Prehospital Emergency Care

Much has been written recently regarding the need for increased utilisation of end-tidal Carbon Dioxide (ETCO2) monitoring in both peri-arrest and post arrest situation in addition to the cardiac arrest scenario.  Whilst it has been demonstrated that ETCO2 is key in terms of ensuring correct endotracheal tube placement in cardiac arrest Sinclair and Sartin (2015) has highlighted that ETCO2 monitoring has significant benefits when further utilised in the wider context of cardiac arrest to potentially improve patient outcomes.  The Use of ETCO2 monitoring allows the clinician a greater insight into the true current condition of the patient when compared with oxygen saturation monitoring (SPO2) as it has been noted that if a patient was to hold their breath, to simulate apnoea, capnography would show a marked reduction in exhaled Carbon dioxide (CO2) whilst SPO2 readings would continue to show ‘normal’ saturation levels for a number of minutes. 

This Paper will further highlight the benefits of ETCO2 use in the pre and out of hospital environment in many more situations aside from just the cardiac arrest situation.  Furthermore, this paper will propose its use in patients where acidosis and carbon retention are key factors such as Chronic Obstructive Airways Disease (COPD) and Diabetic Ketoacidosis (DKA), Asthma and Congestive Heart Failure (CHF) to name but a few. This paper will then go on to provide a robust justification and argument why the monitoring of ETCO2 is necessary and of patient benefit both in the pre and in hospital environment and additionally will also support the clinician in providing the optimal quality of care within the pre hospital environment.

El manejo de la vía aérea en el ambiente prehospitalario supone un gran reto para los profesionales de la salud ya que existen muchos factores que lo dificultan o imposibilitan. El objetivo de esta investigación fue diseñar e imprimir en 3D un dispositivo supraglótico para la intubación orotraqueal a ciegas y evaluar la eficiencia en costos para su uso rutinario por los servicios de atención médica de urgencias. El diseño se hizo basándose en el dispositivo comercial “SALT” y se realizó un estudio económico prospectivo para calcular su costo total de producción. Los resultados demostraron un ahorro del 92.64% en comparación con el costo promedio de dispositivos comerciales similares.

Background: The Scandinavian society of anaesthesiology and inten- sive care medicine task force on pre-hospital airway management was asked to formulate recommendations following standards for trustworthy clinical practice guidelines.
Methods: The literature was systematically reviewed and the grading of recommendations assessment, development and evaluation (GRADE) system was applied to move from evidence to recommendations.
Results: We recommend that all emergency medical service (EMS) pro- viders consider to: apply basic airway manoeuvres and airway adjuncts (good practice recommendation); turn unconscious non-trauma patients into the recovery position when advanced airway management is unavail- able (good practice recommendation); turn unconscious trauma patients to the lateral trauma position while maintaining spinal alignment when advanced airway management is unavailable [strong recommendation, low quality of evidence (QoE)]. We suggest that intermediately trained provi- ders use a supraglottic airway device (SAD) or basic airway manoeuvres on patients in cardiac arrest (weak recommendation, low QoE). We recom- mend that advanced trained providers consider using an SAD in selected indications or as a rescue device after failed endotracheal intubation (ETI) (good practice recommendation). We recommend that ETI should only be performed by advanced trained providers (strong recommendation, low QoE). We suggest that videolaryngoscopy is considered for ETI when direct laryngoscopy fails or is expected to be difficult (weak recommenda- tion, low QoE). We suggest that advanced trained providers apply cricothy- roidotomy in ‘cannot intubate, cannot ventilate’ situations (weak recommendation, low QoE).
Conclusion: This guideline for pre-hospital airway management includes a combination of techniques applied in a stepwise fashion appropriate to patient clinical status and provider training.

Le Service de santé des armées répond aux missions de soutien sanitaire des militaires mais aussi aux besoins de santé du territoire. L’encadrement paramédical des hôpitaux et centres médicaux des armées est réalisé par des cadres de santé diplômés, dont la majorité exerce sous statut militaire. La loyauté en milieu militaire n’est pas implicite, elle est clairement inscrite dans le Code de la défense. C’est un choix personnel fort, un engagement inscrit dès l’origine et constant
pour tout militaire.
Military Health Service, loyalty and management. The French Military Health Service provides healthcare services to members of the armed forces as well as meeting the healthcare requirements of the territory. The paramedical management of hospitals and military medical centres is carried out by qualifi ed healthcare managers, most of whom have military status. Loyalty in the military environment is not implicit, it is clearly written in the French Defence Code. It is a strong personal choice, a commitment made and upheld throughout the career of all members of the armed forces.

