Paramedic

Much has been written recently regarding the need for increased utilisation of end-tidal Carbon Dioxide (ETCO2) monitoring in both peri-arrest and post arrest situation in addition to the cardiac arrest scenario.  Whilst it has been demonstrated that ETCO2 is key in terms of ensuring correct endotracheal tube placement in cardiac arrest Sinclair and Sartin (2015) has highlighted that ETCO2 monitoring has significant benefits when further utilised in the wider context of cardiac arrest to potentially improve patient outcomes.  The Use of ETCO2 monitoring allows the clinician a greater insight into the true current condition of the patient when compared with oxygen saturation monitoring (SPO2) as it has been noted that if a patient was to hold their breath, to simulate apnoea, capnography would show a marked reduction in exhaled Carbon dioxide (CO2) whilst SPO2 readings would continue to show ‘normal’ saturation levels for a number of minutes. 

This Paper will further highlight the benefits of ETCO2 use in the pre and out of hospital environment in many more situations aside from just the cardiac arrest situation.  Furthermore, this paper will propose its use in patients where acidosis and carbon retention are key factors such as Chronic Obstructive Airways Disease (COPD) and Diabetic Ketoacidosis (DKA), Asthma and Congestive Heart Failure (CHF) to name but a few. This paper will then go on to provide a robust justification and argument why the monitoring of ETCO2 is necessary and of patient benefit both in the pre and in hospital environment and additionally will also support the clinician in providing the optimal quality of care within the pre hospital environment.

À l’image des hommes en général, les professionnels de l’urgence (policiers, ambulanciers, pompiers, militaires), lorsque confrontés à des difficultés de nature psychosociale, hésitent à formuler une demande d’aide vers les services professionnels. Les superviseurs, dans les organisations employant ces professionnels, occupent des fonctions privilégiées pour intervenir auprès de ces personnes afin de les orienter vers les ressources appropriées.

Nous nous sommes demandé si une intervention éducative de groupe portant sur la demande d’aide chez les hommes et sur quelques techniques de base en relation d’aide peut permettre d’outiller des superviseurs de professions d’urgence pour mieux intervenir auprès d’un subordonné. À cette fin, l’intervention Urgence Masculinité a été mise sur pied et évaluée. Cette intervention éducative de groupe, d’une durée d’une journée, a été réalisée à six reprises et a permis de rejoindre un total de 43 superviseurs chez les pompiers, les militaires et les policiers.

L’évaluation indique que l’intervention a permis aux participants de se sentir plus outillés pour identifier les problèmes chez leurs subordonnés, pour entrer en relation d’aide auprès de ces derniers et pour les orienter vers les ressources adéquates. De plus, les participants rapportent majoritairement avoir acquis de nouvelles connaissances et habiletés et se disent satisfaits de l’intervention. L’évaluation indique, par contre, que certains participants présentent toujours des besoins concernant l’identification d’un employé vivant des problèmes psychosociaux. Il a, de plus, été impossible de former un groupe de superviseurs ambulanciers, ce qui constitue une limite importante de ce projet.

Coming up from Napoleonic wars triage develops to a professional technique for sorting masses of wounded soldiers via injury severity and urgency of treatment. Particularly in the 19th century military surgeons extend triage to an order producing system. The first part of the article discusses in how far the political circumstances as well as the nature of modern war promote the establishment of triage. Triage is considered an administrative process, in which the physician has to break with his routinized practises. The second part deals with three contemporary basic versions of triage. In the 1980s there was a broad discussion about ethical problems and the implementation of triage into civil space. Variants like “Sichtung” have evolved in emergency medicine and contrary to disaster medicine triage. The medical competencies of the German “Rettungsassistent” are currently under discussion. One big question is whether and in how far she / he is able and allowed to triage without medical control.

