Suicide is a leading cause of deaths in the United States. Although the emergency department (ED) is an opportune setting for initiating suicide prevention efforts, ED-initiated suicide prevention interventions remain underdeveloped. To determine whether an ED-initiated intervention reduces subsequent suicidal behavior. This multicenter study of 8 EDs in the United States enrolled adults with a recent suicide attempt or ideation and was composed of 3 sequential phases: (1) a treatment as usual (TAU) phase from August 2010 to December 2011, (2) a universal screening (screening) phase from September 2011 to December 2012, and (3) a universal screening plus intervention (intervention) phase from July 2012 to November 2013. In Rhode Island and across the country, there is increasing focus on food as a point of convergence and key lever to improve health and wellness, build robust communities,
strengthen local economies, maintain open space, and protect the environment.

Building on this momentum, the Rhode Island Food Council Design Committee contracted Karp Resources to produce this Rhode Island Food Assessment: a comprehensive resource for
stakeholders throughout the state who are working to strengthen the Rhode Island food system, which includes all of the organizations, businesses, individuals, relationships, processes, and infrastructure that move food from Rhode Island farms and fisheries to consumers in all Rhode Island communities. The Food Assessment describes the current
and potential future capacity of the Rhode Island food system to increase community food security in Rhode Island. This means
strengthening the food system, so that all Rhode Islanders, especially those with limited resources, have access to healthy, safe, nutritious, affordable, culturally acceptable, and increasingly locally produced food.

Nearly 8,000 people reside in Rhode Island's (RI's) 84 nursing homes at any single point in time. Many of these people are highly vulnerable because of illness or frailty. In this article, we describe the reasons that RI residents seek care from nursing homes, the associated costs (with a focus on Medicare and Medicaid payment), and different ways to assess nursing home quality. We also describe the home- and community-based services that can help people remain in the community. A resource list provides additional information for those seeking to better understand RI nursing homes and long-term care supports and services.

Creating a habitat classification and mapping system for marine and coastal ecosystems is a daunting challenge due to the complex array of habitats that shift on various spatial and temporal scales. To meet this challenge, several countries have, or are developing, national classification systems and mapping protocols for marine habitats. To be effectively applied by scientists and managers it is essential that classification systems be comprehensive and incorporate pertinent physical, geological, biological, and anthropogenic habitat characteristics. Current systems tend to provide over-simplified conceptual structures that do not capture biological habitat complexity, marginalize anthropogenic features, and remain largely untested at finer scales. We propose a multi-scale hierarchical framework with a particular focus on finer scale habitat classification levels and conceptual schematics to guide habitat studies and management decisions. A case study using published data is included to compare the proposed framework with existing schemes. The example demonstrates how the proposed framework’s inclusion of user-defined variables, a combined top-down and bottom-up approach, and multi-scale hierarchical organization can facilitate examination of marine habitats and inform management decisions.

The definition of "theocracy" has perplexed scholars for many centuries.  Some argue that theocracy exists only when religious leaders are also the actual, official political leaders and, as such, impose their particular theological views on the populace at large.  This essay argues that effective control of a polity by religious leaders and their theology is dispositive, regardless of the governmental organization that exists on paper.  For example, Roger Williams pointed out that in many theocracies (including that of seventeenth-century Massachusetts Bay) "the Magistrate is but the Ministers Cane through which the Clergy speaks . . . ."  "The Bloody Tenent Yet More Bloody" (London, 1652), 131.  The present essay was incorporated, with some additions and revisions, into Appendix B of Alan E. Note:  The sentence and citation on page 5, n.1, in this article referring to John M. Barry's book on Roger Williams should be qualified by Barry's remarks elsewhere in his book:  "Technically, the Bay was not a theocracy. Consistent with the admonition to render unto Caesar the things that were Caesar’s, and to God those which were God’s, the plantation prohibited a minister or church officer from simultaneously holding a government office. (For this reason some historians have argued that Puritan Massachusetts actually advanced the concept of separation of church and state.) Submerged shell midden sites and natural shell deposits can have similar characteristics and can be difficult to distinguish archaeologically. We excavated two test units from a large (at least 35 m x 70 m) submerged shell mound in Fort Neck Cove in southern Rhode Island to assess whether it was natural or cultural in origin. This mound had been recognized as a potential archaeological feature as early as the 1970s. Excavation, radiocarbon dating, and subsequent laboratory analysis of excavated materials suggest that the mound was a natural oyster reef rather than a submerged archaeological site. No artifacts were found; there was no clear evidence for human modification of any shells; small species that would not have been targeted as food were present; and δ13C values of oyster shells from the mound were consistent with freshwater input into their growth environment, suggesting that they grew in an estuarine environment that did not exist prior to the inundation of the ponds. The stratigraphically oldest radiocarbon date we could obtain (430–190 cal BP, 2σ range), from 70 cm below the pond floor, placed deposition of shells at least 3,000 years after the inundation of the pond. The excavation methods that we used and the process of testing, irrespective of whether the feature is cultural, are valuable contributions to the methodological literature on submerged site archaeology and help provide insight for other researchers working to discern natural from cultural shell midden sites.

Developmental toxicity of two different classes of commercial formulations of insecticides was studied by in ovo treatment of fertilized Rhode Island Red eggs. The first one was a combination of chlorpyrifos and cypermethrin and the second one was spinosad, a fermentation product of soil bacterium, Actinomycetes. In this study, the combination pesticide and spinosad of different concentrations were administered as a single dose in ovo in volumes of 50 μL per each egg on day “0” of incubation. Embryonic growth and development, morphological and skeletal malformations, and hatchability were assessed. The combination insecticide induced explicit alterations in the embryonic growth and development and resulted in malformations particularly to the axial and appendicular skeletal structures, whereas the changes were trivial in case of the spinosad exposure. © 2010 Wiley Periodicals, Inc. Seasonal and pandemic influenza clinically remain remarkably similar in long-term care populations. Clinicians cannot distinguish clinical influenza, whether seasonal or pandemic H1N1, from other respiratory viral infections in individual patients. Part of the difficulty in the clinical diagnosis relates to fewer clinical features that might help with diagnostic differentiation, such as fever. However, the nursing home provides an epidemiologic context that can prove helpful to clinicians who inquire--by considering illness patterns among others in the facility, both staff and residents. This can lead to more timely diagnosis and treatment in the resident, and prophylaxis--an opportunity to protect the remaining residents and staff. Check out the treatment guidelines posted on the CDC website to be sure to select the best agents, because antiviral resistance patterns have been rapidly changing.

Health reform strives to be patient-centered but often emphasizes institutional and financial well-being at the expense of patient responsiveness. Rhode Island is a pioneer, with innovative youth engagement programs in health care. The Youth Advisory Board of the Adolescent Patient-Centered Medical Home (PCMH) Initiative at Brown Family Medicine has brought together adolescents to gather feedback about participants' preferences for their health care and bring that feedback to health care providers. The Adolescent Leadership Council (TALC) of Hasbro Children's Hospital is comprised of adolescents with chronic medical illnesses and serves as an advisory group. The Rhode Island Department of Health's Office of Special Needs offers Dare to Dream, a youth leadership development program, a youth advisory council and a healthy lifestyles program. These youth engagement programs allow youth to help shape the health care system to meet their needs and contribute to youth empowerm...

This book is about Roger Williams (ca. 1603-83), who was banished from the colony of Massachusetts Bay for advocating freedom of conscience, separation of church and state, Native American rights, and related matters.  He founded the town of Providence in what became Rhode Island on the basis of full liberty of conscience and complete separation of religion and government.  For additional information, see the uploaded PDF and links.