The medical management of acute coronary syndrome (ACS) follows the recommendations of international medical societies. The call to the emergency services by the patient triggers a race against the clock in pre-hospital care. It is essential to reduce the duration of the inadequate perfusion of the heart in order to limit its consequences. An effective reperfusion strategy must be planned in advance taking into account the logistical constraints. It is crucial that the general public is educated to recognise the signs of ACS and to call the emergency services immediately (such as (15,112, 991).
La prise en charge médicale du syndrome coronarien aigu (SCA) est cadrée par les recommandations des sociétés savantes internationales. L’appel au centre 15 par le patient déclenche une course contre la montre en préhospitalier. Il faut réduire le délai de mauvaise perfusion du cœur et en limiter les conséquences. Une stratégie de reperfusion efficace doit être anticipée avec des contraintes logistiques. L’éducation des populations est primordiale dans la reconnaissance des signes de SCA, avec un appel précoce au centre 15, 112 ou 911.

At the heart of an emergency, nurses work closely with the patient and centralise the focus of the team work. They provide technical assistance through procedures carried out in an optimal safety framework.

Paramedical students often face difficult cases of injury, death, tension in families of patients and public anxiety during clinical practice. The current study sought to identify the emotional challenges faced by paramedic students during clinical practice and its impact on patient care, the professional and personal life of the students, and to identify the strategies used by paramedic students to cope with their emotional challenges. The researchers used a descriptive, cross-sectional design. A close-ended questionnaire distributed to interns and paramedic students who were studying or graduating from the Emergency Medical Services (EMS) bachelor's degree program was used to collect quantitative data. The study was conducted in Riyadh, Saudi Arabia, between June 1 and July 30, 2020.The study included a total of 137 students. Results showed that emotional challenges had a significant negative impact on their patient care, professional life and personal life (p < 0.00). In addition, emotional challenges include the following coping strategies implemented by paramedic students; developing professional skills, modifying a wrong intervention, deliberately facing stressful conditions, reflecting on stressful situations, communication skills to guide challenging interactions, and controlling the mind and body. This study found that paramedic students face emotional challenges that have a negative effect on the quality of patient care, their professional and personal life and use a variety of strategies and techniques during clinical practice to overcome emotional challenges. Before, during and after EMS practical clinical training, educators of paramedical students carry out more instructional and educational duties to reduce the impact of emotional challenges on paramedical students.

[The occurrence of a death in the out-of-hospital environment is often brutal and the reactions of the deceased's family and friends, unprepared for this event, can be unpredictable. After the technical procédures have been carried out and within a limited period of intervention, the caregivers, and in particular the nurse of the mobile emergency and intensive care service, must provide the family and friends with support enabling them to begin the grieving process. To help the cargivers in this mission, a pedagogical tool remains to be developed].
La survenue d’un décès en extrahospitalier est souvent brutale, et  les réactions de l’entourage non préparé à cet évènement, peuvent être aléatoires . A distance des actes techniques réalisés et dans une durée d’intervention limitée, les soignants et particulièrement l’infirmier des équipes du Service mobile d’urgence et de réanimation, doivent apporter à l’entourage une aide permettant de débuter un processus de deuil. Pour aider les soignants dans cette mission, un outil pédagogique reste à développer.

The history of violence in Colombia (South America), initially presented as an obstacle for the adequate operations of the emergency medical system. But with the development of a modern system, articulated with the different basic and advanced emergency response institutions, altogether with a process of academic structuring coordinated by the Colombian Association of Prehospital Care (ACAPH), has allowed that this unfortunate experience serve as a model for governmental educational policy development for voluntary groups that participate in the scene response and for the medical and paramedical personnel both private and public that perform prehospitalc care.

