ObjectiveThis study aimed to analyze the results of hyperbaric oxygen therapy (HBOT) in addition to assisted reproductive technologies in male infertility cases.Material and methodsMedical records of male infertility patients who had HBOT sessions for any reason between January 1, 2015–December 31, 2019, were analyzed retrospectively.ResultsA total of 15 male patients were included. Patients were classified as group 1 [DNA fragmentation (n=5)], group 2 [globozoospermia (n=5)], and group 3 [azoospermia (n=5)]. Round spermatid injection (ROSI), intrauterine insemination (IUI), or intracytoplasmic sperm injection (ICSI) procedures were performed in 10 of the 15 patients. A total of 31 embryos were obtained out of which 19 (61%) were transferred. While a total of 15 embryos were obtained in the globozoospermic group, which was the highest number, 10 were obtained from the azoospermic patients, and 6 from the DNA fragmentation patients (p=0.515). A total of 3 (30%) healthy pregnancies were obtained, overall. The mean sperm count of the patients (n=9) before HBOT was 8.4±11.1 mil/mL, while it was found to be 15.7±15.0 mil/mL after HBOT (azoospermic patients were not included). The TESE results were analyzed for azoospermic patients; positive changes were observed in 4 patients (80%). On the other hand, there was an improvement in 50% of group 1 according to sperm DNA fragmentation after HBOT (p=0.500).ConclusionWhile a total of 15 embryos were obtained in the globozoospermic group, which was the highest number, 10 were obtained from the azoospermic patients.. Further studies should be conducted on HBOT and male infertility.

Recently laparoscopic radical prostatectomy (LRP) is a minimally invasive surgical option for prostate cancer (PCa) treatment in the lack of robot. To eliminate numbers of trocars and to modify surgical technique can make the LRP procedure easier. We aimed to introduce our novel approach on LRP by using just only 3 trocars and to compare conventional extraperitoneal LRP (eLRP) with 3 trocars eLRP. METHODS: Of the 223 PCa patients undergone eLRP were divided into 2 groups as Group 1 (n=69) consisted of conventional eLRP, Group 2 (n=154) consisted of consecutive 3 trocars eLRP. Demographic, operative, postoperative, and short-term follow-up data including functional results were analysed. RESULTS: Mean follow-up was 10.9±5.1 months. Preoperative and demographic parameters were comparable between the groups. Mean operative time, intraoperative used carbon dioxide (CO2) gas, and hospital stay were significantly shorter in Group 2 (for all parameters p<0.001). Haemoglobin decrease was also less in Group 2 without statistical significance. Oncologic and functional results were similar. There was no major complication in Group 2. CONCLUSIONS: According to our results 3 trocars eLRP can be performed safely and effectively in experienced hands. Haemorrhage, operative time, CO2 usage, and hospital stay could be shortened with our novel approach.

Hydatid disease is endemic in many parts of the world. The location is mostly hepatic (75%) and pulmonary (15%), and only 10% of the cysts occur in the rest of the body. It may develop in almost any part of the body. Although hydatidosis can be present in all parts of the human body, urinary tract involvement develops in only 2–4% of all cases, and isolated renal cysts are extremely rare. There are no specific clinical symptoms or signs that will reliably confirm the diagnosis of renal echinococcosis. In addition, there is no laboratory finding that is pathognomonic for hydatid disease except for hydatiduria. Routine blood tests are generally normal except for eosinophilia which is found in only 50% of the cases. Radiological studies have a more important place in the preoperative diagnosis of renal hydatic disease. However, there is no specific sign on plain radiography or intravenous urography, and ultrasound or computed tomography cannot always show a hydatidosis as a specific le...

A 20-year-old man presented with chronic left scrotal pain and discomfort, especially in the upright position. On physical examination the testes and epididymes were normal on palpation but were remarkable for a grade III left varicocele. Multiple solid “bead string” lesions were also palpated in the varicocele veins. The lesions were opaque on x-ray (fig. 1). Gray scale and color Doppler sonography using a 5 MHz. linear phased array transducer revealed a 6.5 mm. left spermatic vein with multiple calcifications and acoustic shadowing. Roentgenographic examination and ultrasonography of the kidneys, ureters and bladder were normal. Metabolic evaluation included serum electrolyte, creatinine, calcium, phosphorus and uric acid tests. Hemogram, urinalysis and urine culture were performed, and 24-hour urine samples were collected and analyzed for calcium, uric acid, creatinine, oxalate, sodium and citrate. In addition, urinary pH and volume were measured and were found to be normal. History, clinical examination and colony counts were also obtained to exclude genitourinary infections. Exploration of the left spermatic cord and testis showed a normal vas and testis but mobile solid lesions were palpated in the varicocele veins (fig. 2). The varicocele vein segment was then excised and the solid lesions appeared as stones. Stone analysis demonstrated whitlockite calcium phosphate. However, histolopathological examination of the varicocele veins revealed thrombosed large vessels with perivascular fibrosis and interstitial edema.

A 52-year-old man presented with obstructive urinary symptoms, including weakness of urinary stream, hesitancy, intermittency and the sensation of incomplete bladder emptying many years in duration. Medical history revealed no urogenital trauma or urethral instrumentation. However, a urogenital infection, probably urethritis, due to prostatitis 5 years ago had not been treated with any drugs. Urinalysis and serum prostate specific antigen were normal. International Prostate Symptom Score was 23. The patient had severe symptoms. Maximum flow rate was 9 ml. per second. On cystoscopy a verumontanum attachment causing urethral obstruction was observed at 12 o’clock (see figure). A resectoscope was inserted into the urethra and the attachment was separated using loop without resection. Benign prostatic obstruction was evident but the urethral lumen was not fully obstructed. Transurethral resection was performed and a urethral catheter was placed.

Although prostatic calculi are relatively common, the etiology of these stones is not clear. We report a case with multiple prostatic calculi causing urinary obstruction and a concomitant bladder stone. We treated these stones endoscopically. We found a lot of different sized stones endoscopically, some protruding into the urethra, some filling different cavities on the prostate. So these cavities suggest prostatic calculi may occur related to intraprostatic reflux in the congenital or acquired diverticulum of the prostatic tissue. In addition, the stone composition of the bladder and prostatic stones was the same. All of these results show that the origin of bladder and prostatic stones can be the same. This case also supports a theory of intraprostatic reflux and urine stasis.

The effect of anatomical factors such as lower infundibulopelvic angle (LIPA), lower infundibulum diameter (LID) and inferior calyceal length (ICL) on renal stone formation was investigated. These parameters were measured from noncalculous kidneys of 40 healthy kidney donors. The same parameters from 119 patients with single, unilateral, nonobstructive lower calyceal stone were also measured. LID and ICL were significantly higher in calculous kidneys when compared to the control group. On the other hand, the difference between the LIPA of the two groups was not significant. It is concluded that LID and ICL could be good indices in determining lower calyceal stone formation.