Paramedics regularly respond to emergency calls for suicidal ideation and attempts. These patients are are experiencing psychological pain, referred to as a  psychache. This pain is so persistent and severe, that lethal self-harm seems like the only way to stop the pain. Until now, there were not many options for paramedics to deal with this life-threatening situation. In the last few years a volume of research indicates that the sub-anesthetic administration of ketamine, administered intravenously over 40 minutes, provides rapid alleviation of suicidal ideation and the symptoms of severe depression. This pharmacologically induced effect lasts only days to weeks, but this is enough time to get the patient “over the hump” and to allow for slower acting, longer lasting, evidence-based treatment modalities to be initiated and take hold. This article demonstrates that psychological pain is more lethal than physical pain and makes the case for prehospital treatment of psychache, which because of hospital delays and extended door to treatment times, is better initiated in the field. Ethical issues, the efficacy of ketamine for psychache, unintended consequences, and next steps are discussed. Recommendations for the prehospital administration guidelines and research considerations are included, along with a sample EMS guideline.

Lo scopo di questa tesi è analizzare il metodo attuale di soccorso e le successive complicazioni in un evento creato da un attentato terroristico o in cui siano coinvolte sostanze pericolose (Hazmat-CBRNe). In particolare, sarà seguito un percorso logico su come si dovrebbe effettuare un soccorso utilizzando un approccio olistico e omnicomprensivo, tenendo in considerazione tutte le variabili ed i pericoli. A questo scopo andrò a individuare un approccio progressivo, inizialmente spiegando la terminologia utilizzata e dei concetti fondamentali, in particolare per quello che riguarda tutti gli aspetti di pianificazione e prevenzione prima che un evento accada.  Seguirò con l’analisi di una scena del crimine e come si deve effettuare un soccorso corretto. In seguito analizzerò il fenomeno del terrorismo, poiché è una scena del crimine con nuovi pericoli. Infine tratterò il rischio CBRNe, in quanto, il terrorismo può utilizzare le armi non convenzionali per un attentato. Quest’ultimo capitolo approfondisce anche l’approccio al soccorso con presenza di sostanze pericolose, in quanto fino a indagini concluse, ci si deve relazionare sempre con una probabile scena del crimine, in questo caso anche contaminata. Come vedremo servirà lavorare approcciandosi con un metodo che tenga conto di tutto ciò, anche quando non si ha la certezza o la scena può evolvere. Attraverso dei “case study” e i fatti di cronaca reperiti attraverso un lavoro di “open source intelligence”, andrò ad analizzare le criticità e i punti deboli dell’attuale sistema del soccorso italiano anche per sottolineare la necessità per cui si adottano determinati protocolli e qual è il loro scopo. Vista la forte differenza con l’uso di terminologie e protocolli esteri, eviterò quanto più possibile l’uso e la descrizione del sistema italiano e di scenderne nel dettaglio, soprattutto perché la terminologia cambia in modo troppo radicale negli anni, e cambierà ancora nel futuro prossimo rendendo poco comprensibile questo lavoro tra qualche anno (oltre che distante dai fini di questa tesi). Come scelta tecnica quindi, ho deciso di adottare come linea di base il protocollo standard dell’ICS (Incident Command System) perché stabile nel tempo da decenni, e che ha una terminologia comune e conosciuta da moltissimi stati. Il nostro sistema riflette in buona parte il sistema ICS anche se con terminologie differenti. Utilizzare, la terminologia italiana (Es.: COC, COI, DICOMAC, ROS…) sarebbe di poca utilità a un lettore al di fuori del nostro Stato, e, tenendo anche in considerazione che il nostro sistema e la nostra terminologia sono sconosciuti alla maggior parte dei nostri stessi operatori, ho tentato di tenere il discorso quanto più semplice possibile e che possa essere una base di partenza per approfondire l’argomento.
Nel contesto italiano l’utilizzo della terminologia “anglofona” di questa tesi, ha un impatto limitato e trascurabile, in quanto, chi conosce il sistema sa facilmente riconoscere l’omologo italiano indipendentemente dalla evoluzione del nome e dalla normativa.