BACKGROUND: Just over 200 years ago, surgeons were puzzled that the use of the tourniquet to control hemorrhage as common sense during surgery was a relatively recent development. Within the last 20 years, much progress has been made to controlling hemorrhage in the prehospital context. Then, as now, it was surprising that progress on something that appeared obvious had occurred only recently , begging the question how controlling blood loss was common sense in a surgical context, but not for emergency treatment. METHODS: This article is a historical survey of the evolution of the medical understanding of hemorrhage along with technological response. RESULTS: The danger of blood loss had historically been consistently underestimated as physicians looked at other explanations for symptoms of how the human body responded to trauma. As the danger from hemorrhage became apparent, even obvious, responsibility for hemorrhage control was delegated down from the surgeon to the paramedic and eventually to individual service members and civilian bystanders with training to " stop the bleed. " CONCLUSION: Hippocratic medicine assumed that blood diffused centrifugally into periphery through arteries. William Harvey's observation in 1615 that blood ran through a closed circulatory system gradually transformed conventional wisdom about blood loss, leading to the development of the tourniquet about a century later by Jean-Louis Petit, which made amputation of limbs survivable. However , physicians were cautious about their application during the First World War over concerns over effects on patient recovery. Hemorrhage had generally been seen as symptom to be managed until the patient would be seen by a surgeon who would stop the bleeding. More thorough collection and analysis of data related to case histories of soldiers wounded during the Vietnam Conflict transformed how surgeons understood the importance to hemorrhage leading to development of the doctrine of Tactical Combat Casualty Care in the late 1990s. Les gelures sont des lésions locales des extrémités liées à une exposition à des températures en dessous de 0°C. Le risque, en l’absence de prise en charge, est l’amputation des zones gelées qui évoluent vers la gangrène. Le traitement et l’évacuation des patients victimes de gelures constituent une urgence. Les infirmiers sont particulièrement associés aux soins secondaires qui peuvent être longs. Frostbites are cold-freezing injuries of the extremities due to the exposure to temperatures below 0°C. The risk, in the absence of treatment, is the amputation of frostbitten areas which become gangrenous. Victims of frostbite must be treated and evacuated urgently. Nurses play an active role in the secondary long-term treatment.

ÖZET: Ankara 112 Acil Sağlık Hizmetlerine (112-ASH) 2002 Yılında Yapılan Başvuruların Niteliklerine Göre Değerlendirilmesi Sonucunda 112-ASH'ye başvuran 15 yaş ve altında bulunan 876 (% 5,4) vakanın değerlendirilmesinin yapılarak özel bir hizmete olan gereksinim durumu saptanmaya çalışılmıştır. Başvurular 15 yaş altı ve 15 yaş üzeri olarak iki gruba ayrılmış, Ankara 112 ASH tarafından konulan öntanılarına göre oluşturulan dört gözlü ki-kare tablolarından farklı olup-olmadıkları belirlenerek, Odds Ratio'ları hesaplanmış ve geriatrik yaş grubunda daha sık görülme olasılığı olan hastalıklar yönünden aşağıdaki bulgulara erişilmiştir. Vakaların % 58,5'i erkek (E/K=1,4)'tir. Vakaların % 12,2'si Haziran, % 11'i Temmuz ayında başvurmuştur. Özellikle tatil aylarında çocuklarda acil başvuruları anlamlı derece artış göstermektedir. Buna karşılık hafta içi (Salı % 16,8) başvurular daha sıktır. Bu başvuruların da % 26,3'ü 16-20 saatleri, % 24,7'si 12-16 saatleri arasında başvurmaktadır. Tüm pediatrik vakaların % 17'si Çankaya İlçesinden başvururken, acil başvurular içinde % 11,1'i pediatrik yaş grubunda olanların oluşturduğu Gölbaşı İlçesi ağırlık kazanmıştır. Bunu diğer perifer ilçeler izlemektedir. Başvuru yollarına bakıldığında pediatrik vakalar ile diğer yaş grupları arasında anlamlı bir farklılık görülmemektedir (% 9,2'si resmi bildirim). Pediatrik vakaların % 48,3'ü tıbbi nedenlerle başvurmuştur. Buna karşılık yangınlarda pediatrik vakalarla karşılaşma oranı % 22,4'dür. Pediatrik vakaların % 74,8'i olay yerinden hastaneye nakledilirken, % 14,8'i hastaneler arasında nakledilmiş, buna karşılık yerinde müdahale % 6,7'de kalmıştır. Vakaların % 21,2'si SSK Dışkapı, % 18,4'ü Sami Ulus Çocuk Hast., % 13,7'si Numune Hastanesine nakledilmiştir. Pediatrik vakalar daha uzak mesafelerden taşınmışlar (33,5 km/ 23 km), ateşleri daha yüksek (38,4ºC/37,8ºC), Nabızları daha hızlı (92,4 atım/dk-82,7 atım/dk). Diastolik ve sistolik kan basınçları daha düşük bulunmuştur. Ulaşma süreleri farklı olarak bulunmamıştır. Vakaların TRISS skalasına göre aciliyet durumları diğer yaş grupları ile düşük olmakla beraber anlamlı bulunmamıştır(Ort. 1,85). Pediatrik vakalarda da hastane red oranları benzer olmakla birlikte (% 0,5), toplam red vakalarının % 7,1'i pediatrik yaş grubunda bulunmaktadır. Pediatrik vakalarda genel olarak müdahale oranları 1,8 kat düşük olmakla birlikte solunum yolu müdahalesi diğer yaş gruplarına göre 2,1 kat daha çoktur. Pediatrik vakaların olay yerinde ex olarak kabul edilme düzeyleri 2,6 kat daha düşüktür.

