The aim of this study is to describe the management of three different complications associated with rhinosinusitis which developed simultaneously in the same patient. A 27-year-old man visited the emergency department with complaints of headache and swelling of the left eye. He had sustained a blunt head injury 1 month earlier. He was diagnosed with preseptal cellulitis, subdural abscess, and Pott’s puffy tumor which was shown to be a result of rhinosinusitis after physical examination and imaging. After surgical and medical treatment, he was followed up without further complications. Complications of acute rhinosinusitis are very uncommon in the antibiotic era. Patients may suffer life-threatening outcomes if complications develop. A prompt multidisciplinary approach must be immediately established by otorhinolaryngologists, ophthalmologists and other specialists of relevance in order to diagnose and treat life-threatening complications.

504 LARENKS VE NAZOFARENKSTE SENKRON MANTLE HÜCRELİ LENFOMA Mantle cell lymphoma is a variant of non-Hodgkin’s lymphoma and is known as a mature peripheral B-cell lymphoid neoplasm. Lymphomas of the larynx and nasopharynx are rare; mantle cell lymphoma is exceedingly rare. In most mantle cell lymphoma cases, the tumoral lymphoid cells express nuclear cyclin D1 due to a t(11;14) chromosomal translocation between an IGH gene and CCND1. This is considered to be an important genetic event in mantle cell lymphoma. There are case reports in the literature on various histologic types of nonHodgkin’s lymphomas which affect the nasopharynx and larynx simultaneously. However, to the best of our knowledge, mantle cell lymphoma involvement of the nasopharynx and larynx simultaneously has not been reported. In this case report, the detailed clinical manifestations and pathological findings of a mantle cell lymphoma patient have been presented along with a literature review of mantle cell lymphoma.

Turkiye Klinikleri J E N T 2004, 4 109 Parotisin benign lenfoepiteliyal kistleri (BLK), ağrısız, kistik ve genellikle iki taraflı tutulum gösteren lezyonlardır. Çocukluk ve yeti şkin dönemde görülebilen BLK’lar iki taraflı oldu ğunda HIV enfeksiyonu bulgusu olarak ortaya çıkabilmektedir. Parotis bezi kistik kitlelerinin ayırıcı tanısında dü şünülmesi gereken bu patolojinin HIV ile birlikteliğinden dolayı erken tanısı önem arz etmektedir. Olgu Sunumu Kırk iki yaşında, erkek hasta sol parotiste a ğrısız kitle sebebiyle klini ğimize başvurdu. Hastanın sol parotis bezindeki a ğrısız kitlesi yaklaşık bir yıldır mevcut olup giderek büyüme göstermekteymi ş. Hastanın öz ve soy geçmi şinde herhangi bir özellik tespit edilmedi. Yapılan muayenesinde, sol parotis bölgesinde 2 x 5 cm, semisolid, a ğrısız, mobil kitle saptandı. Di ğer kulak burun bo ğaz ve sistemik muayenelerinde patoloji saptanmadı. Rutin biyokimya ve hemogram tetkik sonuçları normaldi. Yapılan boyun USG’da sol parotis süperfisyel lobunda kistik, septasyon içeren 3x5x3 cm boyutlarında kitl e izlendi. Parotis bölgesi bilgisayarlı tomografisind e sol parotis süperfisyel lobunda 22 x 47 mm boyutlarında, kistik, multiloküler kitle tespit edildi ( Şekil 1). OLGU SUNUMU / CASE REPORT.

Fibrous dysplasia, which is a localized pathologic disorder of bone, is characterized by the abnormal proliferation of fibrous tissue that is interspersed with normal or immature bone. Involvement of the temporal bone, which is usually unilateral, develops in only 18% of all patients with fibrous dysplasia and occurs more frequently in a monostotic rather than a polyostotic form. To our knowledge, only 3 patients with bilateral temporal bone involvement have been reported in the literature. We present the case of a patient with minimal symptoms who was diagnosed as having bilateral monostotic fibrous dysplasia of the temporal bones and a con-comitant giant cholesteatoma.