Introduction Mental health represents a growing concern for health services in Australia. Background: Cognitive skills and other non-technical skills are key for emergency medical services (EMS); however, there have been a limited number of their markers identified. This study aims to identify markers of cognitive skillssituation awareness and decision making-important for team leaders in EMS. The focus is on any markers of cognitive skills that are associated with quality and safety at EMS work. Method: In-depth semi-structured interviews were conducted with 20 EMS team leaders (10 EMS physician team leaders and 10 paramedic team leaders) and analysed by the structured approach utilising the known framework of cognitive skill elements. Results: The data analysis revealed a set of 50 markers falling into elements of situation awareness (gathering information, interpreting information, anticipating states), elements of decision making (identifying options, implementing decisions, re-evaluating decisions), and an additional cognitive element (maintaining standards). These markers represented cognitive processes, acts, and communications, therefore, some of them can be observable and others rather unobservable. The identified markers were not too specific, applicable mostly in various challenging situations with patients' medical problems and in EMS team leaders working in ground ambulances in urban and rural/remote areas. Conclusion: The findings provide a better understanding of EMS team leaders' cognitive skills, and can aid in the development of assessment and training tools suited particularly to them.

"La risposta CBRNE nel soccorso in caso di spray al Peperoncino nei concerti Italiani" Vedendo la tabella successiva, i fatti di uso di Spray al peperoncino sono continuati drammaticamente dopo gli eventi di Piazza San Carlo a Torino, e questo nonostante le circolari sopra citate ed analizzate, con una frequenza allarmante. Il problema non è lo spray in se, utile per quanto riguarda la prevenzione e il della violenza sulle donne, ma l'uso sistematico per fare rapine o l'uso nei concerti, spesso legato a tale fine, che ha prodotto fortunatamente solo due eventi gravi, quello di Torino appunto e quello di Corinaldo (AN). Il fenomeno sembra collegato alla micro criminalità, in quanto l'uso sistematico è legato ai borseggi da quanto si evince dai fatti di cronaca. Il fatto di essere utilizzato sistematicamente ai concerti, sembra suggerire uno schema pianificato ed è interessante di come si che si sia diffuso così rapidamente l'uso in diverse regioni tra la criminalità. Inoltre il fenomeno sembra limitato alle regioni del Nord e centro Italia.

Aim:
The purpose of this paper is to analyse the legal aspects of the functioning of a paramedic in KSRG units. The paper also defines the areas in which it is advisable to make use of the knowledge and skills of this professional group within the framework of KSRG operations, and offers an assessment of the employment status of this working group in the State Fire Service (PSP).
Introduction:
The number of paramedics in the KSRG has been growing steadily, although the figures differ by voivodeship. The reasons for this state
of affairs cannot be determined unambiguously. Also, one unfavourable trend is noticeable – only one in three paramedics wishes to maintain his or her qualifications obtained during studies.
Conclusions:
An analysis of legal regulations reveals some contradictions that require further clarification in current law. This situation contributes to
the cautious attitude of those who decide on the shape of the KSRG in relation to changes in the field of emergency medical services.
It should be emphasised that the number of so-called isolated medical events, i.e. those in which the KSRG units operate due to the temporary unavailability of the PRM, is constantly increasing. Therefore, the requirements for paramedics providing emergency medical services are growing. We can also observe the tightening of the legal requirements to ensure the safety of paramedics during complicated and dangerous activities and exercises. KSRG units often call for PRM teams to take part in securing their activities and exercises, which is denied (on the grounds of there being no laws governing this area). This necessitates the use of ad-hoc, rather than standardised, solutions and is sometimes associated with additional costs.
Due to the compulsory training of each PSP paramedic in advanced first aid as well as the legally required system of conducting examinations every three years confirming the possession of appropriate qualifications in each JRG, there must be a group of instructors conducting regular professional training.
Also, to maintain a proper level of training for paramedics, it is necessary that a professional development system be created for this group within the PSP units, complete with training programmes, a supervision and certification system, and simulation centers. A particularly problematic issue relates to the purchase of medicines by paramedics for the purposes of providing health services outside of the PRM.
