The response of a stainless steel cylindrical vessel with asymmetric internal blast loading is investigated by experiments, computations, and analysis. Previous research has focused on the response of cylindrical vessels for a centrally located charge. The vessel`s geometry consists of a thin shell cylinder with 2:1 elliptical ends. The high explosive charge is located on the vessel`s axis of symmetry and is axially offset from its geometric center. All explosives are spherical in shape and centrally initiated inside the vessel. The vessel holds the high explosive in air at ambient temperature and pressure. Two-dimensional, Eulerian finite difference calculations from a hydrodynamic code (MESA-2D) are used to study the blast phenomenon and pressure loading on the vessel walls. The loading pulse is compared to data obtained from four experiments where several pressure transducers are positioned along the vessel`s circumference. The calculated two-dimensional loading is applied to a three-dimensional explicit finite element model (DYNA-3D) to predict the structural response of the vessel. The vessel`s structural response from the experimental wave interaction is recorded using strain gages, positioned along its circumference, and then compared with the finite element calculations. These results (experimental and computational) are then compared with analytical predictions and prove to correlate well at distances away from the ends. Experimental and computational results compare well at all other locations.

BACKGROUND: The need of mechanical ventilation among patients with acute neurological diseases is considered a poor prognostic sign.AIM: To determine the mortality and functional recovery of neurological patients requiring mechanical ventilation.PATIENTS AND METHODS: Prospective study of 77 patients (42 men, age 54+/-19 years, with 11+/-4 points of Glasgow coma scale (GCS), 61% with cerebrovascular disease), that were admitted to the intensive care unit with neurological disease and that required mechanical ventilation. Functional recovery was assessed at 18 months with Glasgow outcome scale (GOS) and Barthel index.RESULTS: Thirty percent of patients died during follow up. Among surviving patients, 47% had a good recovery or moderate disability, and 74% had a Barthel index equal to or over 70. Arterial hypertension, age over 70 and mechanical ventilation longer than 6 days were associated with bad functional prognosis.CONCLUSIONS: Neurological patients requiring mechanical ventilation had a lower mortality than previously reported, and half of the survivors have an independent life. This study supports intensive care management in this group of patients.

This project has two objetives. On one hand, it investigates the business sustainability of a distributor located in Provincia de Buenos Aires, between different regulatory alternatives respect to the transfer from investments to rates. And, on the other hand, it aims to the development of another alternative in order to solve investment problems in the electric network of Provincia de Buenos Aires, having several proposals as a model, and the FREBA-FITBA one in particular. To reach these objectives, there is a description of the principle investment problems in Provincia de Buenos Aires, a detailed analysis of FREBA-FITBA`s way to take decisions, a simulation of the electric distributor`s profitability for several investment alternatives, and, finally, a sustainability evaluation under different assumptions about pass-through or recovery existence in the tariff revisions.

La sepsis grave y el shock séptico son entidades catastróficas asociadas a una elevada morbimortalidad. La liberación desproporcionada de mediadores proinflamatorios y antiinflamatorios, ocasionada por el insulto séptico, es la promotora de la disfunción orgánica múltiple. Las técnicas convencionales de hemodiálisis, hemofiltración o una combinación de ambas pueden ser una buena opción para suplir el deterioro de la función renal en pacientes críticos mediante la eliminación de compuestos nitrogenados (moléculas pequeñas). Sin embargo, esta dosis renal resulta insuficiente para la depuración de mediadores inflamatorios (moléculas medianas) y, por tanto, de escaso aporte en la estabilización cardiovascular de pacientes con shock séptico. En este escenario, una mayor dosis de ultrafiltración (>50 ml/kg/h) o dosis séptica podría ser necesaria. En este artículo de revisión se analiza la fundamentación fisiopatológica y clínica para el empleo de la hemofiltración de alto volumen en pacientes con shock séptico.Severe sepsis and septic shock are conditions associated with high morbidity and mortality. The disproportionate release of pro-inflammatory and anti-inflammatory mediators caused by the septic insult is the promoter of multiple organ dysfunction. Conventional hemodialysis, hemofiltration or a combination of both can be a good option to replace the deteriorating renal function in critically ill patients by the removal of nitrogen compounds (small molecules). However, this “renal dose” is insufficient for the removal of inflammatory mediators (medium molecules), and therefore contributes little to the cardiovascular stabilization of patients with septic shock. In this setting, a higher dose of ultrafiltration (> 50 ml/kg/h) or “septic dose” may be needed. In this review article, we have analyzed the clinical and pathophysiological rationale for the use of high volume hemofiltration in patients with septic shock.
