed in three teaching hospitals in Southern Ontario, Canada. Data are analyzed first for all nurses, then separately for full-time, part-time, and casual nurses. Results show that the key to understanding the effects of these variables may be to pay attention to the work status of nurses. With regards to retaining nurses in their hospitals, working in their preferred type of job is important, particularly for part-time nurses. Working unpaid and longer than agreed hours is also a factor for increasing the likelihood of part-time nurses to leave the profession. All nurses are less inclined to leave as the importance of their earnings for the family increases, but it is particularly important for part-time nurses. Stress is an ongoing concern for r...

Since the emergence of computers in schools during the 1980’s, there have been considerable developments by education systems and schools to develop policies and expectations for the use of information and communication technologies (ICT) to enhance learning and teaching. These have not always translated into practice, which has resulted in a focus on the need for improvements in preservice teacher education programs and professional development of practising teachers. This paper starts from the premise that most teacher education have been constrained by using Pedagogical Content Knowledge (PCK) developed by Shulman [1] [2] prior to the dynamic technological changes enabled by the Internet. The authors present the case for the importance of Technological Pedagogical Content Knowledge (TPACK) [3] [4]. Subsequently, the paper provides guidance for auditing the TPACK capabilities of teacher education students through the presentation of an instrument developed, and provides a summary of some of the findings of a study undertaken using that instrument.

To shorten the transplantation waiting time in the United States, federal regulations have been introduced requiring hospitals to develop policies for organ donation after cardiac (or circulatory) death (DCD). The practice of DCD is invoked based on the validity of the University of Pittsburgh Medical Center (UPMC) protocol and relies on the accuracy of the University of Wisconsin (UW) evaluation tool to appropriately identify organ donors. There is little evidence to support the position that the criteria for organ procurement adopted from the UPMC protocol complies with the dead donor rule. A high false-positive rate of the UW evaluation tool can expose many dying patients to unnecessary perimortem interventions because of donation failure. The medications and/or interventions for the sole purpose of maintaining organ viability can have unintended negative consequences on the timing and quality of end-of-life care offered to organ donors. It is essential to address and manage the evolving conflict between optimal end-of-life care and the necessary sacrifices for the procurement of transplantable organs from the terminally ill. The recipients of marginal organs recovered from DCD can also suffer higher mortality and morbidity than recipients of other types of donated organs. Finally, transparent disclosure to the public of the risks involved to both organ donors and recipients may contribute to open societal debate on the ethical acceptability of DCD.

This article analyses the performance of cotton sectors across East, Southern, and West Africa, paying particular attention to the wide diversity of institutional arrangements that they now exhibit. It finds strong support for earlier contentions regarding trade-offs between competition and coordination, and between the roles of public and private sectors. New insights provide concrete and context-specific guidance to policy-makers and stakeholders regarding the key challenges they will face and the risks they will need to manage as they work to improve productivity and ensure an equitable division of benefits within cotton sectors.

Back cover text: Real Social Science presents a new, hands-on approach to social inquiry. The theoretical and methodological ideas behind the book, inspired by Aristotelian phronesis, represent an original perspective within the social sciences, and this volume gives readers for the first time a set of studies exemplifying what applied phronesis looks like in practice. The reflexive analysis of values and power gives new meaning to the impact of research on policy and practice. Real Social Science is a major step forward in a novel and thriving field of research. This book will benefit scholars, researchers, and students who want to make a difference in practice, not just in the academy. Its message will make it essential reading for students and academics across the social sciences.

Occurrence of natural and human induced disasters the world over has overtime escalated in number and magnitude; it appears to have coincided with contemporary climate change events. The devastation in loss of lives and property as well as disruption in national socioeconomic and quality of life indices has been enormous. This is more so in the less advanced countries of the world where capacity to manage disasters is quite limited. The need has arisen therefore for a thorough understanding of the situation in that latter group of countries to resolve this paradox. This is necessary for those countries to be placed in a position for a meaningful and more effective participation in global efforts in the management of urban environmental disasters. Arising from the above, the cardinal aim of this paper is to analyze the trend and pattern as well as adducing factors responsible for building collapse in Nigeria using Lagos State as the study area. This is with a view to offering policy, planning and management implications of this phenomenon in the study area. The study has relied mainly on secondary data presented as Appendix I in a recently published book on Disaster Risk Management in Nigerian Rural and Urban Settlements. The appendix presented a total of 139 reported cases of building collapse in Lagos State over a period of 35 years. These were used for analysis and discussion. The analysis involved the use of table, chart and figures to indicate trends while spatial analysis showing distribution of the collapsed buildings was done using Global Positioning System (GPS) superimposing these as points on the thematic map of Lagos State. Emanating from the study are the following: (1) an observed escalating rate of building collapse in Lagos State over time; (2) a concentration of reported cases in residential land use and in Lagos Island Local Government Area of the State; and (3) identified explanatory factors emanating from professionals in the building industry, developers and policy decision makers. By way of elaboration, the professionals in the building industry go about their duties independently and without an established forum for development process explanation. While the policy makers show lack of political will and influence a lot of development decisions, the system of development as well is fraught with corruption. The public also cannot be exonerated as most developers run foul of the laws governing zoning and development control and not supplying adequate building materials for construction all in a bid to minimize cost of construction. The contractors are also involved. In some situations where materials are adequately supplied, they choose not to use the right mix in order to maximize their returns. This implies that the problem of building collapse in developing countries with particular reference to Nigeria is a function of the negative contribution of the policy decision makers, the professionals and the public. The paper recommends increased political will for more effective policy formulation and implementation; removal of constraints and obstacles relating to coordination and synergy-building among professionals in the building industry; and encouragement of result-oriented public participation in urban environmental disaster management. The expected outcome of all these will increase knowledge towards sustainable urban neighbourhoods especially in developing countries.

This article presents the SOAR framework’s evolution from the fields of strategy, organization development and change, and Appreciative Inquiry (AI) to the discipline of positive organizational scholarship (POS). The ‘positive’ in POS asks scholars and practitioners to view the world through a lens where weaknesses and threats are reframed as strengths and opportunities, and ‘organizational’ refers to positive states of organizing. The premise of POS was both necessary and essential for creating the SOAR framework so that positivity could elevate and extend the capabilities of individuals to perform, change, and transform their teams and organizations....