Use of prehospital stroke scales may enhance stroke detection and improve treatment rates and delays. Current scales, however, may lack detection accuracy. As such, we examined whether adding coordination (Balance) and diplopia (Eyes) assessments increase the accuracy of the Face-Arms-Speech-Time (FAST) scale in a multisite prospective study of emergency response activations for presumed stroke. This was a prospective study of emergency response activations for presumed stroke in Santa Clara County, California. Emergency medical responders were trained in the Balance-Eyes-Face-Arms-Speech-Time (BEFAST) scale and administered the scale on scene to all patients who were within 6 hours of onset of neurological symptoms. En France, il survient 40 000 morts subites par an. Le taux de survie varie de 7 % à 10 %. L’arrivée de l’« Extracorporeal CardioPulmonary Resuscitation » (ECPR) propose un espoir thérapeutique en cas d’arrêt cardiaque réfractaire. L’ECPR change leur prise en charge, avec une place grandissante en médecine d’urgence et particulièrement en pré-hospitalier afin de réduire les délais de prise en charge. Ceci impose une évolution du rôle des paramédicaux dans cette prise en charge. La généralisation de cette technique nécessite plus de preuve scientifique et des efforts de formation.

Conçu par les médecins et chirurgiens des armées pour répondre à l'afflux des soldats blessés de la Grande Guerre, le triage a été adapté aux urgences collectives et aux catastrophes. Ses critères et ses modalités ont répondu à aux types d'agression et aux spécificités de la filière civile de secours et de soins. Ses buts ont été précisés : prioriser les victimes, constituer des groupes homogènes, gérer les flux d'évacuation, dispenser les gestes de ranimation nécessaires. Acte thérapeutique guidé par une démarche diagnostique, le triage vise à organiser au mieux l'évacuation des victimes vers le plateau technique adapté. La multiplicité des situations d'urgences collectives, leurs volumes et leurs natures, les conditions de sécurité et de soutien logistique, ont diversifié la pratique du triage en milieu militaire ou civil. Des critères adaptés aux agressions traumatiques, toxiques et psychiques correspondent aux besoins de chaque spécialité. L'apparente variabilité des méthodes de triage, militaires ou civiles, françaises ou internationales, repose sur un socle commun dont les codes de couleurs sont une des composantes. Depuis des décennies le triage a répondu aux diverses situations exceptionnelles impliquant de nombreuses victimes en l'absence de fondements scientifiques. Sa mise en oeuvre militaire sur les terrains de conflits et civile lors de « tuerie de masse » balistiques terroristes contribue à lui donner une base scientifique. Sa pratique, longtemps réservée aux seuls médecins est initiée par les secouristes à l'avant selon des critères plus simples. Le résultat recherché est la mise en oeuvre de gestes élémentaires de sauvegarde afin de réduire le nombre de morts ainsi que la morbidité des victimes. Les événements récents ont renforcé la pertinence de former les populations et les professionnels de santé primo-intervenants à la réalisation de gestes simples dans le but de secourir et limiter drastiquement les morts évitables dans les toutes premières minutes. Le dénombrement et la traçabilité des victimes catégorisées puis triées médicalement nécessitent un support, papier ou informatique qui permet d'évaluer la qualité du triage et son degré de fiabilité. Later, it was adapted to civilian mass emergencies and disasters with criteria and modalities designed to address the specificities of both the aggressions and the healthcare and emergency systems. The goals of triage were defined as follows: to give priority to the victims, constitute homogeneous groups, manage the evacuation floods, and give first aid resuscitation. Triage is a medical procedure guided by diagnoses aimed at evacuating victims to the most