    BACKGROUND & AIMS The eosinophilic esophagitis (EoE) endoscopic reference score (EREFS) was developed to analyze adults with EoE and has been successfully applied to a pediatric cohort. The present study compares EREFS scores in younger and older children with EoE. METHODS The 99 patients were divided among three cohorts: 44 Active EoE (EoE-A); 16 EoE-remission (EoE-R), and 39 controls (esophageal dysfunction but < 15 eos/hpf). The cohorts were then subdivided into two groups: younger (≤ 10 years) and older (> 10 years) that were compared based on the composite and the individual components of their EREFS scores. RESULTS EREFS identified active EoE in all children with an AUC of 0.85, in older children with an AUC of 0.90 and in younger children with an AUC of 0.77. Mean EREFS for ≤ 10 years was 1.26 ± 1.19 and 2.71 ± 1.33 for > 10 years (p < 0.01). The three most common findings in our entire EoE-A cohort and in both ages were furrows, edema and exudates. EREFS scores in EoE-A patients had similar specificities (0.88 vs 0.89) and positive predictive values (0.89 vs 0.91) in both ages. CONCLUSIONS The present investigation confirms the utilization of EREFS scores in Pediatric EoE. Furthermore, EREFS can detect EoE and document response to treatment in both younger and older children. However, EREFS scores predicted EoE in the older children with a higher sensitivity (0.89 vs 0.63) and a higher negative predictive value (0.87 vs 0.59) than was seen in the younger cohort.

    Electric shocks and fires represent two different ways for rooftop photovoltaic energy systems to affect public health adversely during installation, operation or removal. Analysis of a U.S. Department of Energy residential application demonstration project suggest that the voltage generated by six modules connected in series could be sufficient to cause ventricular fibrillation and possibly death. Extrapolation of these results to future installations is limited by lack of failure mode data for various designs representing state-of-the-art module and rooftop applications. Two causes of fire include (i) electrical fires from short circuits and (ii) spontaneous combustion from heat build-up in dead air space. Crude upper-boundary estimates for fire-related risk are less than one reportable fire and less than 10−2 deaths per year per 10 MWp cumulative installed capacity. Although the marginal risk to society from these hazards may low, the potential risk to exposed individuals is high. Continued evaluation of safeguard effectiveness merits further consideration.

    This paper presents an applied innovative rear-echelon approach for the treatment of recalcitrant soldiers suffering from post-traumatic stress disorder in the 1982 Lebanon-Israel War. This was used as an alternative to psychiatric hospitalization. Use was made of a short-term, goal-directed, multidisciplinary treatment approach within the confines of a clear military setting. Reintegration into military and civilian life was an integral part of the program. Results indicated that at discharge, all soldiers showed a markedly improved clinical picture. Follow-up results 2.5 years later indicated that the percentage of regulars who returned to combat units doubled, whereas the reservists showed a slight tendency toward deterioration. Follow-up 6 years later showed the continuation of the same tendency. Ramifications of our findings are extensively discussed.

    In this study, the amount of psychological distress reported by soldiers is investigated and measured over time. The soldiers--enlisted and career--were randomly chosen while visiting five different military primary care clinics (for ostensible somatic problems) and administered the General Health Questionnaire (Goldberg and Huxley, 1980). A third of the subjects were given the GHQ a year later. Fifty-two percent showed psychological stress: combat soldiers showed considerably less distress than technical/maintenance and administrative soldiers, and the psychological stress findings remained relatively stable over time. Possible explanations for these findings are discussed.

    The terminal fibrinogen degradation product fragment D1 is known to regulate fibrinogen synthesis but the determinants in the D1 molecule responsible for this property are not known. The effect of fragment D1 on fibrinogen synthesis by the isolated perfused rat liver was studied using the (14C) carbonate method. Addition of 25 mg of fragment D1 to 100 ml of whole blood perfusate resulted in a significant increase in fibrinogen synthesis. When the C-terminal end of the gamma-chain of D1 was further plasmin-degraded to form fragment D3 this fibrinogen-enhancing effect was lost. Our studies suggest that the C-terminal of the gamma-chain of fragment D1 plays a role in regulating fibrinogen synthesis by the isolated rat liver.

    This preliminary study concerned examining a number of psychosocial and medical factors involved in the referral of 207 conscripted soldiers to an Israel Air Force Base Mental Health Clinic. These factors were age, sex, education, source of referral, and timing of referral during service. The nature of medical complaints before and after psychological intervention was also examined. Results suggest an effective medical psychological cooperation can significantly reduce minor psychological problems in a population of drafted soldiers.

    This study investigated the amount of psychological distress reported by 182 soldiers in the Israeli military and compared soldiers' self-ratings of distress with ratings by military physicians of observed distress. Comparisons indicated a low detection rate by physicians with only 13% of self-reported cases of emotional distress identified. This and several related findings were taken to indicate shortcomings in medical care. Possible remedies include additional psychiatric training for military physicians, less frequent rotation of physicians to allow for more stable care, and the initiation of mental health education among soldiers.

