Arteriovenous fistulas (AVFs) are common form of chronic hemodialysis access. Pain during areteriovenous fistula (AVF) cannulation remains a common problem in hemodialysis patients. This study was undertaken to assess the effect of cold application on pre procedure pain due to arteriovenous fistula puncture in hemodialysis patients. A simple random sampling technique was used for selecting sample of 60 patients (30 each in experimental and control groups) who were undergoing hemodialysis by using AVF. Hemodialysis patients, who met the inclusion criteria, were randomly assigned to experimental and control groups. The tools used were sociodemographic data and subjective pain scoring was done by using numerical rating scale. Descriptive statistics and inferential statistics such as paired t-tests, Independent t-test, Chi square were used. The study finding reveals that the subjective pain scores were found to be significantly (P = 0.01) reduced within the experimental group by cold application. This study highlights the need for adopting the alternative methods for reducing the pain at AV fistula cannulation site in health care settings.

Effectiveness of Cryotherapy on Pain Intensity at Puncture Sites of Arteriovenous Fistula on Patients with Hemodialysis in Dialysis Unit of a Tertiary Care Hospital, Ludhiana, PunjabPatients with chronic renal failure need treatments such as kidney transplantation, hemodialysis or peritoneal dialysis. The most common treatment for the disease is hemodialysis. Pain during arteriovenous fistula (AVF) cannulation remains a common problem in hemodialysis (HD) patients that leads to noncompliance to lifetime maintenance HD. The recommended hierarchy of management should consist of non-pharmacological treatment as first, then drugs and if necessary, surgery. From a review conducted, cryotherapy is the application of any material that removes heat from the body resulting in decreased tissue temperature. It decreases tissue blood flow by causing vasoconstriction and reduces tissue metabolism and muscle spasm. Total 20 patients selected by purposive sampling technique were studied on 3 successive hemodialysis sessions in dialysis unit of a tertiary care hospital, Ludhiana. On initial observation, baseline pre-and post-puncture of arteriovenous fistula vital signs parameters and post-puncture numerical pain rating scale were measured. On second observation, cryotherapy was given to the patients on the web between thumb and index finger of contralateral arm with measurement of pre-and post-intervention vital signs parameters and post-puncture of arteriovenous fistula numerical pain rating scale. On third observation, cryotherapy was given around arteriovenous fistula site with measurement of pre-and post-intervention vital signs parameters and post-puncture of arteriovenous fistula numerical pain rating scale. The findings revealed that in baseline observation, Mean score was 7.4which decreased to 5.3 in second observation and further decreased to 4.4 in third observation and Standard Deviation was 0.69 in baseline observation which increased to 0.75 in second observation and further increased to 0.86 in third observation with t value of 0.0009 at p<0.05.The study concluded that cryotherapy is effective on arteriovenous puncture pain. Hence, cryotherapy is recommended for arteriovenous fistula puncture pain.

The Effect of Cryotherapy at the Hoku Point on the Severity of Pain of Fistula Catheterization in Hemodialysis PatientsBackground & Objective: Pain is an unpleasant feeling that influences the quality of patients’ life. Arteriovenous fistula catheterization pain can be reduced in hemodialysis patients with nonmedicinal and noninvasive methods of pain relief. This study aimed to investigate the efficacy of cryotherapy at the Hoku point on the severity of the pain of catheterization in hemodialysis patients. Materials & Methods: This quasi experimental study was conducted on 40 hemodialysis patients selected by census sampling method. In the first session, intensity of arterial and venous puncture pain, without applying any intervention, was recorded using the visual analogue scale (VAS). In the second and third sessions before catheterization, ice cubes were placed on the Hoku point for 10 minutes and the intensity of pain of arterial and venous fistula was immediately measured. Data were analyzed using SPSS 19 by independent t-test. Results: The mean score of pain in the first session (arterial: 5.97±2.44, venous: 6.00±2.62) was compared with the mean score of pain in the second (arterial: 2.80±1.92, venous: 2.72±2.09) and third sessions (arterial: 2.42±1.72, venous: 2.50±1.93). The results of repeated measure analysis of variance showed that the mean score of pain significantly reduced in the second and third sessions compared to the first session (P<0.05). Conclusion: Cryotherapy at the Hoku point reduces the severity of pain of arteriovenous fistula catheterization and, thus, can be suggested as an effective noninvasive method for puncture pain relief in hemodialysis patients.