ÖZET Amaç: Ekstraözefageal reflü kulakta östaki tüp hasarı yoluyla otitis mediaya neden olmaktadır. Bu çalışmanın amacı ratlarda deneysel reflünün neden olduğu östaki hasarının histopatolojik özelliklerini multifaktöryel olarak değerlendirmektir. Yöntem: Toplam 19 albino rat HCl/Pepsin göre iki gruba ayrıldı. İnfüzyonları uygulayabilmek amacıyla ratların tümünün nazofarenksine polietilen tüpler yerleştirildi. Tüp yoluyla 7 gün boyunca gruplardan birine HCl/Pepsin diğerine Fosfat tamp. %0.9 NaCl uygulandı. Uygulamanın 7.'nci gününde tüm ratlar sakrifiye edildi. ÖZET Amaç: Allantoinin nazal mukozada yara iyileşmesi üzerine olan etkilerini incelemek ve böylece nazal yıkama solüsyonları ve nazal tampon materyalleri için kullanılabilecek yeni bir solüsyon elde etmek. Gereç ve yöntem: Çalışmamızda ağırlıkları 240-350 gram arasında değişen 36 adet beyaz Spraque Dawley yetişkin erkek rat kullanıldı. Sağ nazal kavitelerine girilerek interdental fırça yardımı ile (brushing technique) septum, konka ve lateral nazal duvarlarında mukozal hasar oluşturuldu. Hayvanlar birinci hafta ve üçüncü hafta olmak üzere iki gruba ve bu iki grup da kontrol, distile su ve allantoin olmak üzere üç alt gruba ayrıldı. Bulgular: Histopatolojik skorlama sonuçlarına göre birinci hafta grubunda ödem ve konjesyon parametreleri açısından istatistiksel olarak anlamlı fark saptandı (p=0.05 ve p=0.005). Objective: The definition of sudden sensorineural hearing loss (SSHL) is widely used in the literature. However, there are many cases in clinical practice that do not meet this definition. We aimed to analyze these unclassified patients to demonstrate their responses to systemic steroid therapy and their clinical features. We also aimed to compare them with the patients in the literature that are compatible with the definition. Methods: Thirty five patients with acute onset hearing loss but not meeting SSHL definition criteria in the literature were analyzed. After statistical analysis of audiometric measurements before and after treatment, two groups were recruited. Objective: This study aimed to investigate the prognostic value of systemic immune inflammation index (SII) in sudden idiopathic sensorineural hearing loss (SISNHL). Methods: Eighty-nine patients diagnosed with SISNHL were included in this study. SII was calculated from the complete blood count when the patients were hospitalised. The patients were treated in accordance with our routine SISNHL protocol. Hearing recovery was evaluated in accordance with Siegel’s criteria, and two main groups were created (complete and incomplete recovery groups). The control group consisted of patients who were admitted to our clinic for hearing examination. Hemangiomas are the most common masses of the major salivary glands in parotid glands in childhood particularly. They occur more frequently in the parotid gland and rarely the submandibular gland. Changes in blood flow dynamics within hemangiomas may induce thrombus formation and phleboliths. Cavernous hemangioma may lead to thrombophlebitis in major salivary glands in adults. To our knowledge, cavernous hemangioma of submandibular glands containing phleboliths in childhood has not been described so far in the literature. In this article, we report the first pediatric case of a cavernous hemangioma containing multiple phleboliths in the submandibular gland mimicking submandibular sialolithiasis in a seven-year-old boy.

Objective:To investigate the relationship between development of laryngeal cancer and the presence of polymorphisms of the MnSOD Val16Ala, CAT-262 C < T and GPx1 Pro198Leu genes in a smoking population.Patients and methods:Single nucleotide polymorphisms were determined in DNA from the peripheral blood erythrocytes of 48 heavy smokers (25 patients with laryngeal cancer and 23 cancer-free controls), using polymerase chain reaction.Results:There were no significant differences in age, smoking duration or smoking intensity, comparing the two groups. The homozygous AA genotype of MnSOD Val16Ala was significantly more prevalent in the cancer group than the control group (92vs13 per cent, respectively), while the heterozygous AV genotype of MnSOD Val16Ala was more prevalent in the control group than the cancer group (87vs8 per cent, respectively) (p < 0.001). Intranasal corticosteroids appear to be useful in reducing nasal polypoid lesions and the likelihood of polyp recurrence after surgery. The reasons and frequency of dry tap at myringotomy in pediatric patients with otitis media with effusion Objectives: To determine the frequency and causes of dry tap at myringotomy in pediatric patients myringotomized due to provi- sional diagnosis of otitis media with effusion. Methods: Charts of 435 ears of 256 patients (158 male, 98 female) myringotomized due to otitis media with effusion were reviewed between years 1999-2003. The mean age was 7.1 ± 3.4, ranging between 2-16 years. Preoperative appearance of tympanic mem- brane, tympanogram, acoustic reflex, age, gender, season of oper- ation were all investigated in relation to dry tap at myringotomy.