    ABSTRACT The relative absence of {bar {nu}}{sub {mu}}-induced charged current events with respect to {nu}{sub {mu}}-induced events at large ({gt}0.45) and large {ital y} ({gt}0.70) enables us to limit the right handed coupling of the weak current. Our data restrict {vert bar}{eta}{vert bar}{sup 2}={vert bar}{ital g}{sub {ital R}}/{ital g}{sub {ital L}}{vert bar}{sup 2}{lt}0.0 015 with 90% C.L. Within the framework of left-right symmetric models, this measurement imposes a limit upon the mixing angle of the left and right handed bosons. Unlike the limits imposed by the {mu}-decay and nuclear {beta}-decay experiments, our limit is valid irrespective of the mass of the right handed neutrino.

    ABSTRACT Measurements have been made of muon-production and penetration-depth probabilities (punchthrough probabilities) for showers produced by 40-, 70-, and 100-GeV negative hadrons incident on an iron-scintillator calorimeter of a maximum depth of 4.8 m of steel. These results are also given for incident 70-GeV identified pions and kaons. Comparisons are made of punchthrough probabilities measured with pulse heights from scintillation counters and drift chambers. The transverse-radius distributions of particles in hadronic cascades are presented as a function of depth in the calorimeter.

    Soldiers with severe Posttraumatic Stress Disorder who did not respond favourably to the front echelon treatment units, were treated in a rear echelon treatment centre, named Combat Fitness Retraining Unit, an alternative to psychiatric hospitalization. The soldiers were given individual clinical interviews; their combat potential scores were measured to tap premorbid factors which may have contributed to the development of symptoms. Analysis indicated that (1) in the clinical interview subjects showed clear interpersonal, school, family, and army adjustment problems prior to the trauma, (2) soldiers with Posttraumatic Stress Disorder treated in the rear unit tended to come from the lower end of the military combat potential level. Ramifications of these findings were carefully elucidated.

    The importance of health responsibility as one aspect of a health-promoting lifestyle has been emphasized repeatedly. Yet there are only a few empirical studies of its role in preventive behavior. We examined the relationship between health responsibility and early-detection practices for breast and cervical cancer. A group of 253 women employees of a large industrial company participated in a cancer screening program subsidized by the employer. They completed questionnaires assessing health responsibility and reported early-detection practices: frequency of breast self-examination and physician breast examinations, frequency of Pap tests, and time lapsed since last Pap test and breast examinations. Health responsibility was a significant independent predictor of breast examination indicators but not of Pap tests. Education level was an important predictor for Pap tests, and age predicted most early-detection practices. The findings lend some support to the role of health responsibility in initiating breast examinations. Better prediction of early-detection practices could be achieved by adding cognitive and emotional components to the existing responsibility scale and by distinguishing between retrospective and prospective responsibility.

    Wheat germ agglutinin (WGA) has been used to define biochemical differences between exudate and resident macrophages in the peritoneal cavity of mice. Western blotting with 125I-WGA identifies a restricted set of glycoproteins in elicited peritoneal macrophages (M phi) (recruited with thioglycollate or periodate) and fully activated M phi (recruited with live Bacille Calmette-Guérin, or heat-killed Corynebacterium parvum); the major species migrates with Mr 89-115 kDa in 10% acrylamide gels. These glycoproteins are not detected in resident peritoneal M phi, nor in thymocytes, neutrophils, lymphocytes and a variety of non-M phi cell lines. The binding of WGA is sensitive to neuraminidase, which exposes binding sites on these proteins for peanut agglutinin and reduces their electrophoretic mobility; these features are typical of O-linked sialo-oligosaccharides. The current focus of EoE pathologic evaluation is the peak eosinophil count (PEC), although histologic features other than eosinophilic inflammation are also commonly observed.     The yield behaviour of poly(methylmethacrylate) (PMMA) has been investigated in tension and compression over a range of testing temperatures and strain-rates. Both tensile and compressive yield stresses were found to increase monotonically with increasing strainrate and decreasing temperatures. Compressive yield stresses were in general found to be more dependent on strain-rate.

    To compare anxiety of 15 hirsute women and of 20 control subjects (student nurses) seen at the Soroka Medical Center, Israel, three hypotheses were considered: patients with hirsutism have significantly higher state anxiety than the control sample; patients with hirsutism show significantly higher trait anxiety than the control sample; the extent of hirsutism generates no significant differences in state or trait anxiety. The two groups were compared on the Hebrew version of the State-Trait Anxiety Inventory. The first two hypotheses were confirmed but no correlation was found between extent of hirsutism and state or trait anxiety. The findings are discussed and implications made with regard to physicians' understanding of the hirsute patient.
