    Rural mental health service resources and intervention approaches to meet the needs of rural elders are in a state of flux. Even within this fluctuation, some relatively stable and beneficial resources are available to rural elders and their families. This article identifies those resources within the context of the growing influence of managed behavioral health care. Systems of managed care are directing the types of mental health services available and their patterns of coordination with other health and social service resources used by rural elders.

    The special and extensive social needs of older women have been seriously neglected by the profession and social work education. This article discusses a five-year MSW-level training project that designed and offered a curriculum on the mental health of older women in graduate social work education. The final results of that pilot program are presented, including a summary of the professional practice rationale, curriculum content, and some implications for the broader social work education arena.

    There is increased domestic and international attention to, and diversified focus upon, the maltreatment of elders. This agenda is helping to further explicate the broad spectrum of what constitutes elder endangerment. The World Health Organization (WHO) currently recognizes elder maltreatment, both at home and within institutional care settings, as a most critical issue within the global scope on mental health issues affecting older adults.Research has helped to characterize the most common features among self-neglect cases and refine the profile(s) of very high-risk, self-neglecting persons. Conditions of self-neglect may be present in substantiated cases in which there are multiple forms of caregiver abuse, neglect, or exploitation.     The special and extensive social needs of older women have been seriously neglected by the profession and social work education. This article discusses a five-year MSW-level training project that designed and offered a curriculum on the mental health of older women in graduate social work education. The final results of that pilot program are presented, including a summary of the professional practice rationale, curriculum content, and some implications for the broader social work education arena.

    This article addresses some of the recent information that has been collected on the issues of alcohol misuse in later stages of the life span. It discusses the risks to both long-term alcoholics who have grown old with their drinking problem and the issues facing those persons for whom the risks of alcohol become apparent in later life. Three cases are included to illustrate the different consequences of alcoholism in the family as it impacts on separate members and generations within the family system. Authors discuss some principles of intervention and prevention that can and should be applied by different mental health and health practitioners that work with older persons and their family groups.

    Patients with end-stage renal disease are being kept alive by dialysis treatment which may be provided either in an institution or in the patient's home. However, medical advancements have outstripped the ability of the treatment team to insure the quality of life for these long-term patients. Although the predominant pattern of care continues to be the hospital- or institution-based unit, an attempt is being made by both patients and physicians to make home dialysis a more viable alternative. There are two major barriers to the large-scale effectiveness of treatment by home dialysis: 1) lack of criteria for the selection of candidates, and 2) lack of a continuing supportive service system. Home care requires a total understanding of the psychosocial functioning of the patient and the family.

    Increasing interest is manifest in the development of alternative models of counseling for older persons. Irrespective of the particular model used, the counseling of aged persons who have emotional and psychologic problems may require a treatment plan for health, mental health, and social services, to resolve the problems and to assess the functional limitations. Health and mental health centers employing both professional and nonprofessional counselors need to determine the value of adding outreach components to their services, and agencies which already have outreach programs may need to determine their relevance and effectiveness. Effective outreach requires a high level of gerontologic knowledge and clinical skills with aged patients. Agencies and clinics can design and implement mechanisms for supporting the elderly as they make use of multiple sources of service.     This article reports on a workshop in which the major focus was a review of the barriers that prevent access to the array of community-based services available to the rural elderly. The demographics of the elderly were outlined and key components of the service system described. Attention was given to access hospital-based care, the closing of hospitals and the reasons for bypassing rural hospitals for those in large towns or cities. Special emphasis also was given to mental health services and their uneven accessibility. A review of the policy implications closed the workshop.