appropriate medical care centres. The number and nature of emergency situations, as well as the conditions of security and logistical support have diversified triage in military as well as civilian life. The criteria adapted to traumatic, toxic and psychological aggressions correspond to the specific needs of the situations. Nevertheless, the various triage methods all share common rules such as 'the colour code'. For decades, triage has dealt with many exceptional situations involving numerous victims, without any scientific foundation. It has started to acquire a scientific base with experience, by being implemented on military battlegrounds and civilian mass casualty scenes resulting from terrorist attacks. For a long time, only doctors could implement it, today rescue workers on site can implement a simple version of triage. The aim is to implement preventive first aid so as to reduce the fatalities and the morbidity of disaster victims. Recent events have confirmed the importance of training both the general public and professional first responders, to perform simple first aid within the very first minutes after the incidents, so as to rescue victims and drastically reduce avoidable fatalities. Counting and being able to track the victims divided into categories then medically sorted, requires some kind of support, on paper or on a computer, in order to assess the quality and reliability of the triage.

Colourless and odourless, each year carbon monoxide is responsible for several thousand cases of poisoning. Often collective, their symptoms are non specific and can result in serious neurological sequelae or even death, if they are not detected in time. The (pre-) hospital emergency nurse plays an important role in the management of these patients, in terms of assessment, treatment and monitoring as well as the organisation of the admittance of victims, categorisation and medical triage. As part of a team, the nurse ensures that the patient enters an adapted, regulated pathway, with the most serious cases being directed towards a hospital equipped with a hyperbaric medicine facility.

Par le passé familière et communautaire, la mort est aujourd’hui institutionnelle et confiée aux soignants. Grâce à un questionnaire de ressenti sur les décès préhospitaliers, le vécu et le besoin de formation des soignants d’un service mobile d’urgence et de réanimation ont été analysés. Les professionnels ont majoritairement été confrontés à des difficultés face aux décès préhospitaliers. La connaissance des rites religieux est faible, même s’il s’agit d’un point important de la prise en charge. Le désir de formation est important. Le support pédagogique proposé, permettant de répondre aux attentes exprimées, a été reconnu utile et mérite une diffusion plus large.
Reflections on prehospitalisation deaths. In the past, death was a family and community affair, but today it is institutional and entrusted to healthcare personnel. Thanks to a questionnaire on their feelings about prehospitalisation deaths, the experience and training needs for healthcare personnel at a mobile emergency and intensive care service were analysed. The majority of these professionals had been confronted with diffi culties when faced with prehospitalisation deaths. There is little understanding of religious rites, even though this is an important point in dealing with the situation. There is a strong desire for training. The pedagogical support offered in response to the needs expressed was recognised as being useful and should be more widespread.

The organization of a emergency medical services (EMS)

Le plus souvent infirmier sapeur-pompiers, il peut être médecin voir pharmacien. Expert auprès du commandement, ses objectifs sont d’optimiser et d’anticiper la réponse sanitaire des secours aussi bien au quotidien que lors d’évènements impliquant de nombreuses victimes ou des risques technologiques, naturels ou terroristes de plus grande ampleur.

