L’année 2020 a été marquée par la crise sanitaire du COVID 19. La 1ère vague notamment a dégradé les conditions de travail des sages-femmes pratiquant les accouchements à domicile (AAD) tout comme elle l’a fait pour les hôpitaux. Elle a nécessité des aménagements rapides et inédits. Dans certaines régions la pratique a été suspendue pendant quelques semaines du fait de la surcharge de travail dans les SAMU et services d’urgence. Nous avons en parallèle, constaté une augmentation considérable de la demande, à laquelle notre nombre n’a pas permis de répondre. Au-delà du manque d’effectif, nous avons été confronté à une population nouvelle où plus que d’habitude l’évaluation de la motivation des couples et la pertinence d’un AAD était à interroger. Nous avons enfin dû faire face à des familles vivant des conditions très anxiogènes, ce qui nous le savons bien, impacte le bon déroulement des grossesses et des naissances.
Ce sont donc 1503 femmes qui ont été suivies en vue d’un AAD en 2020 soit une augmentation de 16%.
Mais après une année 2020 difficile, c’est aussi avec une note d’espoir qui nous rédigeons ce rapport. Celle de voir qu’enfin un cadre officiel soit donné aux Accouchements Accompagnés à Domicile. Nous avions rencontré la Fédération Française des Réseaux de Santé Périnataux (FFRSP) en novembre 2019 pour leur présenter nos travaux et solliciter une concertation nationale sur la question de l’AAD. Si 2020 ne nous a pas permis d’avancer, début avril 2021 la 1ère réunion de concertation a pu avoir lieu avec toutes les sociétés savantes, ordre et associations professionnelles et usagères, parties prenantes. Les travaux devraient débuter cet automne avec un rendu espéré pour le 1er semestre 2022. Cela est d’autant plus attendu que le nombre de couples se tournant vers des accouchements non assistés augmentent lui aussi considérablement, la plupart du temps en raison de l’absence de sage-femme disponible pour 1 AAD. De bonnes conditions de travail devraient permettre de disposer d’une offre plus en adéquation avec les besoins de la population française.
Les résultats présentés dans le présent rapport nous montrent une nouvelle fois, le besoin et la pertinence de l’offre AAD en France, mais aussi le besoin d’organisation interdisciplinaire notamment autour de la gestion des risques rares mais graves.

The Baptism of illegitimate Children (XVIth-XXIth centuries)
For the Catholic Church, all the children must be baptized, but legitimacy or illegitimacy of birth is considered as a fundamental element of the individual identity. Using various sources from the XVIth-XXIth centuries, both ecclesiastical and lay, this chapter studies the proceedings of baptismal celebrations, sacramental registration, and the godparenthood of illegitimate children in France. We wonder in particular if those children were victims of discriminations. It’s clear that the Catholic Church always considered the registration of illegitimacy as compulsory. In regards to celebrations proceedings, the Church tended towards validation of laymen wishes: in the early modern period and the beginning of the XIXth century, the Church confirmed the existing practices of stigmatization, and, since the end of the XIXth century, it has followed the path towards equality among all the baptized children.

In this chapter, I tell the story of the waxing and waning of the status of the traditional birth attendant in global maternal health policy from the launch of the Safe Motherhood Initiative in 1987 to the present. Once promoted as part of the solution to reducing maternal mortality, the training and integration of TBAs into formal healthcare systems in the global south was deemed a failure and side-lined in the late 1990s in favour of ensuring a skilled attendant at every birth. The TBA question – whether and how to effectively engage TBAs in the effort to reduce maternal mortality globally – has been one of the core policy debates in the history of the global maternal health movement and TBAs continue to be regarded with deep ambivalence by many researchers and policymakers at the national and global levels, not to mention front-line healthcare providers. Yet a growing number of NGOs are re-engaging TBA-like community health actors as important players in the deployment of new technologies and drugs for maternal health. In the final section of this chapter, I track the TBA question to the present, observing how TBAs have been re-engaged in the push for new technological innovations, the rise of evidence-based medicine and advocacy, and the new self-care agenda of the WHO.
