ST. MARTIN MONTESSORI SCHOOL INC., a company duly recognized and existing under
Philippine Laws with office/business address at Tatala 504 Binagonan Rizal, represented herein
by Principal, MR. JEFFERSON F. MILAN, hereinafter referred to as the “PARTY OF THE SECOND
PART”.
WITNESSETH
The parties hereby bind themselves to undertake a Memorandum of Agreement for the purpose
of supporting the PARTY OF THE FIRST PART’S on-the-job training for student-trainees under the
following terms and conditions:
1. The PARTY OF THE FIRST PART shall be responsible for briefing the on-the-job
student-trainees who intend to conduct practicum exposure in the PARTY OF THE
SECOND PART as part of the PARTY OF THE FIRST PART’S curriculum;
2. The PARTY OF THE FIRST PART shall provide the on-the-job student-trainee the basic
orientation on work values, behavior, and discipline to ensure smooth cooperation
with the PARTY OF THE SECOND PART;
3. The PARTY OF THE FIRST PART shall issue an official endorsement vouching for the
well-being of the on-the-job student-trainee which shall be used by the PARTY OF THE
SECOND PART for processing the application of the student-trainee;
4. The PARTY OF THE FIRST PART shall voluntarily withdraw a student-trainee who is
found to misbehave and/or act in defiance to existing standards, rules, and
regulations of the PARTY OF THE SECOND PART and impose necessary Party of the
First Part sanctions to the said student-trainee;
5. The PARTY OF THE SECOND PART will provide a training program that is aligned to
the course of the on-the-job student-trainee and ensures effective conduct of
experiential learning and development of technical skills.
6. The PARTY OF THE SECOND PART will assign a training coordinator who shall
supervise the on-the-job student-trainee and confers with the PARTY OF THE FIRST
PART’s Faculty Coordinator whenever necessary.
7. The PARTY OF THE SECOND PART may or may not grant allowance and/or adequate
insurance to student-trainee in accordance with the PARTY OF THE SECOND PART’S
existing rules and regulations;
8. The PARTY OF THE SECOND PART upon consultation with the PARTY OF THE FIRST
PART, may require qualified students to submit themselves to examinations,
interviews, and file pertinent documents to support their application;
9. The PARTY OF THE SECOND PART will monitor the progress of the on-the-job student-
trainee and evaluates the performance of each student during the training program
and inform the PARTY OF THE FIRST PART as to the progress/status of the trainees in
relation to their training, behavior and regularity of attendance.
10. The PARTY OF THE SECOND PART is not obliged to employ the on-the-job student-
trainee upon completion of the training; however, the party of the second part may
give employment opportunities in its establishment for commendable and qualified
trainees.
11. The student-trainee shall be personally responsible for any and all liabilities arising
from negligence in the performance of his/her duties and functions while under
training;
13. That at the end of the training period, the PARTY OF THE SECOND PART shall
accomplish an Evaluation Form to be provided by the PARTY OF THE FIRST PART and
shall issue a Certificate of Completion/Training stipulating the number of hours
rendered by the student/trainee.
14. The duration of the program shall be equivalent to 600 working hours for College of
Business Administration students unless otherwise agreed upon by the PARTY OF
THE SECOND PART and the PARTY OF THE FIRST PART;
15. Any violation of the foregoing covenants will warrant the cancellation of the
Memorandum of Agreement by the PARTY OF THE SECOND PART within thirty (30)
days upon notice to the PARTY OF THE FIRST PART.
16. This Memorandum of Agreement shall become effective upon signature of both
parties and implementation will begin immediately and shall continue to be valid
hereafter until written notice is given by either party thirty (30) days prior to the date
of intended termination.
IN WITNESS WHEREOF, the parties have signed this Memorandum of Agreement in the City of
Antipolo, this _____ day of __________, 20__.
Before me, a Notary Public in the City of Antipolo, known to me to be the same persons who
executed the foregoing instrument and they acknowledged to me that the same is their free will
and voluntary deed and that of the institutions herein represented.