Cet article propose une contribution à la sociologie du travail et à la sociologie des professions. L’auteure s’est livrée à une observation participante directe du quotidien des ambulanciers privés français dont le travail reste un angle mort des travaux sociologiques consacrés aux métiers de la santé. Au-delà des représentations sociales dominantes de l’ambulancier « transporteur », l’enquête révèle les multiples tâches de soins qu’il réalise et notamment, les soins techniques pratiqués dans le cadre de transports sanitaires d’urgence. En 2007, la création du diplôme d’État d’ambulancier, qui prévoit un élargissement de leur mandat sanitaire dans ce domaine, marque un tournant dans l’histoire de ce groupe professionnel. Il constitue une opportunité de satisfaire les revendications d’un segment du groupe qui cherche à conquérir le statut de spécialiste de l’urgence en s’appuyant sur l’exemple des ambulanciers paramedics européens et américains. Toutefois, en dépit de facteurs technologiques et scientifiques favorables, l’aboutissement de ce processus de segmentation reste aujourd’hui incertain au regard des obstacles internes au groupe professionnel, mais aussi des freins politiques et corporatistes qu’il rencontre.

Pelayanan Gawat Darurat di Indonesia telah diatur oleh Permenkes (Peraturan Menteri Kesehatann) no 19 tahun 2016. Didalamnya terdapat penjelasan mengenai SPGDT (Sistem penanggulangan Gawat darurat terpadu).

L’incidence des états de mal épileptiques (EME), difficile à estimer, serait de l’ordre de 6 à 40 nouveaux patients pour 100 000 individus par an. L’EME tonico-clonique généralisé (EMETCG) en est l’expression la plus connue et la plus spectaculaire. Les EME se présentent aussi sous des aspects moins inquiétants (EME partiels moteurs, etc.), ou trompeurs, sans manifestations cliniques convulsives (aphasiques, visuels, etc.). Ils peuvent encore être méconnus (coma lié à un EME larvé après un EMETCG, aspect confusionnel des EME non convulsifs, etc.) ou découverts fortuitement sur des tracés EEG effectués chez des patients comateux. Les recommandations établies en 2008 avaient été élaborées dans une double perspective scientifique et pragmatique avec le soutien d’autres sociétés savantes et le concours de praticiens issus d’horizons très divers. Dix ans après, la Société de réanimation de langue française et la Société française de médecine d’urgence ont décidé d’élaborer de nouvelles recommandations avec l’ambition de répondre le plus possible aux nombreuses questions pratiques que soulèvent les EME. Le praticien exerçant en préhospitalier ou en hospitalier devra se fonder sur une analyse soigneuse des données cliniques (et parfois EEG) visant à préciser la nature réellement épileptique des manifestations cliniques, le type de l’EME et son étiologie. Il devra aussi lutter d’emblée contre les facteurs d’agression cérébrale puis utiliser les différents médicaments antiépileptiques selon des protocoles gradués, personnalisés, modulés selon le type d’EME, son étiologie et en tenant compte de sa gravité potentielle.
Les recommandations avec le niveau de preuve le plus fort ne concernent que l’EMETCG : l’usage des benzodiazépines en première ligne (clonazépam en intraveineux direct ou midazolam en intramusculaire) est recommandé, répété cinq minutes après la première injection (à l’exception du midazolam) en cas de persistance clinique. En cas de persistance cinq minutes après cette seconde injection, il est proposé d’administrer la seconde ligne thérapeutique : valproate de sodium, (fos-)phénytoïne, phénobarbital ou lévétiracétam. La persistance avérée de convulsions 30 minutes après le début de l’administration du traitement de deuxième ligne signe l’EMETCG réfractaire. Il est alors proposé de recourir à un coma thérapeutique au moyen d’un agent anesthésique intraveineux de type midazolam ou propofol. Des recommandations spécifiques à l’enfant et aux autres EME sont aussi énoncées.

La Société française de médecine d’urgence (SFMU), en partenariat avec la Société française de cardiologie (SFC), a décidé d’adapter ces recommandations aux situations rencontrées en médecine d’urgence, que ce soit dans un cadre préhospitalier ou hospitalier. Nous présentons en priorité les recommandations de l’ESC et celles de l’American Heart Association quand elles sont complémentaires. Enfin, nous formulons des recommandations d’experts SFMU, en partenariat avec la SFC, dans le domaine spécifique de la médecine d’urgence en France. Toutes les recommandations sont formulées selon leur classe.
